Summary: Immune function changes with age, and infection with CMV accelerates these changes, resulting in increased mortality and impaired responses in elderly individuals. Following organ transplantation, CMV+ recipients show accelerated aging of CD8 T cells.

Summary: After conducting the study, it was found that the clinical manifestations of CMV retinitis differed between the SOT and HSCT groups, with different poor prognostic factors associated with each. The presence of concurrent CMV disease and foveal involvement were identified as factors associated with poor visual outcomes in CMV retinitis following transplantation in both the SOT and HSCT groups.

Summary: This article discusses the conventional and unconventional immunological mechanisms underlying the development of immune tolerance in solid organ transplant (SOT) recipients, as well as how these mechanisms can inform clinical progress. Specifically, recent mechanistic studies elucidating the role of regulatory cells in dampening cytotoxic immune reactivity and fostering a tolerogenic environment are reviewed. The understanding of regulatory cells can shape graft engineering and other therapeutic strategies to improve long-term outcomes for patients receiving hematopoietic stem cell transplantation (HSCT) and SOT.

Summary: Understanding of the involvement of the gut microbiota in human health has significantly grown in recent decades, particularly in the fields of metabolism, inflammation, and immunology. Immunosuppressive treatments given to solid organ transplant recipients can cause changes in the gut microbiota that impact various physiological processes. Medications such as Tacrolimus and steroids induce dysbiosis and metabolic disorders, while Mycophenolate Mofetil can lead to gastrointestinal side effects.

Summary: Organ transplantation survival rates have been improving due to advancements in the field, allowing for more patients with higher risk profiles to be added to waiting lists. Strategies to tackle organ shortage include the use of hypothermic and normothermic machine perfusion, as well as the implementation of hyperspectral imaging for assessing organ quality. Combining organ machine perfusion with hyperspectral imaging is seen as a prospective feasibility concept for the future of organ transplantation.

Summary: Due to impaired cell-mediated immunity, solid organ transplantation (SOT) recipients are at increased risk of developing nontuberculous mycobacterial pulmonary disease (NTM-PD). The clinical course of NTM-PD in SOT patients and the impact of SOT on the prognosis of NTM-PD remain unclear.

Summary: The endothelial glycocalyx, a thin layer lining vascular endothelial cells, is damaged during solid organ transplantation, impacting organ function. Normothermic machine perfusion technology offers a potential solution for organ preservation and quality improvement by treating the glycocalyx during storage.

Summary: As life expectancy among pediatric solid organ transplant recipients improves, the risk of comorbid conditions such as malignancy post-transplantation has also increased. Different risk factors exist for PTLD, and skin and solid cancers, and optimal cancer treatment strategies depend on the specific cancer type, stage, and patient comorbidities. Lifestyle counseling regarding smoking avoidance and sun protection, as well as human papillomavirus vaccination, is important for cancer prevention in pediatric SOTRs.

Summary: PSOT-BL is a clinically aggressive malignancy and a rare form of PTLD. Most patients developed PSOT-BL more than 2 years after transplantation. Heart and liver were the most frequently transplanted organs. The outcomes of intensive regimens for PSOT-BL patients are comparable to immunocompetent children with BL. Multi-institutional collaboration is important for collecting prospective data to determine the optimal treatment regimen.

Summary: This retrospective study examined patients who underwent cardiac valve operations after solid organ transplantation and found high prevalence of postoperative complications, especially in patients with endocarditis or undergoing concomitant transplantation and valve surgery. However, survival conditioned to hospital discharge was satisfactory.

Summary: The immune system plays a crucial role in recognizing and attacking non-self antigens, leading to immunity against infection. However, it also causes immune rejection during organ transplantation. Understanding the mechanisms of maternal-fetal immune tolerance can potentially improve transplantation outcomes by balancing immune tolerance and infection defense. Studies exploring the interconnection between immune tolerance in pregnancy and organ transplantation highlight the impact of pregnancy alloimmunization on transplant success.

Summary: A combined digital droplet PCR (ddPCR)/pyrosequencing assay system was developed for molecular genetic diagnostic applications. The results of ddPCR were in good agreement with established markers, and it showed higher sensitivity and precision in quantification. The assay is expected to be suitable for various applications.

Summary: Intestinal transplantation is a treatment option for patients with irreversible intestinal failure. This case report describes a patient who underwent temporary desensitization using the donor spleen, which resulted in successful removal of DSAs and a positive post-transplant outcome. Further research is needed to evaluate the long-term efficacy of this strategy.
