Summary: This study found that MPA-ILD represents a distinct phenotype with poor prognosis. A lower %FVC was identified as an independent prognostic factor, associated with increased mortality and risk of developing AE in MPA-ILD patients. Specific management strategies for MPA-ILD and AE should be established in the future.

Summary: This study retrospectively analyzed data from patients with RA-ILD and IPF, and found that AE is not uncommon in both diseases. Hypoalbuminemia and %DLCO were identified as independent risk factors for AE in RA-ILD patients. AE was also the most frequent cause of death in both RA-ILD and IPF patients.

Summary: This study reports two cases of acute exacerbation of preexisting interstitial pneumonia associated with mRNA COVID-19 vaccination. The lung disease was stable before vaccination, but initial steroid therapy showed unfavorable responses and further treatment was required. The study suggests that clinicians should be aware of the possibility of acute exacerbation of pneumonia in patients with interstitial lung disease after mRNA COVID-19 vaccination and monitor them closely.

Summary: This article reports on the pathogenesis and characteristics of a rare disease called TAFRO syndrome. TAFRO syndrome is characterized by renal dysfunction, fever, reticulin fibrosis, anasarca, thrombocytopenia, and organomegaly. The article also discusses the complications of this disease, including interstitial pneumonia. Based on the patient's clinical presentation and laboratory test results, the diagnosis was TAFRO syndrome with acute exacerbation of interstitial pneumonia.

Summary: This study found that there was no significant difference in 1-year survival between patients undergoing lung transplantation during AE-ILD and those transplanted with stable disease. Patients with AE-ILD who underwent transplantation had higher survival rates compared to those who were medically treated.

Summary: This study analyzed the clinical characteristics and prognosis of acute exacerbation (AE) in patients with idiopathic pulmonary fibrosis (IPF) and pulmonary emphysema. Among the 62 patients included, 46% had concomitant pulmonary emphysema. Patients with IPF and emphysema had lower 90-day mortality rate and longer median survival time compared to those with non-emphysematous IPF.

Summary: This study aimed to investigate the treatment outcomes of infectious etiology in acute exacerbation (AE) of myositis-related interstitial lung disease (ILD). The findings revealed that infectious AE is an important cause of mortality in these patients, showing a similar risk of mortality as non-infectious AE.

Summary: This study aimed to investigate the risk factors for AE of interstitial lung disease caused by chemotherapy for lung cancer. Through meta-analysis, it was found that age, FVC, CT pattern, and serum SP-D levels were significant risk factors for AE in these patients. Therefore, caution should be taken when administering chemotherapy to lung cancer patients with interstitial lung disease based on these risk factors.

Summary: Steroid pulse therapy for drug-induced hypersensitivity syndrome/drug reaction with eosinophilia and systemic symptoms carries risks of cytomegalovirus reactivation, persistency, and high mortality, compared to conventional treatment, while showing lower rates of herpesvirus 6 reactivation or type 1 diabetes. This suggests that the administration mode may differentially impact inflammatory responses and associated consequences. Based on the study findings, steroid pulse therapy should be avoided for these severe adverse reactions.

Summary: The nutritional status may affect the efficacy of nivolumab in NSCLC patients. A post hoc analysis of 158 NSCLC patients receiving nivolumab showed that low Geriatric Nutritional Risk Index (GNRI) was associated with shorter progression-free survival and overall survival. GNRI can predict the efficacy of immune checkpoint inhibitor therapy.

Summary: This study aimed to identify educational needs for preventing severe immune-related adverse events (irAEs) that result in unscheduled hospitalizations. The study found that the frequency of severe irAEs requiring hospitalization was high in patients with advanced lung cancer receiving PD-1/PD-L1 inhibitors. The main educational needs were focused on early recognition of potentially severe common irAEs and the importance of reporting changes in symptoms.

Summary: Carboplatin plus etoposide is a standard treatment for older extensive-stage small-cell lung cancer (ES-SCLC) patients with performance status (PS) 2. The therapeutic efficacy and safety of this treatment in older patients with poor PS (3, 4) is poorly understood. We retrospectively analyzed 63 patients with ES-SCLC aged >= 71 years and PS >= 2 who received first-line carboplatin plus etoposide therapy to determine their treatment outcomes. Our results showed that carboplatin plus etoposide therapy provided comparable tumor shrinkage in older ES-SCLC patients with PS >= 3 compared to those with PS 2, but resulted in shorter progression-free survival and overall survival. Supportive care, such as prophylactic G-CSF administration, may be necessary to ensure safety and survival.
