Summary: An analysis of the German nationwide inpatient sample from 2005 to 2016 found that hospitalizations for heart failure increased during this period, while the inhospital case fatality rate and major adverse cardiovascular and cerebrovascular event rate decreased. Independent predictors of inhospital death included age 70 years and older and a diagnosis of cancer.

Summary: Cardiorenal syndrome (CRS) is a complex and heterogeneous spectrum of symptoms that has posed challenges for cardiologists for many years. The coexistence of heart failure (HF) and deteriorated renal function adds to these challenges and is a strong predictor of poor outcomes. However, with the development of new renal assessment methods, we now have the opportunity to treat CRS more effectively than ever before. This review focuses on explaining the pathophysiology, recent advances, and current practices of monitoring renal function in patients with acute CRS, aiming to improve patient care and guide treatment strategies.

Summary: Cardiorenal syndrome is a broad spectrum disorder affecting the heart and kidneys. India has a high burden of acute cardiorenal syndrome, with 46.1% of patients diagnosed in India. Acute cardiorenal syndrome involves sudden kidney deterioration in acute heart failure patients. The pathophysiology includes hyperactivation of the sympathetic nervous system and the renin-angiotensin-aldosterone system. Complications increase the risk of mortality, making it a global healthcare burden. Current biomarkers have limitations in early detection, highlighting the need for novel proteomic biomarkers to prevent progression.

The acute-to-chronic glycemic ratio is significantly associated with mortality in subjects hospitalized for acute HF, with improved predictive ability compared to glucose levels at admission or average chronic glucose concentrations, especially in subjects with diabetes.

Summary: Despite significant progress in managing chronic heart failure, the treatment for acute heart failure remains largely unchanged. Hospitalization for acute decompensation of heart failure is primarily due to fluid overload. Intravenous loop diuretics are the main therapy, but some patients show suboptimal response, leading to incomplete decongestion upon discharge. Combination diuretic therapy, using loop diuretics with an add-on agent, is commonly used to counter renal sodium avidity. However, the evidence for this strategy is not strong, and recent studies have renewed interest in sequential nephron blockade.

In sheep with established heart failure, catheter-based RDN led to decreased heart rate, increased parasympathetic control, and improved kidney function, indicating potential benefits for both the heart and kidneys in heart failure.

Summary: This retrospective cohort study aimed to determine the incidence and predictive factors of acute kidney injury (AKI) after off-pump lung transplantation. The study included 148 adult patients who underwent lung transplant without cardiopulmonary bypass or extracorporeal membrane oxygenator at the Vanderbilt University Medical Center between 2006 and 2016. The results showed that AKI is a common complication after off-pump lung transplantation, associated with increased hospital length of stay, and independently associated with younger age, unilateral lung transplant, and diagnosis of chronic obstructive pulmonary disease.

Summary: The advances in cardiac surgical techniques have improved outcomes, but have also led to complications and mortality outside of the operating room. To address this issue, a focus on evidence-based practice and standardized perioperative care is needed. The POQI and ERAS (R) Cardiac have identified significant evidence gaps in areas related to cardiac surgery, including patient blood management, goal-directed therapy, acute kidney injury, opioid analgesic reduction, and delirium.
