Polymorphisms of PAI-1 and platelet GP Ia may associate with impairment of renal function and thrombocytopenia in Puumala hantavirus infection

Introduction: Puumala virus (PUUV) infection is a viral hemorrhagic fever with renal syndrome (HFRS) characterized by thrombocytopenia and acute impairment of renal function. We aimed to assess whether genetic polymorphisms of platelet antigens together with those of von Willebrand factor (VWF) and plasminogen activator inhibitor (PAI-1) correlate with disease severity. Patients and methods 172 consecutive hospital-treated patients with serologically confirmed acute PUUV infection were included. Platelet glycoprotein (GP) IIIa T > C (rs5918), GP Ia T > C (rs1126643), GP Ib C > T (rs6065), GP VI T > C (rs1613662), VWF A > G (rs1063856) and PAI-1 A> G (rs2227631) were genotyped. The associations of the rarer alleles with variables reflecting the severity of the disease were analyzed. Results: PAI-1 G-carriers had higher maximum creatinine level compared with the non-carriers (median 213 mu mol/l, range 60-1499 mu mol/l vs. median 122 mu mol/l l, range 51-1156 mu mol/l, p = 0.01). The GG-genotypes had higher creatinine levels than GA- and AA-genotypes (medians 249 mu mol/l, 204 mu mol/l and 122 mu mol/l, respectively, p = 0.03). Polymorphisms of GP VI and VWF associated with lower creatinine levels during PUUV infection. The minor C-allele of GP Ia associated with lower platelet counts (median 44x10(9)/l, range 20-90 x 10(9)/l vs median 64 x 10(9)/l, range 3-238 x 10(9)/l; p = 0.02). Conclusions: Polymorphism of PAI-1, a major regulator of fibrinolysis, has an adverse impact on the outcome of kidney function in PUUV-HFRS. Platelet collagen receptor GP Ia polymorphism associates with lower platelet count. (c) 2011 Elsevier Ltd. All rights reserved.

Severity Biomarkers in Puumala Hantavirus Infection

Summary: In Europe, over 10,000 cases of hemorrhagic fever with renal syndrome (HFRS) are reported annually, with Puumala hantavirus (PUUV) being the main causative agent. PUUV usually causes a mild disease with low fatality rates, but the factors influencing disease severity are largely unknown. Host genes have been found to have an effect. Typical clinical features of PUUV infection include acute kidney injury, thrombocytopenia, and increased vascular permeability. Although PUUV does not directly cause cytopathology of the endothelial cells, significant changes in both the barrier function of the endothelium and the infected endothelial cells occur. Host immune or inflammatory mechanisms are likely important in the development of capillary leakage. Several immunoinflammatory biomarkers have been studied to assess the severity of PUUV-induced HFRS. Most of them are not currently used in clinical practice, but the increasing knowledge about these biomarkers has helped to elucidate the pathogenesis of PUUV infection.

Long-Term Consequences of Puumala Hantavirus Infection

Summary: Several viral infections can lead to both acute and long-term complications. There have been numerous reports in the past two years about post-infectious symptoms experienced by COVID-19 patients. Serious complications may occasionally arise during the acute phase of Puumala orthohantavirus-induced nephropathia epidemica, but severe long-term consequences are rare. Fatigue lasting for several weeks is quite common, and hormonal insufficiencies should be ruled out if the patient does not recover normally.

Summary: This study aimed to evaluate the amount and distribution of fluid collections in patients with acute PUUV infection and their association with symptoms and clinical findings. Results showed that fluid collections were found in all patients. The amount of intraperitoneal fluid correlated positively with CRP levels and inversely with serum creatinine concentrations. Back pain was associated with higher creatinine levels but not with the presence of fluids.

The severity of the infection may be influenced by genetic factors such as human leukocyte antigen (HLA) alleles and genes related to tumor necrosis factor (TNF) and complement system. Autoimmune phenomena, Epstein-Barr virus infection, and individual differences in ocular and central nervous system manifestations are also associated with PUUV infection.

Adhesive properties of plasma-circulating and platelet-derived microvesicles from healthy individuals

Summary: In this study, we investigated the adhesive properties of circulating plasma-derived and platelet-derived microvesicles (MVs) and their potential role in thrombus growth. The results showed that both types of MVs weakly bound fibrinogen but did not stably adhere to immobilized fibrinogen. Moreover, the presence of these MVs did not significantly affect thrombus formation. Therefore, MVs present in the plasma of healthy individuals are unlikely to directly contribute to thrombus formation under arterial flow conditions.

Thrombotic triad in microgravity

Summary: Thrombotic disease may be underdiagnosed in microgravity conditions, and the underlying factors are still poorly understood. A case of internal jugular vein thrombosis was recently diagnosed in a low-risk female astronaut, and six out of ten additional crew members showed risk factors for jugular venous flow. Observations in space and studies on Earth suggest that microgravity affects blood flow, coagulation, and vascular function.

Driver gene KRAS aggravates cancer-associated stroke outcomes

Summary: The incidence of cancer-associated stroke has increased with the prolonged survival times of cancer patients. In this study, the researchers focused on the driver gene KRAS and found that KRAS mutations exacerbated neurological deficits after stroke and promoted tumor growth. They also identified the upregulation of pro-inflammatory responses and the promotion of STAT3 phosphorylation in mice with KRAS mutations. Additionally, the researchers found that colorectal cancer patients with stroke onset within 3 months after cancer diagnosis had a worse prognosis.
