Summary: Human T-lymphotropic virus type 1 (HTLV-1) is a global viral infection that affects an estimated 5 to 10 million people and can lead to severe and potentially fatal diseases. The infection is geographically concentrated, with high prevalence in specific regions and populations. Despite screening in high-income countries, there is a lack of public health measures to prevent HTLV-1 infection. This review emphasizes the need for further research and collaboration to improve prevention, management, and awareness of HTLV-1 infection.

Summary: By analyzing over 3 million blood donations in Ireland from 1996 to 2020, it was found that 6 donations were HTLV positive, confirming the risk of TT-HTLV. Modeling the risk of TT-HTLV showed that by combining leucodepletion with universal screening or selective testing, a TT-HTLV infection could be prevented every 468-3776 years. These findings will inform blood donation screening policy in Ireland and advocate for a selective HTLV donor screening algorithm.

Summary: Maternal obesity has been shown to disrupt fetal immune system development and increase susceptibility to infection in offspring. Offspring of obese mothers show higher frequencies of memory CD4 T cells, potential signs of in vivo activation, and possible functional defects. These abnormalities are associated with genomic changes and may explain the link between the obesogenic maternal environment and fetal immune reprogramming.

Summary: This study reveals the silent spread of Human T-Lymphotropic Virus Type 1 (HTLV-1) from high-endemic to low-endemic areas in China. Among the blood donors tested, 50 were confirmed to be HTLV-1/2 positive, and all 26 HTLV-1 carriers had the HTLV-1a genotype. Additionally, the spouses of male index donors had a significantly higher HTLV-1 infection rate compared to the husbands of female index donors. However, the children of HTLV-1 positive women tested negative.

Summary: This study investigated the prevalence, occurrence of new cases, and subtypes of HTLV infection among prisoners in closed prisons in Brazil. The prevalence rate of HTLV infection was similar to the general population, highlighting the need for preventive interventions and harm reduction measures to reduce the risk of HTLV transmission among prisoners.

Summary: In the past, direct neonatal phlebotomy was commonly used to obtain blood for admission laboratory studies in neonates. However, over the past decade, there have been increasing studies evaluating the feasibility and clinical impact of using cord blood samples for admission testing. This article reviews multiple studies that have demonstrated the acceptability and benefits of using cord blood samples in neonatal admission testing.

Summary: Parallel testing of blood donations for HCV antibody and RNA has been standard practice in Australia since 2000. Improved NAT technologies and the curability of HCV have reduced the risk and consequences of HCV transmission through transfusion. This study aimed to determine the optimal testing strategy by estimating the residual risk under various testing options.

Summary: This nationwide serological survey among Chinese blood donors revealed that the overall prevalence of HTLV infection is low in most areas of China, but varies significantly in different geographic regions. Female gender was identified as an independent risk factor for HTLV infection in China, and monitoring seroindeterminate donors for seroconversion is crucial for blood safety.

Summary: This study aimed to establish the construct validity of the home parenteral nutrition patient-reported outcome questionnaire (HPN-PROQ) with 77 HPN-dependent patients. Results showed that most patients with chronic intestinal failure reported a major impact of underlying illness on quality of life, and emotional difficulty in coping with HPN was positively associated with its impact on quality of life.
