This review analyzes the currently available literature on circadian rhythms of intraocular pressure (IOP), blood pressure, and calculated ocular perfusion. pressure (OPP) in patients with open-angle glaucoma. Although adequately powered, prospective trials are not available. The existing evidence suggests that high 24-hour IOP and OPP fluctuations can have detrimental effects in eyes with glaucoma. The currently emerging continuous IOP monitoring technologies may soon offer important contributions to the study of IOP rhythms. Once telemetric technologies become validated and widely available for clinical use, they may provide an important tool towards a better understanding of long- and short-term TOP fluctuations during a patient's daily routine. Important issues that need to be investigated further include the identification of appropriate surrogate measures of IOP and OPP fluctuation for patients unable to undergo 24-hour measurements, the determination of formulae that best describe the relationship between systemic blood pressure and IOP with OPP, and the exact clinical relevance of IOP and OPP fluctuation in individual patients. Despite the unanswered questions, a significant body of literature suggests that OPP assessment may be clinically relevant in a significant number of glaucoma patients. Pre-operative conjunctival flora in patients with local and/or systemic risk factors for post cataract surgery infection in Northern Italy

Carlo Bruttini, Carlotta Pallone, Alice Verticchio Vercellin, Giovanni Acerbi, Santina Baschetti, Pietro Bruttini, Ivano Riva, Luciano Quaranta

Summary: The study investigated the pre-operative conjunctival flora in cataract surgery patients with major risk factors for post-operative infection. Results showed a low positivity rate of swabs, with Staphylococcus aureus and Staphylococcus epidermidis being the most isolated microorganisms. Gram-negative bacteria, including Pseudomonas aeruginosa, were also present in a small percentage of cases.

Identification of SARS-CoV-2 RNA in the conjunctival swab of an Italian pediatric patient affected with COVID-19: A case report

Luciano Quaranta, Francesca Rovida, Ivano Riva, Carlo Bruttini, Ilaria Brambilla, Fausto Baldanti, Giovanni De Angelis, Gian Luigi Marseglia

Summary: This article reports the identification of SARS-CoV-2 RNA in ocular specimen of a pediatric patient without signs of ocular involvement. However, it is still unclear whether this could be considered an important route for the spread of SARS-CoV-2.

Human recombinant nerve growth factor (Cenegermin) in a patient affected by primary congenital glaucoma with neurotrophic keratopathy

Ivano Riva, Eleonora Micheletti, Riccardo Fausto, Carlo Bruttini, Giovanni De Angelis, Roberto Ceccuzzi, Luciano Quaranta

Summary: A case of neurotrophic keratopathy in a patient with primary congenital glaucoma was effectively treated with human recombinant Nerve Growth Factor eye drops, leading to complete resolution of corneal integrity and improvement in visual acuity.

Effect of COVID-19-related lockdown on ophthalmic practice in Italy: A report from 39 institutional centers

Summary: The number of eye surgical procedures in Italy significantly decreased during the lockdown due to the COVID-19 epidemic, with only 70% of recommended surgeries being performed. Elective surgeries decreased by 96.2%, urgent surgeries by 49.7%, and intravitreal injections by 48.5% during the lockdown period.

Evaluation of macular and optic nerve head parameters in amblyopic eyes: An optical coherence tomography study

Rosanna Guagliano, Donatella Barilla, Chiara Bertone, Silvia Montescani, Alice C. Verticchio Vercellin, Cristina Arpa, Carmine Tinelli, Giovanni De Angelis, Luciano Quaranta

Summary: This study found significant differences in optic nerve morphology, central macular thickness, and OCT scan quality between amblyopic patients and normal individuals, but no significant impact on retinal nerve fibers layers and ganglion cell complex was observed in monolateral amblyopia.

A new look at the safety and tolerability of prostaglandin analogue eyedrops in glaucoma and ocular hypertension

Andreas Katsanos, Ivano Riva, Banu Bozkurt, Gabor Hollo, Luciano Quaranta, Francesco Oddone, Murat Irkec, Gordon N. Dutton, Anastasios G. Konstas

Summary: Topical prostaglandin analogues (PGAs) have become the first line and first choice therapeutic options in the management of glaucoma and ocular hypertension (OHT) in the past 25 years. Despite extensive research on their short-term efficacy and safety, the long-term safety and tolerability of PGAs still need further investigation. New opportunities and future challenges are presented by recently launched novel PGAs or those still in development.

Physiology-Enhanced Data Analytics to Evaluate the Effect of Altitude on Intraocular Pressure and Ocular Hemodynamics

Alice Verticchio Vercellin, Alon Harris, Aditya Belamkar, Ryan Zukerman, Lucia Carichino, Marcela Szopos, Brent Siesky, Luciano Quaranta, Carlo Bruttini, Francesco Oddone, Ivano Riva, Giovanna Guidoboni

Summary: Altitude has an impact on intraocular pressure, blood pressure, and retinal hemodynamics. The study found that increasing altitude leads to an increase in blood flow and pressure in the retinal vasculature, while decreasing intraocular pressure. These findings provide insights into the underlying mechanisms of changes in ocular blood flow and pressure at different altitudes.

Adherence and Persistence to Medical Therapy in Glaucoma: An Overview

Luciano Quaranta, Alessio Novella, Mauro Tettamanti, Luca Pasina, Robert N. N. Weinreb, Alessandro Nobili

Summary: Glaucoma is a progressive optic neuropathy characterized by retinal ganglion cell loss and visual field deterioration. While the exact cause of glaucoma is still unknown, elevated intraocular pressure has been identified as a modifiable risk factor. Treatment with eye drops to lower intraocular pressure has been proven effective in reducing the risk of disease progression. However, many patients with glaucoma struggle with adherence to medication, which is associated with poor treatment outcomes and increased healthcare costs.
