A high sensitivity assay is more accurate than a classical assay for the measurement of plasma CRP levels in endometriosis

Background: Endometriosis is associated with chronic subclinical inflammation. C-reactive protein (CRP), a marker of inflammation, could serve as a biomarker of endometriosis. We tested the hypothesis that a high sensitivity CRP assay (hsCRP) is more accurate than a classical CRP assay in the detection of subclinical inflammation in plasma of women with endometriosis. Methods: CRP levels were measured by hsCRP and classical CRP assays in plasma of 204 women with endometriosis and 91 women without endometriosis. Both assays were compared with respect to their value for the diagnosis of endometriosis. Results: The number of plasma samples with detectable CRP was significantly higher (100%) using the hsCRP assay when compared to the classical CRP assay (42.7%) (p < 0.0001). Significantly increased CRP plasma levels were found in women with endometriosis when compared with controls when the hsCRP assay was used in samples obtained during the luteal phase (p = 0.008). The highest discriminative ability for the diagnosis of endometriosis was also obtained using the hsCRP assay during the luteal phase, especially for moderate -severe endometriosis. At a cut-off level of hsCRP > 0.71 mg/L, moderate-severe stages were diagnosed with 80.7% sensitivity and 63.9% specificity during the luteal phase. Using a similar cut-off value for CRP analyzed by the classical method, moderate-severe endometriosis was diagnosed with lower sensitivity (67.7%, p = 0.06) and comparable specificity (63.9%). Conclusions: The hsCRP assay was superior to the classical CRP assay for the detection of low CRP levels and for revealing subclinical inflammation in plasma of women with endometriosis.

TRP channel expression correlates with the epithelial-mesenchymal transition and high-risk endometrial carcinoma

These findings suggest that TRP channels could serve as biomarkers for high-risk endometrial cancer and potential targets for treatment.

The impact of chronic endometritis on infertility: prevalence, reproductive outcomes, and the role of hysteroscopy as a screening tool

Summary: This study aimed to investigate the prevalence of chronic endometritis (CE) in infertile women, its impact on reproductive outcomes, and the accuracy of hysteroscopy as a screening tool for CE. The study included 514 asymptomatic patients with infertility who underwent hysteroscopy and endometrial biopsy (EMB). CE was identified in 2.8% of patients starting assisted reproductive technologies (ART). The clinical pregnancy rate (CPR) did not differ significantly between patients with CE and the entire cohort of patients without CE, but was significantly lower in the 2 years after EMB in patients with CE compared to matched controls. Hysteroscopy had low sensitivity and high specificity in diagnosing CE.

Summary: Long-read amplicon sequencing is a simple and cost-effective strategy for preclinical preimplantation genetic testing in couples with a de novo pathogenic variant. This approach allows for the identification of the parental origin of the mutant allele and provides access to universal genome-wide haplotyping-based PGT programs. The success rate of this method is 75%.

The need for longitudinal data on the value of fertility treatment for infertile patients' quality of life: A review and pilot study

Summary: Infertility can have a negative impact on the life goal of parenthood and the quality of life of individuals, and the process of fertility clinics can be burdensome. This review and pilot study aimed to explore the effects of the pre-IVF fertility clinic trajectory on patient-reported outcomes measures (PROMs) for emotional well-being and quality of life. Findings showed that the diagnostic workup reduced infertility-specific distress in men, but there were conflicting results regarding its effects on the anxious and depressive reactions of (wo)men. Intrauterine insemination (IUI) was found to increase depressive reactions in (wo)men. However, more research is needed on infertility-specific, health-related, and overall quality of life. The pilot study also indicated that the overall quality of life of (wo)men was not affected by the diagnostic workup but decreased by the time of the third IUI. Longitudinal studies on the impact of starting the fertility clinic trajectory on PROMs are crucial for patient-centered clinical and policy decision-making.

Experiences of nonpregnant couples after receiving reproductive genetic carrier screening results in Belgium

Summary: The aim of this study was to assess the satisfaction, anxiety, knowledge retention, psychosocial and counseling-related aspects among couples who chose reproductive genetic carrier screening (RGCS). The findings showed that all participants felt they had enough information to make an informed choice and none of them regretted their decision to have RGCS. The study demonstrated that the information/counseling and reporting strategy used in this context led to high participant satisfaction, increased knowledge, and positive psychosocial and counseling-related outcomes.

Impact of Indication for Oocyte and Embryo Reception on Reproductive and Obstetric Outcomes

Summary: This study aimed to assess the impact of the indication for oocyte reception (OR) or embryo reception (ER) on reproductive and obstetric outcomes. The results showed that the indication for OR/ER seems to have little impact on the outcomes, except for patients with premature ovarian insufficiency (POI) due to Turner syndrome or treatment with chemotherapy/radiotherapy.

Summary: Early embryo implantation involves a complex process of communication between the blastocyst and the endometrium. Proteases secreted by the blastocyst stimulate calcium signaling pathways in endometrial epithelial cells. This study identified protease-activated receptor 2 (PAR2) as the molecular initiator of calcium responses in endometrial epithelial cells and revealed the downstream signaling pathways involved.

Summary: This study compared the cumulative pregnancy rates of supernumerary embryos vitrified on Day 3 and Day 5/6. The results showed no significant difference between the two groups. It is known that extended embryo culture and blastocyst transfer on Day 5/6 yield higher pregnancy and live birth rates in IVF or ICSI treatment. However, this study focused on vitrification for cryopreservation of supernumerary embryos, rather than the traditional slow freezing protocol.
