Summary: This study aimed to compare clinical features and indicators of disease progression in hospitalized patients with community-acquired pneumonia caused by Legionela pneumophila and bacteremic Streptococcus pneumoniae. The study found that variables that may distinguish Legionnaires' disease from Streptococcus pneumoniae include male gender, diarrhea, hyponatremia, higher temperature on admission, higher BMI, and fewer comorbidities. Bacteremic Streptococcus pneumoniae was associated with poorer outcomes than Legionnaires' disease including higher rates of septic shock, mechanical ventilation, ICU admission, and death.

Summary: This study aimed to evaluate the changes in pathogens causing hospital-acquired bacterial pneumonia (HABP), their antimicrobial resistance patterns, and the risk factors for 28-day all-cause mortality in patients with HABP. The annual incidence of Staphylococcus aureus HABP decreased while that of Klebsiella pneumoniae HABP increased significantly. The resistance rate of S. aureus to methicillin decreased, while the non-susceptibility rate of K. pneumoniae to carbapenems increased. HABP caused by certain pathogens and preceded by COVID-19 was associated with increased mortality and incidence of bacteraemic pneumonia.

Summary: This study retrospectively reviewed liver biochemistry data of patients infected with different human coronaviruses, finding that ALT/AST elevation was common in COVID-19 patients and independently associated with adverse clinical outcomes. Use of lopinavir-ritonavir, with or without ribavirin, interferon beta and/or corticosteroids was also independently associated with ALT/AST elevation.

Summary: Malnutrition, including undernutrition and obesity, has an impact on the risk and prognosis of adults with CAP. Undernutrition is associated with unfavorable clinical outcomes, while obesity is generally linked to higher survival rates in CAP patients. However, the etiology of CAP and the influenza A (H1N1) infection may influence these outcomes. Further larger-scale studies are needed to understand the physiological mechanisms behind these effects.

Summary: This study aimed to define the clinical presentation and outcomes of immunocompromised adults (ICA) hospitalized with pneumococcal pneumonia (PP) and compare the data to non-immunocompromised adults (non-ICA) hospitalized with PP. The results showed that in hospitalized patients with PP receiving appropriate antibiotic therapy, the presence of an abnormal immune system did not significantly impact clinical outcomes.

Summary: This study compared the clinical features and 60-day ICU all-cause mortality between monobacterial and polybacterial MDR Klebsiella spp. VAP episodes. The results showed that polybacterial episodes had higher mortality rates and were associated with factors such as less-severe disease and extended drug-resistant Klebsiella spp. Monobacterial episodes were associated with prior hospitalization, diabetes mellitus, obesity, prior antibiotic therapy, prior treatment with cephalosporins, and specific symptoms at VAP onset. Patients with polybacterial VAP were more likely to be comatose and had higher SAPS II scores at VAP onset.

Summary: Myositis associated ILD is a commonly under-recognized condition. This multicenter retrospective study identified 75 patients with myositis associated-ILD and found that specific ILD radiographic patterns can indicate underlying inflammatory myopathies. Incorporating these radiological patterns in the diagnostic criteria may improve timely interventions and positively impact patients' survival.

Summary: This study found that PCN allergy label is associated with worse clinical outcomes in bacterial pneumonia, including increased risks of hospitalization, acute respiratory failure, intubation, intensive level of care, and mortality. Patients with PCN allergy label had decreased use of PCNs and cephalosporins and increased utilization of other antibiotic classes. PCN allergy label was also associated with increased risk of adverse drug reactions.
