Summary: Non-alcoholic fatty liver disease is a major cause of liver transplant globally, with limited treatment options available. Bariatric endoscopic and surgical therapies are considered effective strategies for improving quality of life, reducing cardiovascular risk, and enhancing metabolic profile, despite being costly. This review suggests considering these therapies early, along with liver fibrosis evaluation, in order to achieve long-term cost-effectiveness when lifestyle modifications and pharmacological treatments fail.

Summary: Post-surgical leaks and fistulas are common and difficult to treat complications of bariatric surgery. Endoscopic therapy plays an increasingly important role in managing these conditions, with early diagnosis and initiation of treatment being crucial for success. Different endoscopic techniques with various mechanisms of action are available, and treatment should be individualized based on patient and defect characteristics. Although standardized treatment algorithms are lacking, this narrative review aims to provide a summary of current scientific evidence and make recommendations for the best endoscopic approach.

Summary: This study examined the safety, efficacy, durability, and physiologic effects of the POSE 2.0 procedure in obese adults. The results showed that POSE 2.0 significantly reduced body weight, improved obesity-related comorbidities, and had a durable effect up to one year post-surgery.

Summary: This study aimed to evaluate the quality of surgical guidelines on bariatric/metabolic surgery. Four independent reviewers used the AGREE II tool to assess the methodological quality of the included guidelines and conducted a comparative analysis of the main recommendations for surgical methods. Results showed that nine surgical guidelines were included, with five articles worthy of clinical recommendation based on AGREE II scores over 60%. The overall rigor of development was relatively low with an average score of 50.82%. Only 4 out of 15 key recommendations had a grade A rating.

Summary: ML models developed using preoperative data showed poor performance in multicenter external validation. Best performance was achieved by linear regression (LR) and support vector machine models for TWL% and EWL% with the addition of follow-up data, indicating potential for improving predictive capacity. In single-center internal and temporal validation, similar results were observed and early follow-up data addition improved prediction accuracy.

Summary: This study analyzed the American College of Surgeons National Surgical Quality Initiative Program database from 2005 to 2017 and found that there is no increase in postoperative complications within 30 days when endoscopy is performed during revisional bariatric surgery (RBS). The results are similar to those of non-revisional bariatric surgery.

Summary: Since its first description in 2013, there has been growing evidence supporting the efficacy and safety of endoscopic sleeve gastroplasty (ESG). Some studies have even reported 5-year data, and ESG combined with pharmacotherapy may achieve weight loss similar to surgical sleeve gastrectomy. While ongoing randomized trials are awaited, current data support the clinical use of ESG, especially for patients who are unwilling or unfit for bariatric surgery.

Summary: This study aimed to evaluate the diagnostic yield of routine preoperative esophagogastroduodenoscopy (p-EGD) in patients undergoing bariatric surgery. The study found that gastroesophageal reflux disease (GERD) and hiatal hernias are common in the bariatric population, and routine preoperative EGD can frequently identify clinically significant upper gastrointestinal (UGI) pathology. This has important implications for medical and surgical management.

Endoscopic sleeve gastroplasty (ESG) videos have higher quality compared to videos related to Roux-en-Y gastric bypass (RYGB) and vertical sleeve gastrectomy (VSG). Educational videos have higher quality compared to weight loss transformation and personal experience videos.

Summary: Comparing transnasal endoscopy (TNE) to esophagogastroduodenoscopy (EGD) in preoperative screening for bariatric surgery, TNE was found to be associated with less post-procedure care, shorter procedure time, and fewer charges compared to EGD. TNE could be considered as an initial screening tool for patients undergoing bariatric surgery, while EGD could be selectively used in patients with abnormal TNE findings.

Summary: This study retrospectively analyzed over 2000 patients to evaluate the safety of endoscopic sleeve gastroplasty (ESG) assisted by postgraduate medical trainees. The results showed that procedures performed by attending physicians alone had lower adverse event rates and re-operation rates, while procedures assisted by trainees had longer duration and length of stay.
