Changes in interleukin-10 mRNA expression are predictive for 9-day survival of pigs in an emergency preservation and resuscitation model

Aim of the study: This study aimed at evaluating (I) the impact of different intra-arrest hypothermia levels on the expression of selected cytokines and (II) their prognostic value for 9-day survival. Methods: Female Large White pigs (n = 21, 31-38 kg) were subjected to 15 min of ventricular fibrillation, followed by intra-arrest cardiopulmonary bypass cooling for 1, 3, or 5 min achieving brain temperatures (Tbr) of 30.4 +/- 1.6, 24.2 +/- 4.6 and 18.8 +/- 4.0 degrees C. After 40 min of controlled rewarming, pigs were defibrillated and kept at Tbr of 34.5 degrees C for 20 h, survival was for 9 days. Plasma samples were analysed for interleukin (IL)-6, tumor necrosis factor-alpha (TNF-alpha), and IL-10 levels by ELISA. Total RNA out of peripheral blood mononuclear cells was analysed by real-time PCR for IL-1, IL-2, IL-4, IL-10, TNF-alpha, interferon-gamma, inducible NO synthase, and heme oxygenase-1 gene expressions. Results: Plasma IL-6 and TNF-alpha levels significantly (p = 0.0001 and 0.0003) increased in all animals within 1 h after resuscitation with no significant differences between groups. Pigs surviving exhibited a decrease in IL-10 expression between baseline and intra-arrest values as compared to non-surviving animals, which showed a slight increase (p = 0.0078). ROC curve analysis revealed that changes in IL-10 expression had a good prognostic power for survival to day 9 (area under the curve = 0.882). Conclusion: The systemic inflammatory response syndrome after cardiac arrest was reflected by a remarkable increase of plasma IL-6 and TNF-alpha levels. Intra-arrest hypothermia levels did not influence the expression of selected cytokines. As prognostic marker for survival IL-10 was identified with decreasing mRNA levels during cardiac arrest in survivors. (c) 2010 Elsevier Ireland Ltd. All rights reserved.

Immune checkpoints and liver resection after neoadjuvant chemotherapy including bevacizumab in patients with microsatellite-stable colorectal liver metastases

Summary: This study retrospectively investigated the clinical value of immune checkpoint expression as prognostic biomarkers in patients with resected microsatellite-stable colorectal liver metastases who were pretreated with bevacizumab. The results showed that the expression of IDO-1 and CTLA-4 were associated with treatment response and survival, suggesting their potential as predictive biomarkers in this patient population.

Summary: This study found that patients aged 50-65 who underwent bioprosthetic aortic valve replacement (B-AVR) had lower long-term survival rates and higher risks compared to mechanical aortic valve replacement (M-AVR). B-AVR also had higher risks of reoperation and myocardial infarction. However, the risk of heart failure and stroke did not significantly differ after AVR in any age group.

Perioperative Supplemental Oxygen and Plasma Catecholamine Concentrations after Major Abdominal Surgery-Secondary Analysis of a Randomized Clinical Trial

Summary: Perioperative stress can cause increased heart rate and blood pressure, leading to perioperative myocardial ischemia. Previous studies have shown that supplemental oxygen can reduce sympathetic nerve activity. This study aimed to investigate whether perioperative supplemental oxygen can attenuate sympathetic nerve activity in patients at risk for cardiovascular complications. The results indicate that supplemental oxygen during surgery does not appear to affect endogenous catecholamine release.

The Effect of Pregabalin on the Minimum Alveolar Concentration of Sevoflurane: A Randomized, Placebo-Controlled, Double-Blind Clinical Trial

Summary: The aim of this study was to investigate the effect of two different doses of pregabalin on the minimum alveolar concentration (MAC) of sevoflurane. The researchers found that a dose of 300mg pregabalin significantly reduced the MAC of sevoflurane, while a dose of 150mg had a less significant effect. Pregabalin use led to a slight reduction in post-operative pain levels but increased side effects as the dose increased.

Impact of adaptive filtering on power and false discovery rate in RNA-seq experiments

Summary: The study shows that filtering can improve the power of RNA-sequencing data, but overly aggressive filtering may lead to a decrease in power. There is no uniformly optimal filter, and different filters may be optimal depending on the scenario. An adaptive filtering strategy is proposed to maximize the number of rejections.
