The relevance of piroxicam for the prevention and treatment of nonmelanoma skin cancer and its precursors

Piroxicam (PXM), a nonsteroidal anti-inflammatory drug, is an enolic benzothiazine and a potent member of the oxicam series. The drug suppresses the synthesis of proinflammatory enzymes, such as cyclooxygenases-1 and -2 (COX-1 and 2), downregulates the production of prostaglandins (PGs) and tromboxanes, and inhibits polyamines production by blocking ornithine decarboxylase induction involved in nonmelanoma skin carcinogenesis. In addition, PXM is able to induce tumor cell apoptosis and suppresses metalloproteinase 2 activities. Skin carcinogenesis is a multistep process in which the accumulation of genetic events leads to a gradually dysplastic cellular expression, deregulation of cell growth, and carcinomatous progression. COX-1 upregulation plays a significant role in PG and vascular epidermal growth factor production supporting tumor growth. Increased level of PGs in premalignant and/or malignant cutaneous tumors is also favored by upregulation of COX-2 and downregulation of the tumor suppressor gene 15-hydroxy-prostaglandin dehydrogenase. Chemoprevention can be a hopeful approach to inhibit carcinoma occurrence before an invasive tumor develops. The chemopreventive effect of nonsteroidal anti-inflammatory drugs on nonmelanoma skin cancers has been established. In this study, we highlighted the different modalities of action of PXM on the pathogenesis of nonmelanoma skin cancer, analyzing and evaluating binding modes and energies between COX-1 or COX-2 and PXM by protein-ligand molecular docking. Our clinical experience about the local use of PXM on actinic keratoses and field cancerization is also reported, confirming its efficacy as target therapy.

This case report highlights the importance of considering lifestyle and cutaneous background before starting immunotherapy for cancer treatment, with a focus on pharmacogenomics and the possibility of modified skin microbiota predisposing to cutaneous infections in patients treated with PD-1 inhibitors.

Computational and experimental validation of phthalocyanine and hypericin as effective SARS-CoV-2 fusion inhibitors

Summary: In this study, computational simulations and experimental results supported the role of phthalocyanine and hypericin as potential inhibitors of SARS-CoV-2 entry into cells. The simulations showed their binding to key protein functional regions and their ability to insert in the viral membrane. In vitro experiments demonstrated strong inhibition of viral entry, consistent with previous literature on the efficacy of these compounds against SARS-CoV-2.

Role of Neutrophil-to-Lymphocyte Ratio (NLR) in Patients with Mycosis Fungoides

Summary: The neutrophil/lymphocyte ratio (NLR) is found to be associated with the severity and prognosis of various cancers, but its role in mycosis fungoides (MF) is unclear. This study aimed to assess the association of NLR with different stages of MF and determine if higher values are related to a more aggressive MF. The retrospective analysis of 302 MF patients found that higher NLR values were significantly associated with advanced stages of MF, suggesting that NLR can serve as a marker for advanced MF, guiding physicians in patient management.

A Real-Life Study on the Use of Tildrakizumab in Psoriatic Patients

Summary: Tildrakizumab is an approved treatment for moderate-to-severe plaque-type psoriasis, targeting the p19 subunit of interleukin IL-23. A study was conducted on 53 psoriatic patients who received Tildrakizumab every 12 weeks for 52 weeks, showing significant improvement in Psoriasis Area and Severity Index (PASI), Dermatology Life Quality Index (DLQI), Nail Psoriasis Severity Index (NAPSI), and Palmoplantar Psoriasis Physician Global Assessment (PPPGA). Disease remission was achieved from week 16 and maintained until week 52.

The Diagnosis and Management of Cutaneous Metastases from Melanoma

Summary: Melanoma, the deadliest form of skin cancer, accounts for almost 90% of skin cancer mortality. Despite advances in immune and targeted therapies, many patients still experience disease progression. Skin metastases, a common occurrence in melanoma, can be treated through various methods including surgery, injections, and systemic therapies. Further research is needed to understand the underlying mechanisms and differences in behavior and prognosis among melanoma patients with skin metastases.

Summary: The most promising method for monitoring patients with minimal morbidity is the detection of circulating melanoma cells (CMCs). We have shown that CD45(-)CD146(+)ABCB5(+) CMCs are a rare primitive stem/mesenchymal CMCs population associated with disease progression. Investigating the gene expression of isolated CMCs' epithelial-to-mesenchymal transition (EMT) gene signature can provide potential clinical value.

Summary: Ichthyoses are genetically determined cornification disorders of the epidermis. Phenotypic overlap among different genetic entities can occur due to variable penetrance of mutations. This study identified a family with mild phenotypes resembling erythrokeratodermia variabilis (EKV), but genetic analysis revealed double heterozygosity for two different mutations in the ABCA12 gene responsible for harlequin ichthyosis.

In Vivo Identification of H3K9me2/H3K79me3 as an Epigenetic Barrier to Carcinogenesis

Summary: The dynamic structure of chromatin, controlled by epigenetic alterations of histones and DNA, plays a crucial role in cellular plasticity and the rewiring of epigenetic landscape for cell differentiation or reprogramming. This study investigates the epigenetic switch in cancer cells by analyzing Histone 3 (H3) modifications during osteogenic differentiation and examining methylation states of H3K27, H3K9, and H3K79 in different types of tumors. The findings suggest that the decreased levels of H3K9me2 and H3K79me3 in cancer specimens, compared to healthy tissues, may serve as important epigenetic barriers to tumorigenesis, making them potential diagnostic markers and therapeutic targets.

Summary: The study aimed to assess the association between CT scan results and MR-proADM levels in order to improve the prediction of in-hospital mortality of COVID-19 patients at the triage. The key role of CT score and MR-proADM levels in determining the correct clinical setting for these patients was also evaluated. The results showed that using CT score in combination with MR-proADM levels was particularly effective in risk stratification and decision-making in the emergency department, optimizing hospital resources.

Performance Evaluation of the New Chemiluminescence Immunoassay CL-1200i for HBV, HIV Panels

Summary: Infectious diseases like HIV and HBV have a global impact on public health and healthcare costs. Timely diagnosis plays a central role in controlling the spread of these infections. This study compared the performance of the Abbott system and the new analyser Mindray 1200i in detecting HBV and HIV infections. Clinical serum samples from randomly selected patients were tested and compared with the routine instrument, Abbott Architect. The results showed that the Mindray CL-1200i platform offers high-level performance with accurate and consistent test results, making it a valuable tool for routine analysis.
