It is increasingly accepted that chronic inflammation participates in obesity-induced insulin resistance and type 2 diabetes (T2D). Salicylates and thiazolidinediones (TZDs) both have anti-inflammatory and anti-hyperglycemic properties. The present study compared the effects of these drugs on obesity-induced inflammation in adipose tissue (AT) and AT macrophages (ATMs), as well as the metabolic and immunological phenotypes of the animal models. Both drugs improved high fat diet (HFD)-induced insulin resistance. However, salicylates did not affect AT and ATM inflammation, whereas Pioglitazone improved these parameters. Interestingly, HFD and the drug treatments all modulated systemic inflammation as assessed by changes in circulating immune cell numbers and activation states. HFD increased the numbers of circulating white blood cells, neutrophils, and a pro-inflammatory monocyte subpopulation (Ly6C(hi)), whereas salicylates and Pioglitazone normalized these cell numbers. The drug treatments also decreased circulating lymphocyte numbers. These data suggest that obesity induces systemic inflammation by regulating circulating immune cell phenotypes and that antidiabetic interventions suppress systemic inflammation by normalizing circulating immune phenotypes.

Summary: Increased concentrations of serum TNFRs are positively associated with ACR and negatively associated with eGFR in patients with type 2 diabetes. The study shows that serum TNFRs are more strongly correlated with kidney measures, while urinary TNFRs are primarily associated with ACR.

Summary: This study compared the effects of patient-led and physician-led insulin titration in Japanese insulin-naive T2DM patients. It found no significant difference in emotional distress and treatment satisfaction between the two groups, but higher self-efficacy scores in the patient-led group. In terms of glycemic control, the patient-led group showed significantly larger decreases in HbA1c and fasting plasma glucose levels.

Summary: Residual pancreatic endocrine function is crucial for metabolic status after pancreatectomy. This study investigated the changes in pancreatic glucagon secretion and found that fasting plasma glucagon levels decreased after pancreatectomy, while postprandial glucagon responses were maintained. The difference in postprandial glucagon response between pancreaticoduodenectomy and distal pancreatectomy suggests its involvement in postoperative nutritional status.

Role of obesity-induced inflammation in the development of insulin resistance and type 2 diabetes: history of the research and remaining questions

Summary: Obesity-induced chronic inflammation is closely linked to the development of type 2 diabetes, primarily regulated by adipose tissue-resident immune cells. Research areas include factors inducing adipose tissue macrophage inflammation, effector functions promoting insulin resistance by adipose tissue immune cells, and the potential targeting of early immunological events to halt the progression of insulin resistance.

Summary: The study comprehensively investigated the effects of ipragliflozin in different conditioned type 2 diabetes patients, showing significant improvements in glycemic control and metabolic changes. Ipragliflozin demonstrated contrastive effects on various clinical parameters compared to DPP-4i, with better clinical outcomes when co-administered with a DPP-4i.
