Women with familial hypercholesterolemia phenotype are undertreated and poorly controlled compared to men

Alberto Zamora, Rafel Ramos, Marc Comas-Cufi, Maria Garcia-Gil, Ruth Marti-Lluch, Nuria Plana, Lia Alves-Cabratosa, Anna Ponjoan, Celia Rodriguez-Borjabad, Daiana Ibarretxe, Irene Roman-Degano, Jaume Marrugat, Roberto Elosua, Anabel Martin-Urda, Lluis Masana

Summary: Familial hypercholesterolemia (FH) is a common autosomal dominant disease associated with early coronary heart disease (CHD). A study of over 2 million individuals showed that women with FH are less likely to receive high-intensity lipid-lowering therapy (LLT) and have poorer adherence to LLT compared to men. Additionally, women have a lower probability of achieving their LDL-c goals, especially in secondary prevention.

Glycoprotein and Lipoprotein Profiles Assessed by 1H-NMR and Its Relation to Ascending Aortic Dilatation in Bicuspid Aortic Valve Disease

Borja Antequera-Gonzalez, Marta Faiges, Neus Martinez-Micaelo, Laura Galian-Gay, Carmen Ligero, Maria Ferre-Vallverdu, Lluis Masana, Nuria Amigo, Arturo Evangelista, Josep M. Alegret

Summary: This study analyzed and compared the lipoprotein and glycoprotein profiles in 152 patients with bicuspid aortic valve (BAV) and ascending aorta dilatation (AAoD), and found that parameters related to lipoprotein profile and a novel inflammation biomarker were significantly increased in the dilated group. In long-term follow-up, proatherogenic lipoprotein parameters were associated with ascending aorta dilation velocity. Therefore, BAV patients with AAoD have a more pro-atherogenic profile, which appears to contribute to the increased growth rate of ascending aorta diameter.

Tokgoezoglu, Alberico L. Catapano, Kausik K. Ray

Summary: This 2023 statement provides updated clinical guidance for the management of homozygous familial hypercholesterolaemia (HoFH), including criteria for diagnosis, genetic testing interpretation, and treatment recommendations. The statement emphasizes the importance of considering both phenotype and genotype in the diagnosis of HoFH, with a LDL-C level >10 mmol/L (>400 mg/dL) being indicative of the condition. Combination therapy targeting LDL-C, including the use of novel therapies like proprotein convertase subtilisin/kexin type 9 inhibitors, is recommended to achieve LDL-C goals and reduce the need for lipoprotein apheresis. To improve HoFH care globally, the statement recommends the implementation of national screening programs, education to increase awareness, and management guidelines adapted to local healthcare systems.

Lipoprotein profile assessed by 1H NMR, BMI and blood pressure are associated with vascular alterations in children with familial hypercholesterolaemia

D. Llop, A. Feliu, D. Ibarretxe, J. Escribano, N. Plana, C. Borjabad-Rodriguez, L. Masana, J. C. Vallve

Summary: This study examines the arterial structure and functional characteristics in children with familial hypercholesterolaemia (FH) and finds that alterations in lipoprotein parameters are associated with early arterial damage and stiffness. Body mass index (BMI) and blood pressure play a role in these associations. Therefore, early cardiovascular prevention should be implemented in children with FH, including adopting a healthy lifestyle.

Lipid lowering combination therapy: From prevention to atherosclerosis plaque treatment

Lluis Masana, Nuria Plana, Natalia Andreychuk, Daiana Ibarretxe

Summary: Statins have played a significant role in preventing atherosclerotic cardiovascular events and deaths by reducing LDL cholesterol levels. International guidelines recommend low LDL cholesterol goals for patients at high cardiovascular risk. However, these goals may not be attainable with statins alone. Recent trials have shown that nonstatin drugs like PCSK9 inhibitors, ezetimibe, bempedoic acid, and icosapent ethyl can also provide cardiovascular benefits. Physicians should use combination therapies based on a patient's risk and LDL cholesterol levels to achieve LDL cholesterol goals and improve cardiovascular outcomes.

Summary: This study aimed to explore the prognostic value of cardiac-specific fatty acid binding proteins (FABP3) and adipose-tissue-specific fatty acid binding proteins (FABP4) in chronic heart failure (CHF) patients, with and without type 2 diabetes mellitus (T2D). It was found that the concentrations of FABP3 and FABP4 were associated with all-cause and cardiovascular mortality, particularly in patients with T2D. The study suggests that serum FABP3 and FABP4 concentrations can serve as independent predictors for the occurrence of all-cause and cardiovascular mortality in ambulatory CHF subjects with T2D.
