Effectiveness of second-generation antipsychotics with acute-phase schizophrenia

Purpose: Although olanzapine may have advantages over other second-generation antipsychotics (SGAs) regarding longer time to treatment discontinuation among chronically ill patients, little evidence has been provided for the comparative effectiveness of SGAs in the acute phase. We aimed to determine if any of four SGAs were more effective in treating newly admitted acute schizophrenic patients. We performed a rater-blinded, randomized controlled trial of four SGAs in 15 psychiatric emergency sites. Eligible patients were 18-64 years old and met diagnostic criteria for schizophrenia, acute schizophrenia-like psychotic disorder, or schizoaffective disorder. A final total of 78 patients were randomly assigned by means of sealed envelopes to receive risperidone (3-12 mg/day; n = 20), olanzapine (10-20 mg/day; n = 17), quetiapine (300-750 mg/day; n = 20), or aripiprazole (12-30 mg/day; n = 21), with follow-up at 8 weeks. The primary outcome measure was all-cause treatment discontinuation. Results: Overall, 37% (29/78) of patients discontinued the study medication before 8 weeks: 25% for risperidone; 12% for olanzapine; 55% for quetiapine; and 52% for aripiprazole. Time to treatment discontinuation for any cause was significantly longer in the olanzapine group than in the quetiapine (p = 0.006) or aripiprazole (p = 0.008) groups, but not compared to the risperidone group (p = 0.32). Time to treatment discontinuation was significantly longer in the risperidone group than in the quetiapine group (p=0.048), but not compared to the aripiprazole group (p = 0.062). However, the rate of p.r.n. intramuscular haloperidol use was significantly higher in the aripiprazole group than in other groups (p = 0.029). Conclusion: Olanzapine and risperidone are superior to quetiapine and aripiprazole for the acute treatment of psychosis in hospitalized patients. (C) 2009 Elsevier B.V. All rights reserved.

Summary: This study aimed to investigate the characteristics of patients with schizophrenia and major depressive disorder who receive psychotropic pro re nata (PRN) prescriptions. The results showed that the proportion of psychotropic PRN prescriptions was lower in elderly patients with schizophrenia and higher in female patients with major depressive disorder. In addition, there was an association between psychotropic PRN prescriptions and regular use of multiple psychotropic medications in schizophrenia.

Summary: This study investigated the molecular biological changes in the fetal hippocampus exposed to haloperidol (HAL) as a model of schizophrenia. The results showed significant gene expressional changes in the HAL group compared with the control group. The study also predicted that miR-137-3p possibly regulates the expression of Nr3c1 gene, and observed a significant decrease in miR-137-3p expression in the hippocampus of the first generation.

Dissecting early life stress-induced adolescent depression through epigenomic approach

Summary: Early life stress (ELS), such as abuse and neglect during childhood, can result in psychiatric disorders in adulthood. Limited studies have investigated the epigenetic modifications associated with ELS and psychiatric disorders in adolescents, and the mechanisms are not fully understood. This review critically evaluates the current evidence on DNA methylation and microRNA expression as epigenetic modifications in adolescents with ELS and clarifies the impact of ELS on psychiatric disorders in order to predict and prevent neuropsychiatric disorders through personalized medicine.

Summary: The EGUIDE project was launched in 2016 to disseminate, educate, and validate psychiatric clinical practice guidelines. This study compared the effectiveness of web-based courses and face-to-face courses offered by the project. The results showed that web-based courses were rated as more satisfying than expected and there were no significant differences in satisfaction and clinical knowledge between the two formats. This suggests that web-based courses could be widely adopted alongside face-to-face courses after the COVID-19 pandemic.

Potential of Circulating miRNAs as Molecular Markers in Mood Disorders and Associated Suicidal Behavior

Summary: Mood disorders, such as major depressive disorder (MDD) and bipolar disorder (BD), are prevalent psychiatric disorders that can lead to significant disability and increased risk of suicide. Biomarker studies, especially in circulatory miRNAs, have shown potential for better diagnosis and personalized treatment of these disorders. Understanding the role of miRNAs in managing neuropsychiatric conditions has advanced our knowledge of prognosis, diagnosis, and treatment response. This review highlights the potential of circulatory miRNAs as screening tools for assessing MDD, BD, and suicidal behavior.

Prevalence of Comorbid Dementia in Late-life Depression and Bipolar Disorder: A Retrospective Inpatient Study

Summary: This study investigates the prevalence of dementia in patients with late-life major depressive disorder (MDD) or bipolar disorder (BD) and identifies the clinical characteristics associated with the diagnosis of dementia. The results show that the prevalence of dementia is significantly higher in MDD compared to BD, and the MDD group with dementia has older age of onset, more severe early insomnia symptoms, higher insight, and lower recall. These findings contribute to the diagnosis of dementia in late-life depression.

Motor Functional Characteristics in Attention-Deficit/Hyperactivity Disorder and Autism Spectrum Disorders: A Systematic Review

Summary: This review examines the physical abilities of individuals with attention-deficit/hyperactivity disorder (ADHD) and autism spectrum disorders (ASDs) compared to normal individuals. The study identifies three types of physical activities/exercises and finds that people with ADHD/ASDs generally exhibit poorer physical abilities, such as temporal discoordination of movement in ADHD and integration or synchronization of separate movements in ASDs. The specific deficits in physical ability attributed to ADHD/ASDs require further investigation.
