Obesity has become a very frequent condition with important consequences for the health of affected individuals. Current evidence shows that the excess of adipose tissue as observed in obesity is responsible for secreting inflammatory mediators in a deregulated manner, thus inducing a chronic state of systemic low-grade inflammation that underlies the metabolic and cardiovascular outcomes in these populations. This article reviews the state of the art regarding mediators produced in the adipose tissue, their roles in the pathophysiology of obesity-associated insulin resistance and diabetes, and finally, tries to build a bridge between these mechanistically oriented insights and clinical practice.

Needle Technology for Insulin Administration: A Century of Innovation

Summary: Innovations in syringe and pen needle technology have significantly improved insulin delivery for people with diabetes. Advances in needle design, including shorter lengths and finer gauges, have led to improved injection experience. Coupled with education on proper injection technique, these advancements contribute to better clinical outcomes and patient satisfaction. Insulin syringes and pens will continue to be reliable and cost-effective means of insulin delivery, especially in developing countries.

Assessment of Two Different Glucagon Assays in Healthy Individuals and Type 1 and Type 2 Diabetes Patients

Summary: Methods for glucagon analysis in the past were not specific and had a narrow sensitivity range, leading to inaccurate results. Newer methods have shown that actual glucagon levels are lower than previously thought. This study compared glucagon levels in healthy individuals and diabetes patients using two different methods and found that the choice of method can significantly affect the results.

Summary: This study analyzed the safety and efficacy of dasiglucagon, a glucagon analogue, in treating severe hypoglycemia in patients with type 1 diabetes. Summary: The effects of fasting on glucose metabolism were examined in non-obese and obese participants and people with type 2 diabetes. The results showed that after 36 hours of fasting, glucose, insulin, and C-peptide levels were significantly decreased. Non-obese participants had higher glucose levels but lower insulin levels after 36 hours of fasting, while no significant differences were observed in obese participants or people with diabetes. Insulin sensitivity improved in all cohorts after 36 hours of fasting. Metabolomics analysis revealed subtle baseline differences and attenuated metabolic response to fasting in obese participants and people with diabetes.

Hypoglycaemia frequency and physiological response after double or triple doses of once-weekly insulin icodec vs once-daily insulin glargine U100 in type 2 diabetes: a randomised crossover trial

Summary: This study compared the frequency and recovery of hypoglycemia after double or triple doses of once-weekly insulin icodec and once-daily insulin glargine U100. The study also compared the symptomatic and hormonal responses to hypoglycemia between the two treatments. The results showed that there was no significant difference in the risk of hypoglycemia between icodec and glargine U100, but icodec elicited slightly higher symptomatic and endocrine responses during hypoglycemia compared to glargine U100.

Summary: The aim of this study was to determine the acceptability and psychometric properties of the Hypo-METRICS app, which is designed to assess the impact of hypoglycemia on daily functioning in people with insulin-treated diabetes. Participants completed daily check-ins for 10 weeks using the app and answered questions about their subjective daily functioning. The results showed high completion rates, satisfactory psychometric properties, and demonstrated that the app is acceptable and reliable for exploring the daily impact of hypoglycemia.

Perioperative Insulin Regimens in Patients With Insulin-Treated Type 2 Diabetes Mellitus Hospitalized for a Short Time for Minor Eye Surgery

Summary: For patients with insulin-treated type 2 diabetes mellitus undergoing elective surgery, a standardized insulin and meal regimen on the day of surgery can lead to better management of diabetes in terms of blood glucose levels. Both standardized and well-defined insulin and meal regimens resulted in better average blood glucose levels in the perioperative period, especially in the morning after the surgery.

Summary: This study compared the use of continuous glucose monitoring (CGM) with self-monitored blood glucose (SMBG) in insulin-treated diabetic pilots certified to fly commercial aircraft. The results showed that CGM is a credible alternative to SMBG for monitoring glucose levels.

AMH in Males: Effects of Body Size and Composition on Serum AMH Levels

Summary: Serum concentrations of AMH decrease with increasing BMI, possibly due to either an adverse effect of adiposity on AMH production or dilution from greater blood volume. We investigated the relationships between AMH levels and body composition parameters in adult males and found that body weight, lean mass, and body surface area were better predictors of AMH than measures of adiposity. Since both lean mass and body surface area correlate with plasma volume better than adipose tissue, we conclude that hemodilution of AMH does occur in adult males and should be considered for normalization in future studies.
