Increased Postprandial Energy Expenditure May Explain Superior Long Term Weight Loss after Roux-en-Y Gastric Bypass Compared to Vertical Banded Gastroplasty

Background and Aims: Gastric bypass results in greater weight loss than Vertical banded gastroplasty (VBG), but the underlying mechanisms remain unclear. In addition to effects on energy intake the two bariatric techniques may differentially influence energy expenditure (EE). Gastric bypass in rats increases postprandial EE enough to result in elevated EE over 24 hours. This study aimed to investigate alterations in postprandial EE after gastric bypass and VBG in humans. Methods: Fourteen women from a randomized clinical trial between gastric bypass (n = 7) and VBG (n = 7) were included. Nine years postoperatively and at weight stability patients were assessed for body composition and calorie intake. EE was measured using indirect calorimetry in a respiratory chamber over 24 hours and focused on the periods surrounding meals and sleep. Blood samples were analysed for postprandial gut hormone responses. Results: Groups did not differ regarding body composition or food intake either preoperatively or at study visit. Gastric bypass patients had higher EE postprandially (p = 0.018) and over 24 hours (p = 0.048) compared to VBG patients. Postprandial peptide YY (PYY) and glucagon like peptide 1 (GLP-1) levels were higher after gastric bypass (both p<0.001). Conclusions: Gastric bypass patients have greater meal induced EE and total 24 hours EE compared to VBG patients when assessed 9 years postoperatively. Postprandial satiety gut hormone responses were exaggerated after gastric bypass compared to VBG. Long-term weight loss maintenance may require significant changes in several physiological mechanisms which will be important to understand if non-surgical approaches are to mimic the effects of bariatric surgery.

Recent advances in the treatment of patients with obesity and chronic kidney disease

Summary: Obesity is a chronic disease characterized by excess adiposity that negatively impacts health. The high prevalence of obesity increases the risk of long-term medical complications, including type 2 diabetes and chronic kidney disease. Studies have shown that certain medications and surgical procedures, such as sodium-glucose cotransporter 2 inhibitors and bariatric surgery, can have positive effects on diabetic kidney disease. However, more research is needed to effectively treat patients with obesity and chronic kidney disease and reduce morbidity.

βHB inhibits glucose-induced GLP-1 secretion in GLUTag and human jejunal enteroids

Summary: Ingestion of nutrients stimulates the secretion of incretins, such as GLP-1, which regulate postprandial insulin release and satiety. This study investigated the inhibitory effect of beta HB on glucose-induced GLP-1 secretion using cell cultures. The results showed that beta HB significantly inhibited GLP-1 secretion in both murine GLUTag cells and differentiated human jejunal enteroid monolayers. The findings suggest that beta HB may act through multiple downstream mediators to regulate GLP-1 secretion.

Anaemia in patients with self-reported use of iron supplements in the BAriatric surgery SUbstitution and nutrition study: A prospective cohort study

Glucagon-like peptide-1 agonists combined with sodium-glucose cotransporter-2 inhibitors reduce weight in type 1 diabetes

Summary: This study assessed the effects of adding sodium-glucose cotransporter-2 inhibitors (SGLT2i) and/or glucagon-like peptide-1 receptor agonists (GLP1-RA) to insulin on weight and glycemic control in individuals with type 1 diabetes. The results showed that both SGLT2i and GLP1-RA alone improved weight and glycemia, but combining these medications resulted in greater weight loss.

Changes in adipose tissue distribution and relation to cardiometabolic risk factors after Roux-en-Y in adolescents

Summary: This study found that Roux-en-Y gastric bypass (RYGB) surgery in obese adolescents resulted in reductions in various body composition measures, with the greatest reduction observed in visceral adipose tissue (VAT). While other fat depots showed regain from 1 to 5 years post-RYGB, VAT remained lower than baseline values. The changes in VAT were associated with changes in triglycerides and fasting plasma insulin.

Summary: Obesity is now recognized as a disease associated with significant morbidity and mortality. Weight loss has been found to ameliorate the metabolic abnormalities underlying type 2 diabetes and improve glycemic control. Intentional weight loss can benefit patients with type 2 diabetes and obesity by not only improving glycemic control but also reducing cardiometabolic disease risk factors and enhancing well-being.

Obesity Characteristics Are Poor Predictors of Genetic Mutations Associated with Obesity

Summary: The study aimed to identify associations between clinico-demographical characteristics and genetic mutations associated with obesity. The results revealed that gene mutations associated with obesity are common among patients with a BMI > 40 kg/m(2). However, a patient's BMI, age of onset of obesity, or age of onset of hyperphagia did not provide valuable information in predicting the presence of genetic mutations associated with obesity.

Summary: Bariatric surgery, specifically Roux-en-Y gastric bypass (RYGB), improves dyslipidaemia and reduces body weight. This study found that RYGB may modulate gene expression in fat cells to influence the expression of the PCSK9 and LDLR genes. Lower PCSK9 gene expression and higher LDLR gene expression were observed in adipose tissue after RYGB in rats. Additionally, TNF alpha gene expression was positively correlated with PCSK9 gene expression after RYGB.
