Effect of primary versus revisional Roux-en-Y gastric bypass: inferior weight loss of revisional surgery after gastric banding

Background: Laparoscopic adjustable gastric banding is a popular and effective restrictive bariatric procedure. However, with longer follow-up, it has become clear that a considerable number of patients require revisional surgery, of which Roux-en-Y gastric bypass (RYGB) is the most commonly performed procedure. Studies that compared the outcomes of primary RYGB and revisional RYGB have not been conclusive. Our objective was to determine whether significant differences exist in the 1-year outcomes between primary RYGB (prim-RYGB) and revisional RYGB after laparoscopic adjustable gastric banding (rev-RYGB) at a major training hospital in The Netherlands. Methods: All prim-RYGB and rev-RYGB procedures performed from 2007 to 2009 were analyzed. Data were collected regarding weight loss, hospitalization, operative time, postoperative complications, and co-morbidities. Results: A total of 292 RYGB procedures were performed: 66 rev-RYGB and 226 prirn-RYGB procedures. The operative time was significantly shorter in the prim-RYGB group (136.6 +/- 37.5 versus 167.5 +/- 40.6 min; P < .0001). No significant differences were found in hospitalization time (4.4 +/- 1.7 versus 4.9 +/- 2.4 d; P = .063) or complication rate (14.7% versus 15.2%; P = .962). No deaths occurred in either group. The number of patients with resolved diabetes and hypertension did not differ between the 2 groups (50.1% versus 23.1%; P = .116; and 40.7% versus 25.0%; P = .384, respectively). Weight loss was significantly greater in the prim-RYGB group in terms of excess weight loss (71.6% +/- 20.8% versus 48.4% +/- 26.8%; P < .0001), body mass index reduction (13.0 +/- 3.8 versus 10.2 +/- 5.6 kg/m(2); P < .0001), absolute weight loss (37.4 +/- 11.5 versus 29.3 +/- 17.2 kg; P = .001), and percentage of weight loss (29.7% +/- 8% versus 21.7% +/- 11.5%; P < .0001). Conclusion: rev-RYGB is a safe procedure with outcomes similar to those of prim-RYGB in terms of complication rate, hospitalization time, and effect on co-morbidity. Summary: Laparoscopy during the COVID-19 pandemic may pose a potential risk of SARS-CoV-2 transmission, although the risk is considered very low. Due to the widely known benefits of laparoscopy compared to the minimal risk, there is no need to replace it with laparotomy for COVID-19 infection. Additional safety measures are recommended to avoid potential small risk of viral transmission.

Resizing a large pouch after laparoscopic Roux-en-Y gastric bypass: comparing the effect of two techniques on weight loss

Summary: This study compared the outcomes of two different surgical techniques for revisional bariatric surgery in patients with insufficient weight loss or weight regain due to an enlarged pouch. Resizing the pouch led to additional weight loss, and both techniques showed comparable weight loss outcomes. The SGP technique may be preferable based on average surgical costs.

Four-dimensional flow cardiovascular magnetic resonance imaging (4D flow CMR) is a novel tool that provides comprehensive assessment of flow through encoding velocity in all 3 directions. However, the analysis of 4D flow data is still complex and requires accurate analysis tools.

Summary: This study compared cardiovascular magnetic resonance-derived left ventricular function, strain, and infarct size characteristics in patients with transient ST-segment elevation myocardial infarction (TSTEMI), ST-segment elevation myocardial infarction (STEMI), and non-ST-segment elevation myocardial infarction (NSTEMI). The results showed that TSTEMI patients had better LV function and smaller infarct size compared to STEMI patients, and their characteristics were more similar to NSTEMI patients.

Summary: This study compared the performance of two-dimensional echocardiography (2DE), contrast-2DE and real-time three-dimensional echocardiography (RT-3DE) in the diagnosis of left ventricular apical thrombus (LVAT). The results showed that RT-3DE was more sensitive and specific than 2DE and contrast-2DE in diagnosing LVAT, with higher diagnostic accuracy.

EACVI survey on burnout amongst cardiac imaging specialists during the 2019 coronavirus disease pandemic

Summary: The European Association of Cardiovascular Imaging Scientific Initiatives Committee conducted a global survey to evaluate the impact of the COVID-19 pandemic on the mental well-being of cardiac imaging specialists. The survey results showed that more than half of the specialists experienced anxiety, one-third felt melancholic, and a quarter increased their alcohol intake. More than half reported difficulties in sleeping and 44% considered quitting their job.

Left ventricular four-dimensional blood flow distribution, energetics, and vorticity in chronic myocardial infarction patients with/without left ventricular thrombus

Summary: In this study, the researchers used 4D flow cardiovascular magnetic resonance (CMR) to investigate the differences in left ventricular (LV) flow between chronic myocardial infarction (MI) patients with and without left ventricular thrombus (LVT), as well as healthy controls. They found that MI patients with LVT had altered LV flow components, irregular vorticity vector fields, and changes in diastolic vortex ring geometric properties compared to those without LVT. These findings suggest potential links between global LV flow characteristics and LVT formation.

Evaluation of CPAP adherence in bariatric patients diagnosed with obstructive sleep apnea: outcomes of a multicenter cohort study

Summary: In this study, CPAP adherence was evaluated in obese patients with newly diagnosed obstructive sleep apnea (OSA) who underwent bariatric surgery. The results showed that inadequate CPAP adherence was higher in these patients compared to those with pre-existing OSA. Although there were no significant differences in cardiopulmonary complications and ICU admissions between the two groups, strategies to improve CPAP adherence may be valuable in newly diagnosed OSA patients undergoing bariatric surgery.

Summary: According to a global survey conducted by EACVI, echocardiography remains the most frequently used imaging modality for the evaluation of the right heart, while the use of other imaging techniques, particularly CMR, is on the rise. The majority of survey participants include assessment of right ventricle size and function in echocardiographic reports, but only half routinely include volumetric and ejection fraction assessments in CMR reports.

Summary: This study assesses the relationship between different left atrial (LA) strain methods and atrial fibrillation (AF) recurrence, comparing LA feature tracking (FT) strain with novel rapid LA strain approaches. The results show that both LA rapid strain and LA FT strain are associated with arrhythmia recurrence after ablation in AF patients. Therefore, LA rapid strain can be a convenient and reproducible alternative for assessing LA function in clinical practice.

The Impact of Reoperations Following Bariatric Surgery on Mid-term Outcomes

Summary: This study investigated the relationship between symptom-to-reperfusion time and transmural infarct extent and strain parameters in ST-segment elevation myocardial infarction (STEMI) patients. The results showed that shorter reperfusion time was associated with smaller infarct extent and better left ventricular strain. The study also found a close link between infarct transmurality and residual left ventricular strain.
