Summary: This study aimed to evaluate the impact of a step guided multicomponent interprofessional intervention program on adherence to immunosuppressive medication in kidney or liver transplant recipients. The results showed that the intervention group could compensate higher CV% of Tacrolimus after transplantation in individuals with lower levels of personality functioning and non-adherence.

Summary: This study aimed to compare different methods for reflecting medication non-adherence in tacrolimus concentration variation and found that higher CV and time-weighted CV, as well as lower blood concentrations of tacrolimus, can reflect medication non-adherence. It is suggested that simple IPV calculations be incorporated into medical records as an additional tool for identifying non-adherent patients in everyday clinical practice.

Summary: Prospective serial monitoring of medication non-adherence using the Morisky scale does not predict the occurrence of de novo donor-specific antibodies (dnDSA), rejection, or graft failure in kidney transplant recipients. However, the use of cyclosporine and withdrawal of calcineurin inhibitors (CNI) were strongly associated with the development of dnDSA and rejection, leading to graft failure.

Summary: After heart transplantation, intentional non-adherence (INA) to immunosuppressants is not well understood in terms of its international variability, contributing factors, and impact on clinical outcomes. This study investigated the prevalence and variability of INA, patient-level correlates of INA, and the relationship between INA and clinical outcomes. The results showed a low prevalence of INA and significant associations between patient factors and INA, but no impact on clinical outcomes.

Summary: The study aimed to investigate medication implementation adherence in liver and kidney transplant recipients converted from twice-daily, immediate-release tacrolimus to once-daily, prolonged-release tacrolimus. Results showed that most patients were non-adherent, mainly due to sub-therapeutic tacrolimus trough levels. Conversion from immediate-release to prolonged-release tacrolimus reduced pill burden and increased patient satisfaction.

The study also identifies collaborative in-house physicians and staff's perceived need for nurses in extended roles as important moderators for achieving and sustaining high fidelity.

The study identified several barriers and recommendations for implementing population health management in Belgium, and proposed introducing a population health management approach to secondary prevention of atherosclerotic cardiovascular disease as a proof-of-concept.
