A low FODMAP diet is associated with changes in the microbiota and reduction in breath hydrogen but not colonic volume in healthy subjects

Background & aims Ingestion of poorly digested, fermentable carbohydrates (fermentable oligo-, di-, mono-saccharides and polyols; FODMAPs) have been implicated in exacerbating intestinal symptoms and the reduction of intake with symptom alleviation. Restricting FODMAP intake is believed to relieve colonic distension by reducing colonic fermentation but this has not been previously directly assessed. We performed a randomised controlled trial comparing the effect of a low FODMAP diet combined with either maltodextrin or oligofructose on colonic contents, metabolites and microbiota. Methods A parallel randomised controlled trial in healthy adults (n = 37). All subjects followed a low FODMAP diet for a week and supplemented their diet with either maltodextrin (MD) or oligofructose (OF) 7g twice daily. Fasted assessments performed pre- and post-diet included MRI to assess colonic volume, breath testing for hydrogen and methane, and stool collection for microbiota analysis. Results The low FODMAP diet was associated with a reduction in Bifidobacterium and breath hydrogen, which was reversed by oligofructose supplementation. The difference in breath hydrogen between groups post-intervention was 27ppm (95% CI 7 to 50, P<0.01). Colonic volume increased significantly from baseline in both groups (OF increased 110ml (19.6%), 95% CI 30ml to 190ml, P = 0.01; MD increased 90ml (15.5%), 95% CI 6ml to 175ml, P = 0.04) with no significant difference between them. Colonic volumes correlated with total breath hydrogen + methane. A divergence in Clostridiales abundance was observed with increased abundance of Ruminococcaceae in the maltodextrin group, while in the oligofructose group, Lachnospiraceae decreased. Subjects in either group with high methane production also tended to have high microbial diversity, high colonic volume and greater abundance of methanogens. Conclusion A low FODMAP diet reduces total bacterial count and gas production with little effect on colonic volume.

Summary: This study investigated the changes in intestinal microbiota following gastroenteritis caused by a single bacterium. The results showed that patients with persistent bowel dysfunction had lower diversity and altered composition of the microbiota compared to those who recovered. The initial infection had a greater impact on the microbiota of patients with persistent bowel dysfunction. High fiber diets may help alleviate some of the disturbances associated with persistent bowel dysfunction.

The study also revealed that adolescents with high PLEs were 5.6 times more likely to develop psychosis by age 23.

Summary: In this study, researchers identified a shared spatial pattern of cortical thickness (CT) in normative development and several psychiatric and neurological disorders. Further analysis revealed significant spatial correspondences between this pattern and widespread lower CT observed in psychiatric disorders, as well as the spatial pattern of normative maturation and aging. Transcriptional analysis also identified a set of genes closely related to this pattern, indicating disrupted neurodevelopment in the pathogenesis of psychiatric diseases emerging during adolescence.

Summary: Recent studies have found MRI correlates of prospective anxiety symptoms in youth, but their predictive value is still uncertain. This study used machine-learning algorithms to predict clinical anxiety using MRI data and psychometric scores. The results showed that psychometric features were the main predictors for overall anxiety disorders, while MRI regional volumes were more valuable for predicting GAD. In conclusion, future clinical anxiety in non-anxious 14-year-old adolescents can be individually predicted using psychometric features, and including neuroanatomical data improves the prediction of GAD.

Fecal microbiota transplantation for the maintenance of remission in patients with ulcerative colitis: A randomized controlled trial

Summary: This study investigated the efficacy of fecal microbial transplantation (FMT) in maintaining remission in patients with ulcerative colitis (UC). The results showed that there was no difference in the number of relapses between the FMT group and the placebo group during the 12-month follow-up. Therefore, the use of a single-dose FMT for maintaining remission in UC is not supported by these results.

Advantages of using biologically generated 13C-labelled multiple internal standards for stable isotope-assisted LC-MS-based lipidomics

Summary: In comprehensive lipidomics studies, accurate quantification is hindered by unwanted variations such as lipid degradation, matrix effects, and non-linear responses of instruments. This study employed an in vivo labelling strategy to generate C-13-labelled internal standards (C-13-ISs) from different species for more accurate and quantitative lipidomics analysis. The use of C-13-labelled P. pastoris extract as internal standards resulted in significant reduction in lipid variations compared to other normalization methods. This study highlights the benefit of in vivo labelling strategy in reducing technical and analytical variations in lipidomics studies.
