Purpose: To determine factors affecting the visual outcome of eyes with endogenous Candida endophthalmitis. Methods: Retrospective cohort study of 44 eyes from 36 patients diagnosed with candida endophthalmitis at 2 tertiary referral uveitis centers. Outcome measures included the development of retinal detachment and the occurrence of visual loss (visual acuity of <20/40) and severe visual loss (visual acuity of <20/200). Results: Twenty four of 44 eyes (55%) had visual loss and 16 of 44 eyes (32%) had severe visual loss by the end of the study. Early vitrectomy significantly reduced the risk of retinal detachment (P = 0.02). Factors associated with poor visual outcome included poor presenting visual acuity (relative risk = 2.38; 95% confidence interval, 1.01-5.55; P < 0.05) and centrally located fungal lesions (relative risk = 5.01; 95% confidence interval, 1.00-2.52; P < 0.05). Conclusion: Candida endophthalmitis is associated with a high rate of visual loss, particularly in patients with poor presenting visual acuity or centrally located lesions. Early vitrectomy reduces the risk of retinal detachment. RETINA 32:1129-1134, 2012

The Use of Sustained Release Intravitreal Steroid Implants in Non-Infectious Uveitis Affecting the Posterior Segment of the Eye

Didar Abdulla, Yasser Ali, Victor Menezo, Simon R. J. Taylor

Summary: The development of sustained-release corticosteroid implants has revolutionized the treatment of non-infectious uveitis affecting the posterior segment of the eye. Their use is now supported by healthcare systems based on high-quality randomized controlled trials, providing clinicians with information on their benefits and risks.

Bilateral macular retinitis in patients with presumed rift valley fever from Sudan: A case series

Yousef A. Fouad, Mahmood O. Mekkawy, Ahmed B. Sallam

Summary: This study retrospectively reviewed three cases of post-febrile retinitis in patients traveling from Sudan, with a history and clinical picture suggestive of Rift Valley fever (RVF) retinitis. The study presents multimodal imaging data and highlights the challenges in diagnosing RVF due to the lack of standardized testing methods for most epidemic retinitis-causing pathogens.

Intravitreal Triamcinolone Acetonide Presents a Diagnostic Challenge on Noncontrast Computed Tomography

Kinza T. Ahmad, Michelle L. Huynh, Ramakrishnaiah Raghu, Mohamed Kamel Soliman, William Henry, Ahmed Sallam

Summary: This case report describes an individual who developed eyelid edema and corneal erosions after treatment with triamcinolone acetonide for tractional retinal detachment caused by diabetes. A computed tomography scan and clinical observations confirmed the association between the symptoms and the triamcinolone acetonide injection.

Syphilis-related Ocular Inflammation in HIV-positive and HIV-negative Patients

Victoria Ly, Abdelrahman M. Elhusseiny, Ahmed B. Sallam

Summary: This study evaluated the clinical characteristics and visual outcomes of ocular syphilis in HIV-positive and negative patients. The results showed that posterior uveitis was the most common ophthalmic feature, and neurosyphilis was relatively high in both groups. There were no significant differences in laterality, presence of concurrent rash, RPR titers, and visual acuity between the two groups.

Persistent Subretinal Fluid Following Successful Rhegmatogenous Retinal Detachment Surgery

Yousef A. Fouad, Ahmed M. Habib, Riley N. Sanders, Ahmed B. Sallam

Summary: This article provides an updated critical summary of literature on persistent subretinal fluid (PSF) following successful rhegmatogenous retinal detachment surgery. The incidence rates of PSF vary significantly, and there is no conclusive evidence for its etiology or risk factors. PSF usually resolves gradually without affecting final visual acuity, and observation is currently the preferred management approach for uncomplicated cases.

Current and Novel Therapeutic Approaches for Treatment of Diabetic Macular Edema

Muhammad Z. Chauhan, Peyton A. Rather, Sajida M. Samarah, Abdelrahman M. Elhusseiny, Ahmed B. Sallam

Summary: Diabetic macular edema (DME) is a complex ocular complication of diabetes mellitus, and its treatment includes diabetes management, laser photocoagulation, and anti-VEGF therapies. Anti-VEGF therapies have shown significant clinical improvements compared to laser therapy alone.

Visual outcomes and complications of combined vs sequential cataract surgery and pars plana vitrectomy: multicenter database study

Abdelrahman M. Elhusseiny, Mohamed K. Soliman, Ahmed F. Shakarchi, Yousef A. Fouad, Yit C. Yang, Ahmed B. Sallam

Summary: This study compared the visual outcomes and rates of intraoperative complications between combined cataract extraction and pars plana vitrectomy (combined group) and sequential surgery (sequential group). The results showed that the postoperative visual acuity was worse in the combined group and there was a higher rate of posterior capsular rupture. However, the differences in visual improvement between the two groups decreased with longer follow-up time.

Summary: Eyes undergoing phacoemulsification complicated by posterior capsule rupture (PCR) and dropped lens fragments (DLFs) have worse visual outcomes and higher chances of developing cystoid macular edema (CME), epiretinal membrane (ERM), and rhegmatogenous retinal detachment (RRD) compared to eyes with uneventful surgery.

Summary: This study aims to analyze the impact of axial length (AL) on the visual outcome and rate of perioperative complications in phacoemulsification surgery. The data from 217,556 eyes undergoing cataract surgery were analyzed. The study found that AL did not have a significant impact on visual acuity improvement or the risk of encountering certain complications, but short AL eyes had a higher risk of torn iris and long AL eyes had a higher risk of retinal detachment.

Summary: This study aimed to evaluate the effectiveness of digital visualization in enhancing the visualization of iridocorneal structures during surgical gonioscopy. The images of 26 cases of trabecular stent implantation were recorded with standard colors and optimized digital settings, and were evaluated subjectively by two glaucoma surgeons and objectively through contrast measurements. The optimized digital settings, including the cyan filter and warmer color, were found to enhance the visibility of trabecular meshwork pigmentation and Schlemm's canal, making them useful in minimally invasive glaucoma surgery.

Summary: This study aimed to determine the risk factors for progression in patients with Posner-Schlossman syndrome. The study found that patients with this syndrome have a high relapse rate, and factors such as older age, immunocompromise, chronic elevation of intraocular pressure, and presence of glaucomatous optic neuropathy are associated with increased risk of disease progression.
