The role of adult tissue-derived stem cells in chronic leg ulcers: a systematic review focused on tissue regeneration medicine

Wound healing is an articulated process that can be impaired in different steps in chronic wounds. Chronic leg ulcers are a special type of non-healing wounds that represent an important cause of morbidity and public cost in western countries. Because of their common recurrence after conventional managements and increasing prevalence due to an ageing population, newer approaches are needed. Over the last decade, the research has been focused on innovative treatment strategies, including stem-cell-based therapies. After the initial interest in embryonic pluripotent cells, several different types of adult stem cells have been studied because of ethical issues. Specific types of adult stem cells have shown a high potentiality in tissue healing, in both in vitro and in vivo studies. Aim of this review is to clearly report the newest insights on tissue regeneration medicine, with particular regard for chronic leg ulcers.

Summary: This study compared the risk and determinants of end-stage kidney disease (ESKD) between kidney transplant recipients (KTR) and patients with native chronic kidney disease (CKD). The results showed that the risk of ESKD in KTR was lower in the first 5 years and higher later on compared to CKD patients. Traditional determinants of ESKD accounted for one-third of the variability of time-to-graft failure.

Results of New Dual-Drug Coated Balloon Angioplasty versus POBA for Femoropopliteal Lesions

The results showed that patients treated with the DCB exhibited lower restenosis/reocclusion rate and improved primary patency compared to those treated with plain old balloon angioplasty (POBA).

Molecular Determinants of Chronic Venous Disease: A Comprehensive Review

Summary: Chronic Venous Disease (CVD) is a complex and multifactorial condition involving genetic, proteomic, and cellular mechanisms that result in structural and functional changes to the veins of lower limbs. The expression of certain genes and alterations in the extracellular matrix play a role in the susceptibility to CVD. This article provides an up-to-date overview of the molecular determinants of CVD, including genetic and hormonal influences, ECM imbalance, histopathology, and the role of endothelial dysfunction.

Association between NLRP3 rs10754558 and CARD8 rs2043211 Variants and Susceptibility to Chronic Kidney Disease

Summary: In this study, the association of functional variants of NLRP3 pathway-related genes (NLRP3-rs10754558, CARD8-rs2043211) with CKD susceptibility was investigated for the first time. Logistic regression analyses revealed significantly higher frequencies of the G allele of NLRP3 variant (67.3%) and the T allele of CARD8 variant (70.8%) among cases compared to controls (35.9% and 31.2%, respectively). The results suggest that NLRP3 rs10754558 and CARD8 rs2043211 variants may be associated with susceptibility to CKD.

Metalloproteinases and Hypertrophic Cardiomyopathy: A Systematic Review

Summary: Hypertrophic cardiomyopathy (HCM) is a genetic condition characterized by altered collagen turnover of the extracellular matrix. Matrix metalloproteinases (MMPs) and their inhibitors (TIMPs) are abnormally released in patients with HCM. This systematic review found that MMP levels, particularly MMP2, were higher in HCM patients compared to healthy subjects. MMPs can be used as biomarkers for HCM and for monitoring treatment outcomes.

Social Determinants of Health and Vascular Diseases: A Systematic Review and Call for Action

Summary: Several factors, known as social determinants of health (SDHs), including gender, SES, and ethnicity, contribute to health inequality. This study analyzed inequality in vascular diseases (VDs) such as PAD, CVD, AAA, and CS. Through a systematic review, the study provided a comprehensive overview of the evidence on SDHs in VDs. Findings showed that gender, SES, and ethnicity play important roles in VDs, highlighting specific issues in this context. A model based on action items for SDHs associated with VDs was proposed.

Infection of Vascular Prostheses: A Comprehensive Review

Summary: This review provides the most updated evidence on the pathogenesis, prevention, diagnosis, and treatment of vascular graft or endograft infection. Multiple microorganisms can cause infections, with Staphylococcus aureus being the most common. Diagnosis requires a combination of clinical, surgical, radiologic, and laboratory criteria. Surgery and antimicrobial therapy are the main treatments, with alternative options available for patients who cannot undergo surgery.

A Systematic Review on the Role of Matrix Metalloproteinases in the Pathogenesis of Inguinal Hernias

Summary: This study aimed to investigate the relationship between extracellular matrix disorders and abdominal wall hernias (AWHs) and the levels of matrix metalloproteinases (MMPs). The results showed that AWHs may be a disease of the extracellular matrix, supported by increased levels of MMPs in patients with AWHs. The study also highlighted the impact of tissue inhibitors of metalloproteinases (TIMPs) on AWHs. More prospective studies are needed to confirm the relationship between these biomarkers and the risk of AWHs.

Summary: This multicenter study evaluated the effectiveness and safety of endovascular treatment of visceral pseudoaneurysms using NALEAs. The results showed that both ruptured and unruptured VAPs can be effectively and safely treated with NALEA-based endovascular embolization. The use of NALEAs is especially recommended in specific clinical cases with coagulopathy, fragile vessels, and challenging embolization targets.

Summary: KTS is a rare genetic disease characterized by venous and capillary malformations and lymphatic malformation. This article describes a case where a severe skin wound in a KTS patient was unresponsive to common treatment but showed improvement with a combination therapy of oxygen-ozone therapy, pulsed magnetic fields, and topical medication. This suggests a potential multistep approach for treating rare and severe diseases.
