Summary: This study evaluated the association of preoperative platelet counts and plasma fibrinogen levels with postoperative lymph node involvement and venous invasion in colon cancer patients. The results showed a significant correlation between these hemostatic markers and the presence of lymphatic and venous invasion, suggesting that the PLT count and fibrinogen level may be important markers for monitoring postoperative outcomes in colon cancer patients.

Summary: This study investigated the trajectories of platelet counts after the first cycle of induction chemotherapy in patients with acute myeloid leukemia, and found that these trajectories were significant predictors of all-cause mortality. Timely intervention should be considered for the low-stability group, while the low-level decrease-medium elevation and low-level decrease-high elevation groups were independent protective factors for all-cause mortality.

Summary: High blood pressure after ischemic stroke is common and associated with poor outcomes and increased mortality. The decision on whether to lower blood pressure to improve outcomes or risk worsening cerebral perfusion is still debated. Managing high blood pressure requires consideration of different scenarios and understanding the type, timing, and effects of antihypertensive drugs on cerebral perfusion and platelets.

Summary: We report a case of atorvastatin-induced hypersensitivity reaction in a 58-year-old male who experienced symptoms within 24 hours of first exposure to the drug. The patient presented with throat swelling, trouble swallowing, and throat pain, but without pruritis or skin manifestations. Treatment with epinephrine, famotidine, and diphenhydramine resulted in symptom resolution within 15 minutes.

Summary: The study aimed to validate the phenomenon of TMBs becoming temporarily less detectable in SWI after traumatic brain injury. Results showed that TMBs may become less visible in SWI MRI during the subacute period of 24-72 hours post-injury.

Summary: This study investigated the effects and risks of endovascular thrombectomy 6 to 24 hours after basilar-artery occlusion stroke. The results showed that thrombectomy led to a higher percentage of patients with good functional status at 90 days compared to medical therapy alone, but was associated with more procedural complications and cerebral hemorrhages.

Summary: The present study shows that higher initial serum lactate levels in neurosurgical ICH patients are associated with more severe neurological impairment and cardiopulmonary complications. These patients require more cardiopulmonary support within the first 24 hours of ICU treatment.

Summary: Severe malaria caused by Plasmodium falciparum is difficult to accurately diagnose in children in high-transmission settings. A combination of platelet count and plasma PfHRP2 concentration can be used to identify severe falciparum malaria in severely ill patients. About one-third of children enrolled in clinical studies of severe malaria in high-transmission settings in Africa had other causes of their severe illness.

Summary: The study assessed the clinical significance of mismatch negativity (MMN) in predicting awakening of comatose patients with severe brain injury. It was found that MMN amplitude at Fz was a reliable prognostic indicator for comatose patients with severe brain injury, and when combined with Glasgow Coma Scale (GCS), the prediction accuracy improved to 84.6%. Quantitative approach, such as using a cutoff value of 1.08 μV, could enhance the use of MMN for prognostication.

Summary: This study aimed to establish normal reference ranges and individual variations in heart rate (HR) of healthy term-born infants during the first 24 hours of life using auscultation. The results showed significant variations in HR within the first 24 hours, with factors like awake/asleep state, maternal temperature, and gender influencing the HR values.

Summary: The study found that preoperative serum fibrinogen levels in patients with gallbladder carcinoma were significantly higher than healthy individuals and patients with cholesterol polyps. Fibrinogen levels were associated with tumor depth, metastasis, and TNM stage. Patients with lower fibrinogen levels had a better overall survival than those with higher fibrinogen levels.

Summary: Through collaboration with the state health department, we successfully provided whole blood resuscitation to fire department paramedics, offering an effective treatment for urban trauma patients. After two public hearings, statewide approval was obtained for emergency uncrossmatched blood transfusion by paramedics. Over a year, 160 units of whole blood were provided, with 51 units transfused to 39 patients, including 30 trauma patients.

Summary: This study aimed to survey blood utilization practices in liver transplant centers in the United States and identify opportunities for standardization. The results showed that more blood products are issued during surgery than are actually transfused. Many centers use viscoelastic testing and cell salvage, but lack standardized approaches for plasma and platelets.

Summary: The study found that large language models (LLMs) are highly proficient at answering knowledge-based questions. Publicly available LLMs are able to accurately perform tasks in blood banking and transfusion medicine. GPT-4 consistently scored the highest in all tasks, but further research is needed to assess the utility of LLMs in transfusion medicine practice.

Summary: We investigated whether patients over 50 years of age with acute traumatic brain injury (TBI) have lower platelet counts and whether these lower platelet counts are independently associated with mortality. The results showed that older patients had lower admission platelet counts and higher mortality rates. Factors such as age, platelet count, severity of head injury, and other factors were all independently associated with mortality.

The curriculum was piloted with 2019-2020 anesthesiology residents and showed increased understanding and usefulness according to post-course surveys. As an addition to our existing materials, we are sharing four patient cases with 16 questions for immediate teaching purposes.

Summary: Cardiothoracic surgical critical care medicine (CT-CCM) is a medical discipline focused on perioperative care and is projected to see an increase in utilization due to an aging population and the rise of chronic diseases. This two-part review discusses the importance of assessing current CT-CCM practices, setting goals for the future, and identifying areas of knowledge that need further scientific research. Part one emphasizes the need for growth and promotion of CT-CCM, while part two evaluates scientific data, identifies knowledge gaps, and provides recommendations for future endeavors.
