Summary: Chronic lung allograft dysfunction (CLAD), characterized by decline in pulmonary function and bronchiolitis obliterans, lacks approved therapies. Single-cell RNA-Seq and spatial transcriptomic analysis identified the important role of JAK-STAT signaling in airway epithelial cells in CLAD. Activation of JAK-STAT signaling leads to upregulation of MHC-I in airway basal cells, resulting in cytotoxic T cell-mediated basal cell death. This study provides mechanistic insight and potential therapeutic strategy for CLAD.

Summary: This study analyzed biospecimens from 181 pediatric HCT candidates to investigate the association between pre-HCT pulmonary metatranscriptomes and post-HCT lung injury. The results showed that certain gene expression patterns and microbiome compositions in the lungs prior to transplantation were linked to an increased risk of developing lung injury after the procedure. Data from this study suggest that assessment of pre-HCT BAL fluid could potentially help identify high-risk pediatric HCT candidates who may benefit from targeted interventions.

Summary: This study examined the involvement of the GCN2/eIF2 alpha pathway in the development of both pulmonary fibrosis (PF) and pulmonary hypertension (PH). The researchers found that GCN2 expression was reduced in lung tissue from PF and PH patients, as well as in endothelial cells. Animal experiments showed that GCN2 mutations led to increased fibrosis and vascular remodeling, as well as elevated right ventricular systolic pressure. The results show that tissue HCD levels are uniquely produced by carnosine synthase (CARNS1) in fast-twitch skeletal muscle fibers and brain oligodendrocytes. Additionally, N-acetylcarnosine is a stable plasma HCD and is enriched in human skeletal muscles.

Summary: This study assessed disease progression in COVID-19 patients using in vivo CT and ex vivo microCT, and described the changes in lung volume and density as well as airway morphology. Results showed that COVID-19 lung volumes decreased and densities increased over time, mainly in the lower lobes. Ex vivo microCT revealed a 60% reduction in lung volume compared to controls, and increased visibility of airways in COVID-19 lungs, possibly due to fibrosis and increased airway diameter. Small-partition microCT further showed deformations in airway morphology and heterogeneous fibrotic organization in severely affected regions, which was confirmed by histology.

Summary: Fungal exposure and sensitization negatively impact respiratory diseases, including lung transplant recipients. This study examines the correlation between fungal-specific antibodies, fungal isolation, chronic lung allograft dysfunction (CLAD), and overall survival in 311 lung transplant patients. The results show that elevated levels of Aspergillus fumigatus or Aspergillus flavus IgG are associated with more fungal isolation and CLAD. However, IgE levels and fungal isolation are not correlated with CLAD or death. Mold and Aspergillus species isolation are associated with CLAD occurrence, while Aspergillus species isolation is also linked to impaired survival. Fungus-specific IgG could serve as a non-invasive marker for fungal exposure and a diagnostic tool for identifying high-risk patients in long-term follow-up post-lung transplant.
