Prevalence of chronic obstructive pulmonary disease in Korea: The fourth Korean National Health and Nutrition Examination Survey, 2008

Background and objective: Because the mortality and social burden associated with COPD is increasing, repeated surveys of the prevalence of COPD have been used to assess risk factors, detect potential patients, and establish early diagnoses and management protocols. We report the prevalence of spirometrically detected COPD in Korea in 2008, using data from the fourth Korean National Health and Nutrition Survey. Methods: Using nationwide stratified random sampling, based on the Korean Statistical Office census, 6840 subjects aged >= 19 years underwent spirometry, which was performed by four trained technicians, during 2008. The place of residence, levels of education and income and smoking status, as well as other results from a COPD survey questionnaire were also assessed. Results: Airflow obstruction (FEV1/FVC < 0.7) was detected in 8.8% of subjects aged >= 19 years (11.6% of men and 5.9% of women) and COPD was detected in 13.4% of subjects aged >= 40 years (19.4% of men and 7.9% of women). Of the 6840 subjects, 27.3% were current smokers and 17.2% were former smokers, and the prevalence of COPD was higher in current and former smokers than in never smokers. Approximately 94% of patients with COPD had mild-to-moderate disease, without apparent symptoms; only 2.4% had been diagnosed by a physician and only 2.1% of patients had been treated. The independent risk factors for COPD were smoking, advanced age and male gender. Conclusions: Although the prevalence of COPD in Korea is high, the disease is underdiagnosed and most COPD patients are under-treated.

The Relationship Between Comorbidities and Microbiologic Findings in Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Summary: There is limited data on the relationship between microbiological features and comorbidities in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). This multicenter observational study in South Korea aimed to correlate microbiological findings with comorbidities in moderate to severe AECOPD patients. The study found that bacterial infections and co-infections with bacteria and viruses were more prevalent in patients with pulmonary comorbidities, while bacterial pathogens were detected at a higher rate in patients with extrapulmonary comorbidities. Previous pulmonary tuberculosis, bronchiectasis, and diabetes mellitus were identified as risk factors for bacterial infection, and congestive heart failure was a risk factor for bacterial and viral co-infection. The risk of bacterial infection increased significantly with the number of comorbidities. Pseudomonas aeruginosa was more frequently identified in patients with previous pulmonary tuberculosis and bronchiectasis. Overall, AECOPD patients with comorbidities had a higher risk of infection-related exacerbations compared to those without comorbidities.

Incidence and outcomes of adverse drug reactions to first-line anti-tuberculosis drugs and their effects on the quality of life: A multicenter prospective cohort study

Clinical Outcomes and Prognosis of Patients With Interstitial Lung Disease Undergoing Lung Cancer Surgery: A Propensity Score Matching Study

Summary: We compared the prognosis after lung cancer surgery between 104 patients with ILD and 104 patients without ILD who were matched by key variables. The findings showed that patients with ILD had worse survival rates, especially those with concomitant IPF or high physiological severity of ILD. Evaluating the underlying ILD before surgery is considered helpful in predicting the prognosis after lung cancer surgery.

Characteristics and risk factors of mortality in patients with systemic sclerosis-associated interstitial lung disease

Summary: This study aimed to identify risk factors for mortality and compare the clinical characteristics of patients with systemic sclerosis-associated interstitial lung disease (SSc-ILD). The study found that approximately 10% of SSc-ILD patients showed disease progression, and ILD was detected within less than five years from the first visit. Therefore, careful monitoring of symptoms and signs from an early stage and long-term surveillance are necessary.

Clinical features and molecular genetics associated with brain metastasis in suspected early-stage non-small cell lung cancer

Aspergillus Galactomannan Titer as a Diagnostic Marker of Invasive Pulmonary Aspergillosis in Lung Transplant Recipients: A Single-Center Retrospective Cohort Study

Summary: The potential of Aspergillus galactomannan antigen titer (AGT) in serum and bronchoalveolar lavage fluid (BALF) and serum titers of beta-D-glucan (BDG) in predicting invasive pulmonary aspergillosis (IPA) were investigated in lung transplantation recipients. The cutoff values for AGT were found to be 0.560 in serum and 0.600 in BALF, with sensitivities and specificities ranging from 50% to 85%, and AUCs ranging from 0.724 to 0.895. A lower cutoff value of 1.0 was suggested when IPA was highly suspicious. Serum and BALF AGT showed a correlation with a history of diabetes mellitus.

Identification of Biomarkers for the Diagnosis of Sepsis-Associated Acute Kidney Injury and Prediction of Renal Recovery in the Intensive Care Unit

Summary: Purpose: AKI following sepsis is associated with higher mortality, but reliable biomarkers for AKI development and recovery remain unclear. Materials and Methods: Patients with sepsis in the medical ICU were analyzed. They were divided into AKI and non-AKI groups within 48 hours. Patients with septic AKI were further divided into recovery and non-recovery groups within 7 days. Results: Baseline creatinine, Comorbidity Index, Organ Failure Assessment, and Acute Physiology and Chronic Health Evaluation II scores were higher in the non-recovery AKI group. Non-recovery from AKI was associated with lower survival. High-lactate and kynurenine levels were associated with non-recovery of renal function following AKI. Conclusion: Patients who recovered from AKI had comparable mortality to those without AKI. Lactate and kynurenine could be useful biomarkers for the diagnosis and recovery of AKI following sepsis.

NADPH oxidase 4 signaling in a ventilator-induced lung injury mouse model

Summary: This study investigated the role of NOX4 in ventilator-induced lung injury (VILI) and found that NOX4 knockout, EphA2 knockout, or monoclonal antibodies could attenuate VILI. Furthermore, NOX4 and EphA2 levels were significantly higher in patients with pneumonia and those receiving mechanical ventilation in the intensive care unit. Inhibition of NOX4 may be a potential therapeutic target for the prevention and reduction of VILI.
