Objective. Patients with RA suffer from a higher risk of periodontal attachment loss and increased oral inflammation. We hypothesize that there are pathogenetic and immunological interactions between these diseases that go beyond impaired manual dexterity accompanying advanced RA. The primary objective of the present study was to determine whether a loss of alveolar bone can be detected in RA patients during the early course of the disease. Methods. In this cross-sectional, epidemiological case-control study, 22 patients with early RA (ERA) were compared with 22 matched healthy controls. Oral and periodontal status, clinical activity, and sociodemographic parameters were determined. Oral microbiota were analysed using real-time quantitative PCR specific for leading oral pathogens. Results. More advanced forms of periodontitis were found in ERA patients compared with controls. ERA patients had a greater number of missing teeth [ERA 5.7 (S.D. 5.0), controls 1.9 (S.D. 1.0), P = 0.002], deeper periodontal pockets [clinical attachment level: ERA 3.4 (S.D. 0.5 mm), controls 2.7 (S.D. 0.3 mm), P < 0.000], and greater bleeding on probing [ERA 18.6% (S.D. 9.0%), controls 10.5% (S.D. 5.1%), P = 0.001] despite comparable oral hygiene. Tannerella forsythia (6.77-fold, P = 0.033) subgingivally and Streptococcus anginosus (3.56-fold, P = 0.028) supragingivally were the characteristic pathogens in ERA. Conclusion. Increased loss of periodontal attachment and alveolar bone can be detected in patients with ERA, therefore we propose that the consulting rheumatologists inform the patients that they have a higher risk of periodontal disease. It would be beneficial if these patients were referred directly for intensive dental care.

Summary: Both type 1 and type 2 diabetes mellitus are increasingly prevalent worldwide and are major causes of morbidity and mortality. The TUDID study aims to investigate the causes of individual susceptibility to diabetes-related complications by conducting a comprehensive examination and laboratory tests on adult diabetes mellitus patients in a southern German university hospital. The study focuses on the kidneys, eyes, vasculature, as well as cognition and mood, with standardized investigations for early stages of complications. The analysis of the data generated from this study will contribute to our understanding of the development and progression of diabetes-related complications and aid in the implementation of tailored treatment options to reduce their risk and severity.

Restorative CAD/CAM materials in dentistry: analysis of their fluorescence properties and the applicability of the fluorescence-aided identification technique (FIT)

Christian Klein, Matthias Krespach, Sebastian Spintzyk, Diana Wolff, Christiane von Ohle, Christian Meller

Summary: The study found that most CAD/CAM materials showed little to no total fluorescence in the 395 nm - 415 nm wavelength spectrum used for FIT, except for a few exceptions with fluorescence values similar to tooth hard substances. Resin-based CAD/CAM materials generally exhibited higher fluorescence values compared to tooth hard substances, suggesting their potential suitability for FIT.

A randomized clinical split-mouth trial of a bulk-fill and a nanohybrid composite restorative in class II cavities: Three-year results

Summary: This randomized controlled clinical trial compared the clinical survival and quality parameters of a bulk-fill composite resin and a conventional nanohybrid composite resin in class-II restorations. The results showed that both materials had similar clinical performance and survival rates, indicating that bulk-fill composite resin is a satisfactory alternative in the posterior dentition.

How to Measure Adherence to a Mediterranean Diet in Dental Studies: Is a Short Adherence Screener Enough? A Comparative Analysis

Summary: This study evaluated the application of the Mediterranean Diet Adherence Screener (MEDAS) in a study of the anti-inflammatory effect of a Mediterranean diet intervention on periodontal parameters. The results showed that the MEDAS score was negatively correlated with periodontal inflammation and positively correlated with certain nutrient intakes. The food frequency questionnaire (FFQ) and 24-hour dietary recall (24dr) produced comparable results for most nutrients.

Maintaining oral health for a hundred years and more?-An analysis of microbial and salivary factors in a cohort of centenarians

Caroline Sekundo, Eva Langowski, Diana Wolff, Sebastien Boutin, Cornelia Frese

Summary: This study aimed to investigate the association between oral health-related conditions and the oral microbiome in a sample of centenarians. The findings showed that the diversity and structure of the oral microbiome were influenced by the presence and condition of natural teeth. Additionally, regular dental visits and salivary pH were found to have significant effects on the microbial composition. Most centenarians had hyposalivation and acidic pH, which could negatively impact the microbiome diversity.

Summary: This study reveals that clinical isolates of early colonizers of dental plaque can phenotypically adapt to antiseptics such as chlorhexidine digluconate (CHX) upon repeated exposure. Some strains showed increased biofilm formation capacity and exhibited antibiotic resistance to tetracycline and erythromycin.

Summary: This study examined the relationship between serum PUFA levels, specific food intake, and oral inflammatory parameters in individuals on the Mediterranean diet. The findings suggest that adherence to the Mediterranean diet can decrease serum omega-6 levels, which may have a favorable effect on gingival inflammatory parameters.

Summary: This study aimed to evaluate the long-term clinical quality of subgingivally placed composite resin restorations and their impact on inflammatory status of surrounding supracrestal gingival and periodontal tissues. Results showed no increased inflammation was observed on sites with subgingivally placed composite restorations over an observation period of approximately 3 years. Regular use of interdental brushes was associated with less gingival inflammation.
