Primary Endoscopic Transnasal Transsphenoidal Surgery for Magnetic Resonance Image-Positive Cushing Disease: Outcomes of a Series over 14 Years

BACKGROUND: There are scant data of endoscopic transsphenoidal surgery (ETS) with adjuvant therapies of Cushing disease (CD). OBJECTIVE: To report the remission rate, secondary management, and outcomes of a series of CD patients. METHODS: Patients with CD with magnetic resonance imaging (MRI)-positive adenoma who underwent ETS as the first and primary treatment were included. The diagnostic criteria were a combination of 24-hour urine-free cortisol, elevated serum cortisol levels, or other tests (e.g., inferior petrosal sinus sampling). All clinical and laboratory evaluations and radiological examinations were reviewed. RESULTS: Forty consecutive CD patients, with an average age of 41.0 years, were analyzed with a mean follow-up of 40.2 +/- 29.6 months. These included 22 patients with microadenoma and 18 with macroadenoma, including 9 cavernous invasions. The overall remission rate of CD after ETS was 72.5% throughout the entire follow-up. Patients with microadenoma or noninvasive macroadenoma had a higher remission rate than those who had macroadenoma with cavernous sinus invasion (81.8% or 77.8% vs. 44.4%, P = 0.02). After ETS, the patients who had adrenocorticotropic hormone-positive adenoma had a higher remission rate than those who had not (76.5% vs. 50%, P = 0.03). In the 11 patients who had persistent/recurrent CD after the first ETS, 1 underwent secondary ETS, 8 received gamma-knife radiosurgery (GKRS), and 2 underwent both. At the study end point, two (5%) of these CD patients had persistent CD and were under the medication of ketoconazole. CONCLUSION: For MRI-positive CD patients, primary (i.e., the first) ETS yielded an overall remission rate of 72.5%. Adjuvant therapies, including secondary ETS, GKRS, or both, yielded an ultimate remission rate of 95%.

Summary: This study compared the clinical outcomes of CBT-based Dynesys dynamic stabilization (CBT-DDS) with standard minimally invasive transforaminal lumbar interbody fusion (MI-TLIF). The results showed that CBT-DDS had similar clinical and radiological outcomes to MI-TLIF in patients with L4-L5 or L3-L5 spondylosis and spondylolisthesis, making it a viable and effective alternative with a shorter operation time and similarly limited segmental motility.

A bibliometric analysis of CDA literature reveals a trend towards multilevel CDA and fewer studies comparing CDA to anterior cervical discectomy and fusion (ACDF). CDA has proven to be effective in maintaining segmental mobility and neurologic improvement with low complication rates, especially for 1- and 2-level disc diseases.

Lumbar paraspinal atypical spindle cell/pleomorphic lipomatous tumor: A report of a rare case

Summary: The atypical spindle cell/pleomorphic lipomatous tumor (ASPLT) was classified as a new tumor by the World Health Organization (WHO) in 2020. It is a benign tumor commonly found in the limbs, with rare occurrence in the paraspinal region. This case study reports a 38-year-old male with ASPLT located in the left psoas muscle, presenting with sudden onset back pain. Surgical excision was performed successfully, leading to improved symptoms and no postsurgical neurological deficit. The risk of metastasis was minimal, but there is a nonnegligible risk of local recurrence.

The results showed that serum leptin levels were higher in the OPLL group, and angiogenin, osteopontin, and osteopro-tegerin were also higher in OPLL tissue. Additionally, cytokines promoting bone genesis and inhibiting osteoclastogenesis were up-regulated in OPLL tissue, while molecules promoting angiogenesis were abundant in the OPLL tissue.

Modic Changes Increase the Cage Subsidence Rate in Spinal Interbody Fusion Surgery: A Systematic Review and Network Meta-Analysis

Summary: This study aimed to compare the effect of different Modic changes (MC) grades on the cage subsidence rate after spinal interbody fusion surgery. A meta-analysis was conducted on relevant randomized controlled trials and cohort studies. The results showed that MC may be associated with a higher cage subsidence rate. With the increase in MC grades, the incidence of subsidence decreased gradually, but it was always higher than that in the non-Modic changes (NMC). Oblique lumbar interbody fusion may be a better treatment choice for lumbar degenerative disease with MC.

A Medical Student's Guide to Online Resources in Neurosurgery

This review identifies and describes 16 educational resources, serving as a guide for medical students interested in neurosurgery.
