Summary: A study in Korea found that ERCP-related adverse events most commonly occurred after pancreatic stent insertion, diagnostic ERCP, and endoscopic sphincterotomy. Special caution should be used for young patients receiving diagnostic ERCP due to an increased risk of post-ERCP pancreatitis.

Summary: This study retrospectively analyzed 45 patients with endoscopic retrograde cholangiopancreatography (ERCP) related perforation. The results showed that the incidence rate of perforation was 0.22%, with periampullary and duodenal wall perforations being the most common. Most patients recovered with conservative treatment, while a few required surgical intervention. The mortality rate was 2.2%.

Summary: This study evaluated the incidence and severity of adverse events (AEs) in biliary endoscopic retrograde cholangiopancreatography (ERCP), and identified risk factors and preventive measures for post-ERCP pancreatitis (PEP). The overall incidence of AEs was 10.1%, with a mortality rate of 0.08%. Prophylactic pancreatic stenting and epinephrine spraying were found to prevent PEP effectively.

Summary: Endoscopic management for perihilar cholangiocarcinoma is evolving to improve diagnosis and drainage. Different imaging methods are used to determine resectability and optimal surgical planning, while new diagnostic imaging methods such as video peroral cholangioscopy and probe-based confocal laser endomicroscopy are being clinically applied. The approach to preoperative drainage has shifted to endoscopic nasobiliary drainage to reduce the risk of metastasis seeding. Various endoscopic techniques, including the use of self-expandable metallic stents and endoscopic ultrasonography-guided approach, are employed depending on the resectability and prognosis.

Summary: This study evaluated the effect of biliary complications on the overall patient's survival rate after endoscopic retrograde cholangiopancreatography (ERCP) for liver cancer treatment. Among 967 liver cancer patients, 102 developed biliary complications, and 68 underwent therapeutic ERCP procedures. Patients without biliary complications had similar survival rates compared to those who underwent ERCP. Therefore, although ERCP is useful for managing post liver cancer treatment biliary complications, it often requires multiple procedures and has a relatively high rate of complications, leading to increased mortality.

Summary: This randomized controlled trial evaluated the efficacy and safety of a novel hemostatic gel for post-EST or post-EP bleeding. The results showed that the new hemostatic gel was superior to epinephrine spray for successful primary hemostasis, and there was no significant difference in delayed bleeding. The novel hemostatic gel is expected to achieve easier hemostasis for immediate bleeding after EST or EP.

Summary: This network meta-analysis examined the efficacy of various prophylactic strategies for reducing the risk of postendoscopic retrograde cholangiopancreatography pancreatitis (PEP) in patients with risk factors. The results showed that combination prophylactic methods were more effective in preventing PEP compared to no prophylaxis or active control. The ABC combination strategy was found to be the most effective in reducing the risk of PEP.
