Summary: Based on high-throughput sequencing of the Gene Expression Omnibus database, the study identified the association between rheumatoid arthritis-related usual interstitial pneumonia (RA-UIP) and biological processes such as oxidative phosphorylation. The analysis also revealed differential gene expression and immune regulation characteristics in the lung tissues of RA-UIP compared to idiopathic pulmonary fibrosis-associated UIP.

Summary: This study retrospectively analyzed data from patients with RA-ILD and IPF, and found that AE is not uncommon in both diseases. Hypoalbuminemia and %DLCO were identified as independent risk factors for AE in RA-ILD patients. AE was also the most frequent cause of death in both RA-ILD and IPF patients.

Summary: Respiratory complications are common in rheumatoid arthritis (RA), with interstitial lung disease being the most common manifestation. Recent progress has been made in understanding the management and progression of RA-associated interstitial lung disease (RA-ILD) and other respiratory diseases in RA. In this review, we will update the diagnosis and treatment of RA-associated ILD, methotrexate-associated lung disease, and the complication of Pneumocystis jiroveci pneumonia in RA from a chest physicians' perspective.

Summary: This study investigated the potential of Periostin as a biomarker for rheumatoid arthritis-associated interstitial lung disease (RA-ILD). The results showed that all biomarkers were higher in RA-ILD patients compared to healthy controls and RA patients without ILD. KL-6 accurately detected ILD in RA patients, while SP-D and Periostin moderately detected ILD. Periostin levels were correlated with lung fibrosis but not inflammatory areas. Additionally, Periostin was higher in RA-ILD patients with a typical usual interstitial pneumonia pattern. Immunohistochemical analyses showed that Periostin was expressed in fibroblastic foci. These findings suggest that Periostin can be used as a biomarker for diagnosis, evaluating fibrosis, and deciding therapeutic strategies for RA-ILD patients.

Summary: The study aimed to describe comorbid conditions in patients with rheumatoid arthritis-associated interstitial lung disease (RA-ILD) and analyze factors associated with multimorbidity. The results showed that comorbid conditions were more frequent in RA-ILD patients compared to those with RA but not ILD. Age, inflammatory markers, and lung function were found to be associated with multimorbidity.

Summary: This study investigated the prevalence and clinical implications of coexistent pleuroparenchymal fibroelastosis (PPFE) in patients with rheumatoid arthritis-associated interstitial lung disease (RA-ILD). The results showed that PPFE was not rare in patients with RA-ILD and was significantly associated with an increased risk of pneumothorax and greater decline in lung function, although there was no significant association with mortality.

Summary: This study aimed to identify inflammatory factors and soluble cytokines that act as biomarkers in the diagnosis and prognosis of rheumatoid arthritis-associated interstitial lung disease (RA-ILD). The study found that patients with RA-ILD had higher inflammatory activity and certain cytokines were associated with both diagnosis and poorer prognosis in these patients.

Summary: This review summarizes and analyzes the etiology and pathogenesis of rheumatoid arthritis-associated interstitial lung disease (RA-ILD). The incidence of RA-ILD increases with age and is higher in men, and is also associated with lifestyle factors such as smoking and exposure to pollutants. The presence of anti-cyclic citrullinated peptide antibody, high RA disease activity, and rheumatoid factor positivity increase the risk of RA-ILD.

Summary: This study investigated the prevalence, incidence, and cause-specific mortality of RA-associated interstitial lung disease (RA-ILD) among older US patients with RA. The results showed that RA-ILD was present or developed in nearly 5% of patients, and RA-ILD was associated with excess total, respiratory, and cancer mortality compared with RA without ILD.

Summary: Pulmonary amyloidosis is a less common but clinically important manifestation of transthyretin amyloidosis (ATTR), characterized by deposition of misfolded transthyretin protein in the lung tissue. This study retrospectively reviewed the clinical course, imaging characteristics, pathology results, and outcomes of a patient cohort diagnosed with pulmonary ATTR. The findings highlight the importance of recognizing and understanding pulmonary involvement in ATTR.

Summary: A 79-year-old man with recurrent dyspnea and constitutional symptoms of malaise, fatigue, fevers, and arthralgias over the past 7 years was examined. Despite elevated acute phase reactants and inflammation markers, extensive evaluation did not reveal any identifiable autoimmune disease. Treatment with prednisone provided partial symptomatic improvement. Various cytopenias were present, but bone marrow biopsies did not show any diagnostic findings.

Summary: Combined pulmonary fibrosis and emphysema (CPFE) syndrome is a challenging and potentially life-threatening lung disease that requires accurate diagnosis and optimal management. This narrative review summarizes the literature on CPFE from 1990 to 2022, including its historical background, epidemiology, pathogenesis, clinical features, imaging and pulmonary function findings, diagnosis, prognosis, complications, and treatment. CPFE is not only associated with idiopathic pulmonary fibrosis, but also occurs in patients with various interstitial lung diseases, posing a heightened risk for pulmonary hypertension and lung cancer.
