Metabolite Triplet in Serum Improves the Diagnostic Accuracy of Prediabetes and Diabetes Screening

Summary: A novel diagnostic metabolite triplet (Met-T) consisting of hexose, 2-hydroxybutyric/2-hydroxyisobutyric acid, and phenylalanine has been identified through a novel knowledge-based, multistage discovery and validation strategy to significantly improve the diagnostic accuracy of broad-scale diabetes screening using easy-to-collect blood samples. The combination of Met-T and fasting plasma glucose showed higher diagnostic sensitivities for diabetes screening compared to FPG alone, indicating the potential for reducing missed diagnoses and improving the accuracy of diabetes screening.

Ex vivo instability of lipids in whole blood: preanalytical recommendations for clinical lipidomics studies

Summary: Reliability, robustness, and interlaboratory comparability of quantitative measurements in clinical lipidomics studies are crucial. The different ex vivo stability of lipids in blood poses a risk to data interpretation. Clear recommendations for blood sample collection are necessary. In this study, we used UHPLC-high resolution mass spectrometry to investigate the stability of 417 lipid species in EDTA whole blood under different temperature conditions and time points. Our data suggest that cooling whole blood immediately and continuously, and separating plasma within 4 hours, unless focusing on robust lipids, are recommended. Lists of lipids' ex vivo (in)stability and potential biomarkers in whole blood are provided. Overall, our results contribute to the international efforts in achieving reliable and comparable clinical lipidomics studies in the future.

Summary: The gut microbiome plays a significant role in human health and disease, but it is challenging to identify microbiota-associated molecules in human blood or urine. This study used a non-targeted metabolomics approach and found that depletion of the fecal microbiome affected the levels of metabolite ions in urine. The study also annotated 32 compounds associated with the fecal microbiota.
