Summary: The purpose of this study is to determine the optimal threshold hemoglobin and hematocrit values for transfusing hip fracture patients. The study found that patients transfused at Hgb values of 7.55 to 7.85 g/dL or Hct values of 22.7% to 23.8% were most likely to achieve good outcomes.

Summary: The study found that the use of tranexamic acid in total hip and knee arthroplasties does not increase the risk of complications in patients with preexisting comorbidities. Tranexamic acid was associated with a significant reduction in the need for blood transfusions, and it was equally effective in high-risk patients.

Summary: This study used big data analysis to evaluate the risk factors and the relationship between blood transfusions and postoperative delirium (POD) in hip fracture surgery. The results showed that factors such as age, anesthesia status, dementia, and medication use were associated with POD, and postoperative transfusion significantly increased the incidence of POD.

Summary: A nomogram model was developed based on independent risk factors for blood transfusion after hip fracture surgery. Preoperative intervention can effectively reduce the incidence of blood transfusion after hip fracture operations. Internal validation proved the nomogram model's adequacy and accuracy, showing that the predicted value agreed well with the actual values.

Summary: Pediatric spinal surgery is a blood-intensive procedure, and it is important to identify the risk factors for transfusions in order to implement a rational blood management program. Analysis of a national database from January 2015 to July 2017 revealed that fusing four or more levels and spinal deformity were the most significant risk factors for transfusions. A new patient blood management program is necessary to improve the transfusion rate in pediatric spinal surgery.

Summary: The study revealed that among injured patients requiring surgical intervention at a tertiary hospital in Ghana, inappropriate blood transfusion practices were observed in 31%-39% of cases. The findings suggest a need for clearer guidelines on blood transfusion in Ghana and similar low- and middle-income countries to prevent inappropriate use of blood as a scarce resource.

Summary: This study aimed to investigate the association between autotransfusion of salvaged blood with single leukoreduction and post-transplant tumor recurrence in patients with advanced hepatocellular carcinoma (HCC). The results showed that autotransfusion was not significantly related to HCC recurrence when matched with allogeneic transfusion, and autotransfusion may have potential benefits in patients with advanced HCC.

Summary: This study analyzed data from 37 German air rescue stations between 2015 and 2020 to assess the incidence of massive hemorrhage after trauma and the potential need for prehospital blood transfusions. The study found that although the incidence of hemorrhagic trauma patients is low, the prehospital administration of blood products may serve as a life-saving bridging treatment.

Summary: In older age, the impact of a more inactive lifestyle on cognitive decline was shown to be limited, regardless of depression. Only decline in working memory was significantly worse in depressed individuals compared to non-depressed ones. Therefore, higher levels of physical activity at older age should still be encouraged as it has positive effects on various other health outcomes.

Summary: In older adults, negative emotions rather than indifference and diminished goal-directed activity appear to underlie underestimation of one's physical capacity. Depression severity was negatively associated with discordance-scores at baseline, while apathy was not. Further research is needed to determine whether targeting discordance results in actual preservation of physical functioning and if older persons with apathy and/or depression need different approaches.

Summary: This systematic literature review provides an overview of barriers and facilitators for the acceptance of medication-related CDSS among clinicians, categorized within the HOT-fit model. The most frequently reported factors relate to the usefulness and relevance of information, as well as the ease of use and efficacy of the system. These results can serve as a foundation for future studies in developing medication-related CDSS.

Summary: Hyperkyphosis, an increased kyphosis angle of the thoracic spine, was associated with a higher fall incidence in the oldest quartile of a large prospective cohort of community-dwelling older adults. This suggests that hyperkyphosis may be a contributing factor to falls, especially in the oldest age group.

Summary: This scoping review aimed to summarize approaches and outcomes of clinical validation studies of CDSSs, with most studies focusing on detection of adverse drug events, potentially inappropriate medications, and drug-related problems. Heterogeneity was found in the methods used and outcome measures, emphasizing the need for a more systematic and comprehensive validation strategy in future research to improve the effectiveness and uptake of CDSSs.

Summary: This study developed prediction models for falls and recurrent falls in community-dwelling older adults using a large sample and considering various predictors, including medication use. The models showed positive predictors such as educational status, functional limitations, and falls history, and negative predictors such as body mass index and grip strength. The model for recurrent falls performed favorably.

Summary: Falls and accompanying injuries are increasingly common worldwide, posing a critical global challenge. The recently published World Falls Guideline for Prevention and Management for Older Adults provides a wealth of evidence-based interventions. The core of falls prevention includes risk assessment, optimizing physical function and mobility, and offering holistic interventions to high-risk older adults.

Summary: This study investigated the associations between adherence to a plant-based diet and cognitive ageing. The results showed that overall adherence to a plant-based diet was not associated with cognitive function or cognitive change. However, fish consumption was found to influence the association between adherence to a plant-based diet and cognitive functioning.

Summary: Preventing and managing falls in older adults is a global challenge, and medication use is a key risk factor. The recent World Guidelines for Falls Prevention and Management recommend assessing fall history and risk before prescribing fall-risk-increasing drugs, using a screening tool for medication review, including deprescribing as part of falls prevention, and prioritizing deprescribing in long-term care. This paper outlines how to implement these recommendations in clinical practice and emphasizes the importance of personalized and patient-centered assessment.
