Description and Microbiology of Endotracheal Tube Biofilm in Mechanically Ventilated Subjects

BACKGROUND: A biofilm is found on the inner side of endotracheal tubes (ETT) in mechanically ventilated patients, but its features and role in pneumonia remain unclear. METHODS: This prospective, observational, monocentric study included critically ill ventilated subjects. Measurement of the ETT inner volume was first performed before exhibation using the acoustic reflection method. After extubation, the biofilm was studied by means of optical and atomic force microscopy. Bacteriological analysis was then performed and compared with clinical documentation. RESULTS: Twenty-four subjects were included. Duration of intubation lasted from 2 to 79 d (mean +/- SD: 11 +/- 15 d). The mean percentage of ETT volume loss evaluated in situ (n = 21) was 7.1% and was not linked with the duration of intubation. Analyses with atomic force microscopy (n = 6) showed a full coverage of the inner part of the tube with biofilm, even after saline rinse. Its thickness ranged from 0.8 to 5 mu m. Bacteriological cultures of the biofilm (n = 22) often showed the same bacteria as in tracheal secretions, especially for pathogenic organisms. Pseudomonas aeruginosa and Candida albicans were among the most frequent microorganisms. In subjects who had experienced a successfully treated episode of ventilator-associated pneumonia (n = 5), the responsible bacteria were still present in the biofilm. CONCLUSIONS: ETT biofilm is always present in intubated patients whatever the duration of intubation and appears quickly after intubation. Even after soft rinse, a small but measurable part of biofilm remains always present, and seems strongly adherent to the ETT lumen. It contains potentially pathogenic bacteria for the lung.

Objective diagnosis of internal nasal valve collapse by four-phase rhinomanometry

Zwitterionic Polymers toward the Development of Orientation-Sensitive Bioprobes

Summary: Dynamics with orientational freedom are fundamental in biological events. Lanthanide-doped nanocrystals with polarized luminescence provide precise analysis and are more stable than quantum dots. However, the polarization property often deteriorates in complex physiological environments. In this study, we demonstrate a surface functionalization method for lanthanide-doped nanorods using zwitterionic block copolymers. The coated nanorods exhibit high colloidal stability, preserved shear-aligned ability, strongly polarized emission, and good biocompatibility. This study validates the strategy of coupling zwitterions to lanthanide-doped nanocrystals for various bioapplications.

Delivery of high flow oxygen through nasal vs. tracheal cannulas: A bench study

Oncostatin M Contributes to Airway Epithelial Cell Dysfunction in Chronic Rhinosinusitis with Nasal Polyps

Summary: This study investigated the presence of OSM and IL-6, as well as the expression pattern of tight junctions, in CRSwNP patients and controls. The results showed that OSM and IL-6 were overexpressed, and tight junction (ZO-1 and occludin) expression was decreased in nasal polyps compared to control mucosa. In vitro experiments further demonstrated that OSM could significantly reduce tight junction expression, transepithelial electric resistance, and ciliary beating efficiency, while promoting cell repair mechanisms.

Summary: In early 2020, the emergence of SARS-CoV-2 in Wuhan, China caused a global health emergency. The virus enters cells through the ACE2 protein and TMPRSS2 facilitates membrane fusion. TMPRSS2 is regulated by AR signaling, and inhibiting it with apalutamide reduces SARS-CoV-2 entry and infection in lung and nasal cells. These findings suggest that apalutamide could be a treatment option for PCa patients at risk for severe COVID-19.

Summary: In this study, we evaluated the effects of delay after sampling and the temperature for conservation of respiratory ciliated samples on assessments of ciliary beating. We found that ciliary beating can be evaluated at 37 degrees Celsius up to 9 hours after nasal brushing, but the storage temperature can modify ciliary beating.

A novel method for assessment of airway opening pressure without the need for low-flow insufflation

The results showed that compared to synchronized intermittent positive airway pressure, non-invasive neurally adjusted ventilatory assist significantly improved patient-ventilator synchrony.
