Summary: This study aimed to investigate whether screening for antibodies against human leukocyte antigens (HLA) and interventions to improve medication adherence and optimize immunosuppression could prevent kidney transplant failure. The results showed that the development of antibodies against HLA predicted an increased risk of graft failure, but interventions to improve medication adherence and optimize immunosuppression did not delay transplant failure. Novel interventions are needed before screening can be used to guide transplantation therapy.

Summary: This study reports the first European clinical trial on kidney transplantation aiming to induce immunological tolerance through mixed lymphohematopoietic chimerism. The results demonstrate that combined kidney and hematopoietic stem cell transplantation from an HLA-matched sibling is a feasible and successful approach, while maintaining immune responsiveness to mRNA vaccines.

Factors associated with increased Nocardia infection include age, history of transplant failure or rejection. Early recognition and targeted treatment of nocardiosis in renal transplant patients need to be enhanced based on the study findings.

Summary: Chronic kidney disease (CKD) is a common complication after liver transplantation (LT), especially in children, with varying prevalence based on transplantation era and immunosuppression regimens. Maintaining tacrolimus concentrations >4 ng/mL in long-term observation is associated with a significant increase in microalbuminuria. Microalbuminuria, a risk factor of CKD, emphasizes the necessity of regular comprehensive assessment for patients in long-term follow-up.

Summary: The study retrospectively analyzed 726 renal transplant recipients converted to sirolimus, showing lower cancer incidence rates after conversion. Patients with prior malignancy were older but had lower cancer rates after conversion. Renal function and graft survival were better in all cancer types compared to patients converted to sirolimus without cancers. Patient survival was largely dependent on tumor entity.

Summary: Over 95,000 renal transplantation procedures were completed in 2021. Invasive aspergillosis (IA) affects about 1 in 250 to 1 in 43 renal transplant recipients. Major risk factors for IA include old age, diabetes mellitus, delayed graft function, acute graft rejection, chronic obstructive pulmonary disease, cytomegalovirus disease, and neutropenia. Typical pulmonary features of IA include fever, dyspnea, cough, and hemoptysis. The fastest means of establishing the diagnosis is bronchoscopy for direct microscopy, fungal culture, and Aspergillus antigen testing.
