The pathogenesis of autoimmune hepatitis entails complex interactions between triggering factors, autoantigens, genetic predisposition, and immunoregulatory networks. Implicated triggering factors are numerous and Include toxins, medications, and infectious agents. In this article, we present a unique case of a 31-year-old woman with severe autoimmune hepatitis apparently abruptly triggered by vaccination. All evidence suggests this was an idiosyncratic response to either hepatitis A or yellow fever vaccination.

Summary: The study identified that alcohol-related liver disease, decompensated cirrhosis, and hepatocellular carcinoma are risk factors predicting higher mortality in patients with chronic liver disease and COVID-19, while Hispanic ethnicity and decompensated cirrhosis are associated with severe COVID-19.

Summary: The study developed a DBU phenotyping algorithm that efficiently identified potential HCV treatment candidates and improved treatment completion rates through navigation and coordination. Findings showed that DBU patients who could not be contacted were less likely to receive treatment, with over half of them having significant fibrosis.

Summary: The TEAM-C project aims to evaluate the effectiveness of telemedicine treatment for HCV among those with opioid use disorder, with the goal of improving access to healthcare resources for hepatitis C patients. Through rigorous study design and implementation, the project has provided a new treatment option for this specific population, demonstrating feasibility and engagement principles through the initial randomized trial of telemedicine targeted at a vulnerable population.

The impact of COVID-19 on trends in alcohol use disorder treatment in Veterans Health Administration

Ponni V. V. Perumalswami, Sidonie Kilpatrick, Madeline C. C. Frost, Megan A. A. Adams, Hyungjin Myra Kim, Lan Zhang, Lewei Lin (Allison)

Summary: This study examined the trends of alcohol use disorder (AUD) treatment in the Veterans Health Administration (VHA) in the United States before and during the COVID-19 pandemic. The results showed that the percentage of VHA patients with an AUD diagnosis receiving AUD treatment declined during COVID-19 mainly due to a decrease in psychotherapy. Despite an increase after the start of COVID-19, the percentage of patients receiving AUD treatment remained lower than the pre-COVID-19 period at the end of the study.

Definition and measurement of alcohol-associated insight in early liver transplantation for acute alcohol-associated hepatitis: A systematic review

Gerald Scott Winder, Erin G. Clifton, Anne C. Fernandez, Mark MacEachern, Sarah Andrews, Ponni Perumalswami, Andrea F. DiMartini, Jessica L. Mellinger

Summary: Alcohol is closely related to liver disease and transplantation. Clinical evaluations and decisions in this area are challenging due to the complexity of psychosocial assessments and interpretation of psychiatric and substance use disorder data. Acute alcohol-associated hepatitis (AAH) patients have unique characteristics such as recent or active substance use, high mortality rate, psychiatric comorbidities, and urgent evaluation and treatment timelines. This systematic review highlights the lack of standardized definition and measurement of alcohol-associated insight (AAI) in the evaluation and decision-making process for AAH patients undergoing early liver transplantation (eLT).

Reexamining psychosocial clearance: A procedural framework for psychosocial evaluation in liver transplantation

Gerald Scott Winder, Anne C. Fernandez, Ponni V. Perumalswami, Jessica L. Mellinger, Erin G. Clifton

Summary: Psychosocial and nonmedical phenomena are common in liver transplantation evaluations and pose challenges to clinicians. There is a lack of consensus among clinicians and centers regarding psychosocial policies and practices. This article proposes a framework for psychosocial evaluation procedures, identifies common pitfalls, and suggests practical improvement strategies.
