Stage distribution and histology contribute to age-related survival differences, while frailty and/or therapy may explain age differences within stage groups. Further studies on treatment and geriatric conditions are needed to understand the age gap in colon and rectal cancer survival.

Summary: Despite global screening programs, colorectal cancer (CRC) remains a leading cause of cancer-related deaths worldwide. Locally advanced colon cancer (LACC) presents surgical challenges, and the advantages and disadvantages of preoperative radiotherapy for LACC are still undetermined. While several reliable biomarkers have been proposed for CRC prediction and prognosis, few studies have focused on the treatment of LACC. This comprehensive review highlights the role of predictive biomarkers in the treatment and postoperative oncological outcomes of LACC, as well as discusses the emerging needs and approaches for the discovery of biomarkers to facilitate the development of new therapeutic targets and surveillance of LACC patients.
