Hybrid SPECT/CT and PET/CT Imaging: The Next Step in Noninvasive Cardiac Imaging

The past few years have witnessed impressive advances in the field of noninvasive cardiac imaging. For example, computed tomography (CT) coronary angiography has been adopted into daily clinical routine and, at least in some patient populations, is challenging the role of invasive angiography as the anatomic standard of reference. This is because the latter is associated with a nonnegligible periprocedural morbidity and mortality, which suggests confining its use to patients who will benefit from a revascularization procedure. Many factors that are beyond the quantification of anatomic narrowing and therefore cannot be fully appreciated with morphologic assessment will eventually determine whether or not a given lesion produces stress-induced ischemia. Myocardial perfusion imaging with single-photon emission computed tomography (SPECT) is among the most widely used and well-established noninvasive tools for the diagnosis of ischemic coronary disease. It allows assessment of the physiological relevance of coronary lesions and offers a high prognostic predictive value. Although positron emission tomography (PET) may achieve a higher accuracy than SPECT, its use has so far often been limited to large centers. Recent advances in image processing software and the advent of hybrid scanners have paved the way for fusion of image datasets from different modalities. An ideal, noninvasive technique for the diagnosis of coronary artery disease should provide complementary information on coronary anatomy as well as on pathophysiologic lesion severity. SPECT/CT and PET/CT hybrid imaging can provide such unique information, which not only improves diagnostic assessment and risk stratification but may also guide decision making with regard to revascularization in patients with coronary artery disease. Summary: In this study, a comparison was made between [N-13]-ammonia-PET and cardiac magnetic resonance (CMR) in evaluating left ventricular volumes and function. The results indicate that LVEF and LV wall motion can be accurately assessed using [N-13]-ammonia-PET, although LV volumes are significantly underestimated compared to CMR.

Summary: This study found that a radiomic feature (GLRLM_GLNU) extracted from visually normal 13N-ammonia PET retention images can independently predict reduced global myocardial flow reserve. This concept could potentially be applied to other myocardial perfusion imaging modalities based on relative distribution patterns to improve the detection of diffuse disease.

Non-Invasive Assessment of Endothelial Shear Stress in Myocardial Bridges Using Coronary Computed Tomography Angiography

Summary: Myocardial bridging (MB) is a segment of coronary arteries that is usually free from atherosclerosis, while the adjacent proximal segment is prone to it due to local endothelial shear stress (ESS). This study used coronary computed tomography angiography and computational fluid dynamics to analyze the ESS in coronary arteries with MBs and investigate the association between atherosclerosis and hemorheological characteristics. The results showed that there was no significant difference in ESS between the proximal 5 mm to MBs and the region within MBs. No significant ESS variances were observed around the MB entrance in arteries with plaques, regardless of the morphological features of the bridged segment. MBs have homogeneous, atheroprotective ESS, which may explain the absence of atherosclerosis within bridged segments. The interplay between ESS and atherosclerosis is potentially similar in arteries with MB compared to those without bridges.

Summary: Diagnosing angina pectoris and dyspnea in patients with normal or nonobstructive coronary vessels is challenging. Invasive coronary angiography can identify nonobstructive coronary artery disease, but coronary microvascular dysfunction may actually be the cause of symptoms. Positron emission tomography (PET) allows noninvasive detection and delineation of coronary microvascular dysfunction by quantifying myocardial blood flow at rest and during vasodilation. Individualized or intensified medical therapies may improve symptoms, quality of life, and outcomes. Standardized diagnosis and reporting criteria for ischemic symptoms caused by coronary microvascular dysfunction are crucial for treatment decisions. A consensus document proposes standardized diagnosis, nomenclature, and reporting criteria for coronary microvascular dysfunction. It aims to provide an overview of its pathophysiology, assessment, and classification, which are essential for diagnosis, patient care, and clinical trials. It was found that both tube voltage and reconstruction type significantly affected PCAT(MA), and it is recommended to use the same tube voltage and reconstruction type in multicenter and longitudinal studies.

Summary: This cohort study analyzed 5-year data from an international registry to investigate the associations between baseline aspirin and statin use and mortality, major adverse cardiovascular events, and myocardial infarction in individuals without substantial stenosis. The results showed that in nonobstructive CAD, baseline statin use was associated with improved clinical outcomes, while baseline aspirin use was not.
