Circulating CD4+CD161+T Lymphocytes Are Increased in Seropositive Arthralgia Patients but Decreased in Patients with Newly Diagnosed Rheumatoid Arthritis

Improved understanding of the immune events discriminating between seropositive arthralgia and clinical synovitis is of key importance in rheumatology research. Ample evidence suggests a role for Th17 cells in rheumatoid arthritis. We hypothesized that CD4+CD161+ cells representing Th17 lineage cells may be modulated prior to or after development of clinical synovitis. Therefore, in a cross-sectional study, we investigated the occurrence of CD4+CD161+ T-cells in seropositive arthralgia patients who are at risk for developing rheumatoid arthritis and in newly diagnosed rheumatoid arthritis patients. In a prospective study, we evaluated the effect of methotrexate treatment on circulating CD4+CD161+ T-cells. Next, we assessed if these cells can be detected at the level of the RA joints. Precursor Th17 lineage cells bearing CD161 were found to be increased in seropositive arthralgia patients. In contrast, circulating CD4+CD161+ T-cells were decreased in newly diagnosed rheumatoid arthritis patients. The decrease in CD4+CD161+ T-cells correlated inversely with C-reactive protein and with the 66 swollen joint count. Methotrexate treatment led to normalization of CD4+CD161+ T-cells and reduced disease activity. CD4+CD161+ T cells were readily detected in synovial tissues from both early and late-stage rheumatoid arthritis. In addition, synovial fluid from late-stage disease was found to be enriched for CD4+CD161+ T-cells. Notably, synovial fluid accumulated CD4+CD161+ T-cells showed skewing towards the Th1 phenotype as evidenced by increased interferon-gamma expression. The changes in peripheral numbers of CD4+CD161+ T-cells in seropositive arthralgia and early rheumatoid arthritis and the enrichment of these cells at the level of the joint predict a role for CD4+CD161+ T-cells in the early immune events leading to clinical synovitis. Our findings may add to the development of RA prediction models and provide opportunities for early intervention.

Summary: Arthritis and other rheumatic disorders are common and burdensome conditions. Clinical evaluation alone may not be sufficient, and additional imaging techniques can provide better assessment, prognostic information, treatment evaluation, and diagnosis. Radiological techniques, functional and molecular imaging, including FDG-PET/CT, are valuable tools for these purposes. Alternative tracers are also being developed and applied in clinical trials. It is worth mentioning that 18F-fluoride is more effective than FDG in AS.

Summary: This study aimed to develop an OMERACT ultrasonography score for monitoring disease activity in giant cell arteritis (GCA) and evaluate its metric properties. The study followed the OMERACT Instrument Selection Algorithm and conducted Delphi rounds and online reliability exercise. The results showed that the OGUS had good reliability and sensitivity in assessing disease activity in GCA and could be used in clinical trials.

Monocyte subsets and monocyte-related chemokines in Takayasu arteritis

Summary: This study found that individuals with Takayasu arteritis (TAK) showed abnormal levels of monocyte subsets and related chemokines. Active TAK patients had higher monocyte counts, while glucocorticoid treatment affected the proportion of monocyte subsets. CCL22 was found to have the strongest association with disease activity in TAK.

Positron Emission Tomography Imaging in Vasculitis

Summary: FDG-PET/CT is a valuable diagnostic tool for identifying inflammation in large and medium-sized blood vessels in patients with systemic vasculitides. It can complement other vascular imaging techniques such as ultrasound, CTA, and MRA. In addition, FDG-PET/CT plays an important role in monitoring the treatment of vasculitis patients. The introduction of new immune-cell targeted radiotracers may enable direct visualization of inflammatory cell infiltrates in the vasculature of vasculitis patients.

Summary: This study aimed to provide a new treatment strategy for GPA by modulating the activity of CD4(+)T(EM) cells using the specific peptide inhibitor, ShK-186. The results showed that ShK-186 reduced the expression levels of IFN gamma, TNF alpha, IL-4, IL-17, and IL-21 in CD4(+)T(H) cells from patients with GPA.

Summary: The study investigates the activity of interferon type I (IFN-I) in CD8+ T cells of giant cell arteritis (GCA) patients. The results indicate increased expression of pSTAT1 in CD8+ T cells of GCA patients, while pSTAT3 and pSTAT5 expression did not differ. Additionally, MxA and CD8+ T cells were found to co-localize in artery tissues and biopsy samples of GCA patients. These findings suggest the potential of IFN-I induced biomarkers and novel therapeutic options in GCA.
