Summary: The study investigated humoral immunity in ILD associated with rheumatoid arthritis, Sjogren's syndrome, and mixed connective tissue disease by analyzing bronchoalveolar fluid and serum samples from 15 patients. The researchers found active autoimmunity in the lungs of these patients, with the presence of disease-related autoantibodies and changes in target modification. This suggests a complex immune response and epitope spreading may occur in lungs of autoimmune disease associated-ILD patients.

Summary: Autoimmune rheumatic diseases can affect vital organs, with lung involvement being severe and difficult to treat. Rituximab is an off-label treatment for CTD-ILD, but studies have shown that it can effectively improve pulmonary function and reduce ILD symptoms.

Summary: Connective tissue diseases (CTD) are frequently associated with interstitial lung disease (ILD), significantly impacting morbidity and mortality. This study analyzed the experience of an autoimmune specialized unit in treating CTD-ILD and characterized the patient population based on common diseases, imaging patterns, lung function tests results, serology, and treatment. It also assessed mortality and mortality predictors, with pulmonary hypertension identified as the most important predictor. The study highlights the importance of chest physicians in these specialized units.

Summary: This study aimed to identify inflammatory factors and soluble cytokines that act as biomarkers in the diagnosis and prognosis of rheumatoid arthritis-associated interstitial lung disease (RA-ILD). The study found that patients with RA-ILD had higher inflammatory activity and certain cytokines were associated with both diagnosis and poorer prognosis in these patients.

Summary: This study retrospectively reviewed medical records of 72 patients with RA-ILD and found that increased peripheral monocyte count was significantly associated with poor prognosis. Patients with high monocyte and neutrophil counts had lower survival rates, suggesting their potential as cellular biomarkers to predict outcomes in RA-ILD patients.

Summary: This review summarizes and analyzes the etiology and pathogenesis of rheumatoid arthritis-associated interstitial lung disease (RA-ILD). The incidence of RA-ILD increases with age and is higher in men, and is also associated with lifestyle factors such as smoking and exposure to pollutants. The presence of anti-cyclic citrullinated peptide antibody, high RA disease activity, and rheumatoid factor positivity increase the risk of RA-ILD.

Summary: Antisynthetase syndrome is a subtype of idiopathic inflammatory myopathy that is strongly associated with interstitial lung disease. Diagnosis is made by detecting specific antibodies and relevant clinical and radiologic features. A multidisciplinary team, including rheumatologists, respiratory physicians, and radiologists, is needed for accurate diagnosis.

Summary: The study found that about half of patients with interstitial lung disease associated with systemic autoimmune disease stabilize or improve after a median of 5 years, but over one-third of patients die. Rheumatoid arthritis is associated with higher rates of disease progression and hospitalization in ILD, while usual interstitial pneumonia pattern, FVC%, and smoking are independent predictors for worsening of ILD-SAI.

Summary: This retrospective study aimed to identify predictors of interstitial lung disease (ILD) in patients with mixed connective tissue disease (MCTD). The study found that gastroesophageal involvement and lymphadenopathy at disease onset were associated with ILD. Additionally, it established that lymphadenopathy at disease onset and older age at diagnosis independently predicted ILD in MCTD patients.

Summary: This nationwide multicentre study prospectively evaluated 376 patients with IIP, finding that 18.6% of them met the IPAF criteria. Patients with IPAF were more likely to develop systemic autoimmune diseases, had fewer acute exacerbations of IIP, and showed better survival outcomes. Cluster analysis identified a subgroup with more autoimmune features, predominantly female, and with a better prognosis.

Summary: This multicenter study investigated the impact of end-of-life respiratory modalities on quality of dying and death (QODD) and symptom relief among patients dying with interstitial lung disease (ILD). The results showed that high-flow nasal cannula (HFNC) was associated with higher QODD and symptom relief ratings from bereaved families, suggesting it may improve outcomes for patients dying in a hospital setting with end-stage ILD.

Summary: We report a case of eosinophilic pneumonia induced by trimethoprim-sulfamethoxazole (TMP-SMX) in a 27-year-old woman. Radiological examination showed bilateral nonsegmental airspace consolidation resembling cryptogenic organizing pneumonia. Eosinophil infiltration was observed in lung specimens and there was marked eosinophilia in the peripheral blood and bronchoalveolar lavage fluid. Discontinuation of TMP-SMX improved eosinophilia and radiological abnormality, confirming the association between TMP-SMX use and onset of eosinophilic pneumonia. TMP-SMX induced eosinophilic pneumonia is rare, but clinicians should be aware of the possibility during TMP-SMX administration.
