Summary: A positive crossmatch is usually not recommended for solid abdominal organ transplantation, except for liver transplantation. However, conflicting reports exist on the effects of a positive crossmatch on post-transplant outcomes. This study aimed to assess the influence of a positive crossmatch on relevant outcome parameters in liver transplantation patients. The results showed that a positive crossmatch did not affect patient and graft survival, rejection episodes, or the incidence of biliary and arterial complications. An association was found between a strong positive crossmatch and infection with PCR+ CMV.

Summary: Organ bioengineering provides a promising solution to the shortage of donor organs. Anderson et al. demonstrate a method for seeding and engrafting primary porcine hepatocytes into a bioengineered liver scaffold that had been previously reendothelialized, a significant advancement in generating transplantable livers of a clinically relevant size for humans.

Summary: This study found that female recipients and female surgeons are underrepresented in liver transplant surgery, and the survival rate of female recipients may be affected. Further research is needed to examine the societal factors influencing the outcome of female patients and take measures to possibly improve the prognosis of female liver transplant recipients.

Summary: The study analyzed survival outcomes of living donor liver transplantation (LDLT) for autoimmune liver diseases, particularly focusing on primary sclerosing cholangitis (PSC) patients. It found that adult PSC patients had lower survival rates and increased mortality risk, with male donors and biliary complications identified as important risk factors for death after LDLT in PSC patients.

Summary: Liver transplantation (LT) is a life-saving treatment for end-stage liver disease, and artificial intelligence (AI) offers a data-driven approach to improve clinical decision-making in LT. AI can be applied in optimizing transplant candidacy decision-making, donor-recipient matching, and guiding the management of LT recipients. However, limitations such as dataset imbalances, data privacy issues, and a lack of research practices to benchmark model performance in the real world need to be addressed. Overall, AI has the potential to enhance personalized clinical decision-making in liver transplant medicine.

Summary: This systematic review and meta-analysis compared the outcomes of DD-SLT and LDLT. The study included ten studies with a total of 4836 patients. The analysis showed that LDLT had higher overall survival rates at 1 year, 3 years, and 5 years compared to DD-SLT, and also had better graft survival rates at 3 years and 5 years.

Summary: Dual-graft liver transplantation (DGLT) is a surgical method that expands the pool of donors, ensures donor safety, and treats small for size syndrome (SFSS). This study utilized a rat model to investigate the causes of growth unbalance after DGLT. The findings suggest that perfusion blood flow and apoptosis may contribute to the volume differences between the dual grafts.

Summary: Comparative study of outcomes in liver transplantation between ERLT and FSLT showed that cold ischemia time and recipient's MELD score are important factors affecting ERLT outcomes. ERLT is an adequate alternative to FSLT in adults when CIT is less than 10 hours and recipient's MELD score is <20.

Summary: Prospective serial monitoring of medication non-adherence using the Morisky scale does not predict the occurrence of de novo donor-specific antibodies (dnDSA), rejection, or graft failure in kidney transplant recipients. However, the use of cyclosporine and withdrawal of calcineurin inhibitors (CNI) were strongly associated with the development of dnDSA and rejection, leading to graft failure.

Summary: This study analyzed the clinical impact of pretransplant administration of mycophenolate mofetil in reducing intrahepatic cholangiopathy in ABO-incompatible liver transplantation. The results showed that patients who underwent open surgery and received pretransplant mycophenolate mofetil had a lower incidence of intrahepatic cholangiopathy in ABO-incompatible liver transplantation. This suggests that pretransplant mycophenolate mofetil can reduce the risk of intrahepatic cholangiopathy.

Summary: This study investigated the role of total plasma exchange (TPE) in cancer treatment. It found that TPE improved oncological outcomes in patients with hepatocellular carcinoma (HCC) who underwent ABO-incompatible living donor liver transplantation. Patients who received post-transplant TPE had better HCC-specific recurrence-free survival, especially in advanced cases with microvascular invasion and beyond Milan criteria.

Summary: The study shows that LDLT is comparable to DDLT in terms of graft survival for high-MELD liver transplantation. Biliary complications are more common in LDLT group while viral infection, postoperative uncontrolled ascites, and postoperative hemodialysis are more common in DDLT group. Acute chronic liver failure, intraoperative RBC transfusion, and early postoperative complications are risk factors for graft failure in LDLT group.

Summary: This study evaluated the prognostic value of non-invasive biomarkers after curative liver resection for hepatocellular carcinoma (HCC). The results showed that FIB-4 was a significant predictor of HCC prognosis after curative liver resection, while APRI had less value in prognosis. The hazard ratios for 2-year recurrence-free survival and overall survival were 1.81 and 2.36, respectively, for FIB-4. This study suggests that FIB-4 is an important predictor of HCC prognosis after curative liver resection.
