Socioeconomic Differentials in the Immediate Mortality Effects of the National Irish Smoking Ban

Background: Consistent evidence has demonstrated that smoking ban policies save lives, but impacts on health inequalities are uncertain as few studies have assessed post-ban effects by socioeconomic status (SES) and findings have been inconsistent. The aim of this study was to assess the effects of the national Irish smoking ban on ischemic heart disease (IHD), stroke, and chronic obstructive pulmonary disease (COPD) mortality by discrete and composite SES indicators to determine impacts on inequalities. Methods: Census data were used to assign frequencies of structural and material SES indicators to 34 local authorities across Ireland with a 2000-2010 study period. Discrete indicators were jointly analysed through principal component analysis to generate a composite index, with sensitivity analyses conducted by varying the included indicators. Poisson regression with interrupted time-series analysis was conducted to examine monthly age and gender-standardised mortality rates in the Irish population, ages >= 35 years, stratified by tertiles of SES indicators. All models were adjusted for time trend, season, influenza, and smoking prevalence. Results: Post-ban mortality reductions by structural SES indicators were concentrated in the most deprived tertile for all causes of death, while reductions by material SES indicators were more equitable across SES tertiles. The composite indices mirrored the results of the discrete indicators, demonstrating that post-ban mortality decreases were either greater or similar in the most deprived when compared to the least deprived for all causes of death. Conclusions: Overall findings indicated that the national Irish smoking ban reduced inequalities in smoking-related mortality. Due to the higher rates of smoking-related mortality in the most deprived group, even equitable reductions across SES tertiles resulted in decreases in inequalities. The choice of SES indicator was influential in the measurement of effects, underscoring that a differentiated analytical approach aided in understanding the complexities in which structural and material factors influence mortality.

Summary: Assessing the burden of tuberculosis by sex is crucial. There are significant differences in mortality and incidence rates between males and females among HIV-negative individuals globally, with higher rates among males. Among HIV-positive individuals, females have a higher mortality and incidence rates. Controlling risk factors such as smoking, alcohol use, and diabetes is important in reducing tuberculosis mortality among males.

Smoking cessation Through Optimization of clinical care in Pregnancy: the STOP pragmatic randomized controlled trial

Summary: This study tested the efficacy of the smoking cessation Through Optimization of clinical care in Pregnancy (STOP) clinic as a pathway for smoking cessation in pregnancy. The results showed that women attending the STOP clinic were more likely to quit smoking and smoked fewer cigarettes daily at the time of delivery. However, the intervention did not alter postpartum relapse rates.

E-cigarettes and smoking in Irish teens: a logistic regression analysis of current (past 30-day) use of e-cigarettes

Summary: The study found that adolescent e-cigarette use is influenced by multiple social factors. Male, younger, sport-playing, and higher-educated adolescents are more likely to use e-cigarettes. Tobacco Use Among People Incarcerated in Western Europe: A Systematic Review and Meta-analysis

Global, regional, and national burden of diseases and injuries for adults 70 years and older: systematic analysis for the Global Burden of Disease 2019 Study

Summary: Based on the data analysis of GBD 2019, the global population of older adults has increased since 1990, and overall mortality rates have decreased for both men and women. However, mortality rates due to falls have increased during the same period. The probability of death among people aged 70-90 has decreased, mainly due to reductions in non-communicable diseases. Globally, disability burden is mainly driven by functional decline, vision and hearing loss, and symptoms of pain. Life expectancy at age 70 (LE-70) and healthy life expectancy at age 70 (HALE-70) have shown continuous increases since 1990, with regional disparities. Higher LE-70 results in higher HALE-70 and slightly increased proportion of years in ill health at age 70 (PYIH-70). Sociodemographic factors, healthcare access and quality indices are positively correlated with HALE-70 and LE-70. Limited data are available for various dietary, environmental or occupational, and metabolic risks.
