A Lower Level of Forced Expiratory Volume in 1 Second Is a Risk Factor for All-Cause and Cardiovascular Mortality in a Japanese Population: The Takahata Study

Chronic obstructive pulmonary disease is a known risk factor for cardiovascular death in Western countries. Because Japan has a low cardiovascular death rate, the association between a lower level of forced expiratory volume in 1 s (FEV1) and mortality in Japan's general population is unknown. To clarify this, we conducted a community-based longitudinal study. This study included 3253 subjects, who received spirometry from 2004 to 2006 in Takahata, with a 7-year follow-up. The causes of death were assessed on the basis of the death certificate. In 338 subjects, airflow obstruction was observed by spirometry. A total of 127 subjects died. Cardiovascular death was the second highest cause of death in this population. The pulmonary functions of the deceased subjects were significantly lower than those of the subjects who were alive at the end of follow-up. The relative risk of death by all causes, respiratory failure, lung cancer, and cardiovascular disease was significantly increased with airflow obstruction. The Kaplan-Meier analysis showed that all-cause and cardiovascular mortality significantly increased with a worsening severity of airflow obstruction. After adjusting for possible factors that could influence prognosis, a Cox proportional hazard model analysis revealed that a lower level of FEV1 was an independent risk factor for all-cause and cardiovascular mortality (per 10% increase; hazard ratio [HR], 0.89; 95% confidence interval [CI], 0.82-0.98; and HR, 0.72; 95% CI, 0.61-0.86, respectively). In conclusion, airflow obstruction is an independent risk factor for all-cause and cardiovascular death in the Japanese general population. Spirometry might be a useful test to evaluate the risk of cardiovascular death and detect the risk of respiratory death by lung cancer or respiratory failure in healthy Japanese individuals.

However, antiviral treatment for HCV showed recovery of nerve fiber ratios along with improvement of liver fibrosis.

Treatment algorithm for thrombocytopenia in patients with chronic liver disease undergoing planned invasive procedures

Summary: Thrombocytopenia is common in patients with chronic liver disease, who often require invasive procedures with a risk of bleeding. Platelet transfusion and thrombopoietin receptor agonists (TPORAs) are two options to increase platelet counts in these patients. Treatment algorithms in different countries vary, with the choice of treatment mainly dependent on the physician's judgment and experience. A more guidance-oriented treatment algorithm is needed for optimal decision making in selecting TPORA therapy or platelet transfusion based on the latest evidence and clinical practice.

Epidemiology and surveillance of autoimmune hepatitis in Asia

Summary: Autoimmune hepatitis (AIH) is a chronic liver disease that mainly affects hepatocytes. Recent epidemiological studies in Asia have revealed regional characteristics of AIH, with type 1 being predominant in East Asia and type 2 being as frequent as in Europe and the USA in South Asia. The prevalence of AIH is increasing worldwide, and early diagnosis and treatment can improve the situation in Asia.

Summary: This study aimed to investigate the influence of nutrient composition on the association between gut microbiota and type 2 diabetes. The results showed differences in gut microbiota composition between diabetic and non-diabetic subjects and its correlation with energy, carbohydrates, and fiber consumed. Cluster analysis revealed a negative association between a Blautia-dominant gut microbiota and diabetes, while a Bifidobacterium-dominant gut microbiota was positively associated with diabetes, suggesting the involvement of nutrient factors in the gut microbiota-diabetes association.

Two Distinct Groups Are Shown to Be at Risk of Diabetes by Means of a Cluster Analysis of Four Variables

Summary: Recent attempts to classify adult-onset diabetes into different clusters showed that it can be categorized into five clusters, with four corresponding to type 2 diabetes (T2DM). This study aimed to identify risk clusters for incident T2DM among nondiabetic individuals. The analysis found two clusters, one labeled as obese insulin resistant with sufficient compensatory insulin secretion and the other as low insulin secretion, were at higher risk of developing diabetes during the 5-year follow-up period. Assessing an individual's risk of diabetes can be done more effectively based on their cluster membership.

These findings contribute to the development of EoGE practice.

Role of Bile-Derived Extracellular Vesicles in Hepatocellular Proliferation after Partial Hepatectomy in Rats

Summary: Despite extensive study on liver regeneration, the impact of bile-derived extracellular vesicles (bile EVs) on hepatocytes remains unclear. Using a rat model of partial hepatectomy (PH), this study found that bile EVs released 12 hours after PH significantly promoted hepatocyte proliferation. Transcriptome analysis revealed an upregulation of genes promoting cell cycle progression in hepatocytes treated with PH24-EVs.
