Clinical perspectives of cancer stem cell research in radiation oncology

Radiotherapy has a proven potential to eradicate cancer stem cells which is reflected by its curative potential in many cancer types. Considerable progress has been made in identification and biological characterisation of cancer stem cells during the past years. Recent biological findings indicate significant inter- and intratumoural and functional heterogeneity of cancer stem cells and lead to more complex models which have potential implications for radiobiology and radiotherapy. Clinical evidence is emerging that biomarkers of cancer stem cells may be prognostic for the outcome of radiotherapy in some tumour entities. Perspectives of cancer stem cell based research for radiotherapy reviewed here include their radioresistance compared to the mass of non-cancer stem cells which form the bulk of all tumour cells, implications for image- and non-image based predictive bio-assays of the outcome of radiotherapy and a combination of novel systemic treatments with radiotherapy. Summary: This study investigated the relationship between hippocampal structure and memory deficits in patients with WHO grade 3 and grade 4 gliomas after radiotherapy. The results showed no changes in hippocampal thickness or cognitive abilities three months after completing radiotherapy. However, subjective memory impairment was associated with symptoms of depression, highlighting the importance of intervention to improve the quality of life for these patients.

Summary: The capacities of German Comprehensive Cancer Centers (CCCs) in various aspects of oncology care were significantly affected during the first 2 years of the COVID-19 pandemic. Follow-up, psycho-oncologic care, and tumor surgery were particularly impacted, along with other areas of multidisciplinary oncological care. This study highlights the importance of developing strategies to prevent similar limitations in the future.

Dynamics of CXCR4 positive circulating tumor cells in prostate cancer patients during radiotherapy

Summary: We investigated the circulating tumor cells (CTCs) of patients with metastatic prostate cancer, and found that the number of CTCs is associated with the time of biochemical progression after radiotherapy. Specifically, the expression of CXCR4 and CCL2 is related to cellular radiosensitivity, tumorigenicity, and stem-like potential.

Comprehensive Evaluation of Multiple Approaches Targeting ABCB1 to Resensitize Docetaxel-Resistant Prostate Cancer Cell Lines

Summary: This study investigated various approaches targeting ABCB1 to resensitize DTX-resistant prostate cancer cells. It was found that ABCB1 was the only transporter highly upregulated in the resistant cells, and its inhibition and knockdown completely reversed the resistance to DTX. In addition, partial reversal was achieved by inhibiting glycosylation.

Longitudinal and Multimodal Radiomics Models for Head and Neck Cancer Outcome Prediction

Summary: This study investigates the predictive ability of machine learning-based radiomics models using imaging data obtained during the course of treatment. Features were extracted from pre-treatment CT and FDG-PET images, as well as images obtained two and three weeks after the start of radiochemotherapy. Models comprised of combined features from both modalities and multiple timepoints were evaluated. The results show that predictive model performance is improved when features from in-treatment imaging are used.

Summary: This study suggests that the DNA methylation landscape of tumor cells may indicate epigenetic changes caused by chronic tumor hypoxia. The researchers developed a tumor hypoxia classifier (Hypoxia-M) based on DNA methylation profiles, which showed independent prognostic effects in patients with head and neck squamous cell carcinoma (HNSCC). These findings underscore the importance of DNA methylation-based classifiers as biomarkers of tumor hypoxia.

Preclinical trial comparing radiotherapy alone versus standard radiochemotherapy in three human papilloma virus (HPV) negative and three HPV-positive head and neck squamous cell carcinoma (HNSCC) xenograft tumour models

Summary: This study aimed to compare the efficacy of fractionated radiotherapy versus radiochemotherapy with cisplatin in HPV-positive and negative human head and neck squamous cell carcinoma xenografts. The results showed that RCT significantly increased local tumor control in two out of three HPV-positive tumor models and one HPV-negative tumor model. In the pooled group of all HPV-positive tumors, RCT substantially enhanced local tumor control, while this effect was not observed in HPV-negative tumors.

Immunotargeting of Cancer Stem Cells

Summary: Tumor cells exhibit functional heterogeneity, and cancer stem cells (CSCs) are a population of tumor cells with self-renewal and differentiation properties. CSCs are found in most solid and hematological tumors and are characterized by various markers. These markers can be used to develop tumor-specific antibodies, immune cells, vaccines, and immune responses to tumor cells, including CSCs. This review discusses CSC-directed immunotherapies, their clinical development, safety and efficacy improvements, and future strategies for enhancing anti-CSC immunotherapy.

Establishment and Molecular Characterization of an In Vitro Model for PARPi-Resistant Ovarian Cancer

Summary: We established an in vitro model for PARPi-resistant ovarian cancer and found that PARPi-resistant cells exhibit broad cross-resistance to other PARPis and chemotherapeutic drugs regardless of the BRCA1 status. Co-culture experiments showed that PARPi-resistant cells have a disadvantage in proliferation under drug-free conditions but quickly gain dominance under olaparib selection pressure, which can be blocked by ABC-transporter protein inhibitors. Overall, our well-characterized in vitro model is useful for dissecting mechanisms of PARPi resistance and studying the clonal dynamics of PARPi-resistant cells.

Summary: In this study, researchers aimed to establish a predictive model for radiation-induced brain injury (RBI) in nasopharyngeal carcinoma (NPC) patients by incorporating clinical factors and newly developed genetic variants. They conducted a large-scale retrospective study and a genome-wide association study to develop a polygenic risk score (PRS) for RBI risk prediction. The results showed that the PRS, combined with clinical factors, improved the accuracy of RBI risk stratification and suggested personalized radiotherapy.

The rise of negative portrayals of radiation oncology: A textual analysis of media news

The findings suggest an increasing negative sentiment in media coverage towards radiotherapy, with a shift towards reporting treatment errors, toxicity, and ineffectiveness.
