Effect of Calcineurin Inhibitors in the Outcome of Liver Transplantation in Hepatitis C Virus-Positive Recipients

Background. There is a paucity of good studies evaluating the impact of calcineurin inhibitors on posttransplantation outcome in hepatitis C virus (HCV)-infected liver transplant (LT) recipients. Methods. We sought to determine whether there are differences on posttransplantation survival and histologic recurrence in HCV-LT recipients based on initial immunosuppression (IS) by conducting a prospective study comparing tacrolimus (Tac) versus cyclosporine-based IS in patients undergoing LT between 2001 and 2007. Protocol liver biopsies were performed. Results. Baseline characteristics (demographics, liver function at LT, genotype distribution, donor, surgery, and IS except for the type of calcineurin inhibitor) did not differ between groups. Severe disease (defined as bridging fibrosis, cirrhosis, cholestatic hepatitis, or allograft loss or death because of recurrent disease in the first year) was present in 67 of 253 (26.5%) and was equally distributed in the CsA and Tac groups (27% vs. 26%; P=0.68). Two thirds of protocol biopsies performed at 1 year showed some fibrosis without differences between CsA and Tac groups (75% vs. 70%). Advanced fibrosis (bridging fibrosis and cirrhosis) was diagnosed in 30% CsA and 24.5% Tac patients (P=NS). No differences in survival at 1 and 7 years were observed (83% and 67% vs. 78% and 64%, respectively, P=0.4). In summary, in patients undergoing LT for HCV-related liver disease, posttransplantation outcome is not related to the calcineurin inhibitor used.

Inadequate social support decreases survival in decompensated liver cirrhosis patients

Summary: Inadequate social support is associated with higher mortality in the general population and in patients with chronic diseases. However, there is a lack of studies on social support in liver cirrhosis and its impact on prognosis. This study analyzed the impact of social support on the survival of patients with decompensated cirrhosis. The results showed that inadequate social support predicted lower survival rate, while adequate social support independently improved survival rate, regardless of clinical variables. Therefore, social intervention strategies should be considered in the management of liver cirrhosis.

This can ultimately lead to irreversible conditions like cirrhosis and hepatocarcinoma, resulting in high mortality rates and economic burden. Understanding the immune status of NAFLD patients is crucial for therapeutic approaches, early diagnosis, and prevention.

Summary: Wilson disease is a rare inherited metabolic disorder with diverse clinical presentations affecting the liver, neurological system, psychiatric status, and vision, often in combination. Mutations in the ATP7B gene lead to copper accumulation in hepatocytes and/or neurons, making clinical diagnosis challenging. Diagnosis is complicated by mild and non-specific manifestations, mutations with unclear effects on protein function, and ambiguous laboratory tests, particularly concerning serum ceruloplasmin levels. Establishing a global collaboration of researchers, clinicians, and patient advocacy groups is crucial for identifying and addressing the outstanding challenges of Wilson disease.

Summary: IL-17 plays a crucial role in the pathogenesis of both psoriasis and MAFLD. It is mainly produced by TH17 and Tc17 cells, but other cells also contribute to its production. IL-17 mediates systemic inflammation and the recruitment of inflammatory cells to the liver, and is implicated in the development of fibrosis and insulin resistance. Clinical trials have shown that inhibiting IL-17A in patients with psoriasis could improve metabolic and liver parameters.

Health-Related Quality of Life in Patients Living with Wilson Disease in Spain: A Cross-Sectional Observational Study

Summary: Wilson disease (WD) is a rare copper metabolism disorder caused by gene mutations. This study assessed the impact of WD on patients' health-related quality of life (HRQoL), treatment adherence, drug preference, and healthcare resource utilization. The results showed that about 30% of patients experienced a deterioration in emotional status, especially in those with neurological or mixed involvement. Although most patients were satisfied with their current treatment, many expressed dissatisfaction due to having to take too many pills, indicating a need for more patient-friendly therapies. This study highlights the importance of considering the emotional domain in assessing the HRQoL of WD patients.

Waiting time dictates impact of frailty: A Spanish multicenter prospective study

Summary: This study evaluated the predictive value of the liver frailty index in outpatients in the European liver transplant setting, showing that frailty does not impact pretransplant mortality and/or delisting, but is predictive of higher complication rates and longer post-transplant length of stay. The findings highlight the importance of measuring physical frailty systematically during clinic visits and support further research on rehabilitation strategies in liver transplant settings with short waiting times.
