Summary: This review article examines the use of medical cannabis for chronic nonmalignant pain management. Recent research shows that medical cannabis can effectively manage symptoms of various conditions including cancer, chronic pain, headaches, migraines, and psychological disorders. The active ingredients THC and CBD in cannabis work through the endocannabinoid system to reduce pain sensation and symptom frequency. Medical cannabis provides convenient and effective pain management for patients with chronic nonmalignant pain.

Summary: This study aimed to evaluate the efficacy and adverse effects of methadone as a first-line therapy in patients receiving low doses of opioids or none. Results showed that methadone provided good analgesia with limited adverse effects and minimal opioid-induced tolerance.

Summary: This article aims to evaluate the effectiveness of oxycodone for acute dental pain. The systematic review and meta-analysis found that oxycodone, when combined with paracetamol, provides better analgesia. However, compared to non-steroidal anti-inflammatory drugs, oxycodone is less effective.

Summary: QLB significantly reduced postoperative opioid consumption, pain scores, time to first request for rescue analgesic, and incidence of PONV in women undergoing cesarean section. However, the quality of evidence for most results was still low to moderate.

Summary: Although multimodal management is recommended for chronic noncancer pain (CNCP), little is known about the long-term treatment utilization patterns among individuals using opioids. This study conducted in Australia found that many Australians using opioids for CNCP also used nonopioid pharmacological and nonpharmacological treatments. However, some treatment use was inconsistent with guidelines and the use of nonpharmacological treatments was associated with having private health insurance.

Summary: Many primary care clinics are resistant to accepting new patients taking prescription opioids for chronic pain, particularly those perceived to have aberrant opioid use. The study found that this resistance could limit access to primary care for patients using opioids long-term, especially for those with histories suggestive of aberrant use.

Summary: This study investigates the variables associated with multimorbidity status among older US adults with self-reported pain and opioid use. The study finds that individuals aged 50-64, Hispanic, employed, and those who exercise frequently have lower odds of having multimorbidity.

Summary: After one-month follow-up, cancer patients experienced a lower number of breakthrough pain episodes and peak intensity, potentially due to optimization of background analgesia. The principal characteristics of breakthrough pain did not change significantly, highlighting the importance of careful and continuous assessment for patients to alleviate the burden induced by breakthrough pain episodes.

Summary: Prescription opioids are commonly used for chronic pain conditions, but long-term therapy can lead to risks of addiction and overdose. Studies have shown that opioid addiction and chronic pain conditions involve genetic and epigenetic alterations, but the contribution and mechanisms are not fully understood.

Summary: Cebranopadol has been evaluated as a potential solution to the side effects associated with opioids. By acting on the mu opioid receptor and the nociceptin/orphanin receptor, cebranopadol can reduce the side effects of opioids while still providing analgesia. This review examines the effects, pharmacokinetics, and clinical trials of cebranopadol to explore its promise in pain management.

Summary: In patients receiving low doses of opioids, the prevalence of breakthrough cancer pain (BTcP) was high, with many patients experiencing dissatisfaction with BTcP medications, especially oral morphine. Optimizing background analgesia may help limit the number of BTcP episodes.

Summary: Pain is a common issue among older people, but it is often underestimated and not properly treated. A comprehensive assessment is necessary to identify the cause, severity, impact, and treatment response of pain. Managing pain in older individuals requires a different approach due to frailty, multimorbidity, polypharmacy, sensory deficits, and cognitive impairment. This review provides a summary of a comprehensive approach to pain management in older people.

Summary: This commentary reviews a recent trial on the relationship between COMT genotype and opioid dose requirements, and discusses the potential role of COMT and other genes (such as CYP2D6) in opioid therapy. It also provides an overview of the current evidence for germline pharmacogenetics and available resources for opioid pharmacogenetics.

Summary: This study suggests that omitting step 2 of the WHO ladder in cancer pain management and using low-dose strong opioids as an alternative can be more efficient and cost-effective, with fewer side effects.

Summary: The mechanisms underlying the transition from acute to chronic pain are still unclear, but it may involve the persistence or strengthening of pain memories acquired through associative learning. Recent studies have shown that contextual cues play a critical role in regulating pain memory. Animals and humans exhibit increased pain sensitivity in environments associated with painful experiences. This study suggests that pain perception and activation of endogenous opioid systems can be modified through psychological association with environmental cues.
