Summary: The use of eculizumab in kidney transplantation has reduced the incidence of relapsing aHUS, but the management of aHUS transplant candidates with anti-CFH antibodies is still debated. We present a case of a successful kidney transplant in a patient with elevated anti-CFH antibodies, using eculizumab and obinutuzumab. The patient had a good outcome with undetectable anti-CFH antibodies, B-cell depletion, and no signs of aHUS activity.

Summary: Lung nodules or masses can occur after lung transplant and can be caused by various malignant or benign conditions, including opportunistic infections. In this study, we report the clinical course and management of a lung transplant recipient with pulmonary malakoplakia and provide a literature review. This is the first reported case of pulmonary malakoplakia in a lung allograft caused by Escherichia coli infection.
