Summary: This study compares the long-term outcomes of kidney and liver transplants in HIV-positive patients with matched HIV-negative patients. The results show that HIV-positive patients have comparable long-term outcomes with HIV-negative patients. The incidence of acute rejection is associated with reduced graft survival.

Summary: This study found that in HIV-positive individuals undergoing kidney transplantation, early steroid withdrawal (ESW) may increase the risk of acute rejection (AR). Although AR was more common with ESW at 1 year and over the study period, ESW did not affect death-censored graft failure or mortality.

Summary: A retrospective study was conducted at Newark Beth Israel Medical Center to compare the clinical outcomes of HIV patients hospitalized for COVID-19 with a matched control group. The study found that HIV patients had similar in-hospital mortality, ICU admission rate, and need for mechanical ventilation compared to COVID-19 patients without HIV.

Summary: B cells play crucial roles in cell-mediated alloimmune responses. Studying antigen-specific B cells may identify new targets for intervention. HLA-binding B cells are strongly associated with interferon-gamma production and may not be suppressible by endogenous Tregs.

Summary: This study evaluated the association between viral and host gene expression and NCCR variations in kidney transplant recipients (KTRs) with active and inactive BKPyV infection. The results showed that increasing NCCR variations were correlated with higher viral load of BKPyV, particularly in the Q block. Host transcriptional factors and viral genes had higher expression levels in the active BKPyV group compared to the inactive group and control group. Further studies are needed to confirm the relationship between NCCR variation and BKPyV severity in KTRs.

Summary: The study found that outcomes of renal transplantation in patients with AA and AL amyloidosis were comparable to those with diabetic nephropathy, but poorer than patients with ADPKD. In AL amyloidosis, factors such as interventricular septum thickness and achievement of deep hematologic response pre-transplant significantly impacted patient survival.

Summary: The study found that pancreas transplant alone in uremic (PTAU) patients had lower 5-year patient survival rates, while the pancreas graft survival rates were higher but needed to consider if deaths were censored. It is recommended not to recommend PTAU unless other options are not available.

Summary: The study found that in the current era, ES-ASD or ES-VSD patients represented a decreasing proportion of all lung or heart-lung transplants compared to other indications. The odds of transplantation for ES-ASD and ES-VSD patients were significantly lower compared to other candidates for lung transplantation. In the early era, ES-ASD patients who underwent heart-lung transplants had equivalent survival rates to those who underwent lung transplants, while ES-VSD patients had superior survival rates.

Summary: Italy's policy of giving national allocation priority to patients with a model for end-stage liver disease (MELD) score ≥30 has resulted in more liver transplants, fewer dropouts, and shorter waiting times for patients with MELD ≥30. However, there is still a higher risk of graft loss in these cases. The size of transplant centers and competition among centers may play a role in recipient prioritization and outcomes.

Summary: A mixed-race woman with acute myeloid leukemia achieved remission and a possible cure for HIV-1 through a CCR5A32/A32 haplo-cord transplant. Follow-up showed consistent presence of CCR5A32/A32 cord blood cells and the loss of detectable HIV-1 reservoirs. This case demonstrates the potential of stem cell transplantation in achieving HIV-1 control and possible cure.

Summary: This study investigated the proportion of hospitalized patients in Uganda who initiated ART during hospitalization and identified potential barriers. The results showed that only half of newly diagnosed HIV patients in the high prevalence setting were initiated on ART during hospitalization. Inadequate patient preparation and advanced HIV disease were identified as barriers to rapid ART initiation.

Summary: This study analyzed the prevalence and prognostic relevance of depression and anxiety in patients undergoing heart transplantation in Germany from 2010 to 2018. It found that one third of the patients were diagnosed with depression and anxiety before the transplantation. Patients with depression and anxiety were more likely to have comorbidities such as ischemic cardiomyopathy, myocardial infarction, and stroke.

Summary: The spectrum of placental pathology in HIV infection is wide, including not only the effects of the virus itself, but also coinfections, comorbidities, and changes from antiretroviral therapy. There is a lack of unbiased information on the effects of maternal HIV on the placenta and its relation to birth outcomes. Well-described HIV-associated placental findings include acute chorioamnionitis, low placental weight, and maternal vascular malperfusion.

Summary: Through analysis of UK Transplant Registry data, this study finds that prolonged delayed graft function (DGF) is associated with adverse long-term outcomes, suggesting that alloimmunity may contribute to prolonged DGF by a mechanism distinct from typical early acute rejection.

Summary: Global Health Diplomacy (GHD) is the convergence between public health and international affairs. This case report demonstrates the impact of brain drain on specialist medical services in Botswana and how GHD is being used to address this challenge. Botswana's strategies include partnerships with private institutions and other countries, but past partnerships have faced sustainability challenges. The report highlights the importance of clarity on health priorities at the start of negotiations and suggests that GHD is a new concept worth exploring.
