Brain Injury in Very Preterm Children and Neurosensory and Cognitive Disabilities during Childhood: The EPIPAGE Cohort Study

Objective: To investigate the association of motor and cognitive/learning deficiencies and overall disabilities in very preterm (VPT) children and their relations to gestational age (GA) and brain lesions. Design, Setting, and Participants: EPIPAGE is a longitudinal population-based cohort study of children born before 33 weeks' gestation (WG) in 9 French regions in 1997-1998. Cumulating data from all follow up stages, neurodevelopmental outcomes were available for 90% of the 2480 VPT survivors at 8 years. Main outcomes were association of motor and cognitive deficiencies and existence of at least one deficiency (motor, cognitive, behavioral/ psychiatric, epileptic, visual, and/or hearing deficiencies) in three GA groups (24-26, 27-28, and 29-32WG) and four groups of brain lesions (none, minor, moderate, or severe). Results: VPT had high rates of motor (14%) and cognitive (31%) deficiencies. Only 6% had an isolated motor deficiency, 23% an isolated cognitive one and 8% both types. This rate reached 20% among extremely preterm. Psychiatric disorders and epilepsy were observed in 6% and 2% of children, respectively. The risks of at least one severe or moderate deficiency were 11 and 29%. These risks increased as GA decreased; only 36% of children born extremely preterm had no reported deficiency. Among children with major white matter injury (WMI), deficiency rates reached 71% at 24-26WG, 88% at 2728WG, and 80% at 29-32WG; more than 40% had associated motor and cognitive deficiencies. By contrast, isolated cognitive deficiency was the most frequent problem among children without major lesions. Conclusions: In VPT, the lower the GA, the higher the neurodisability rate. Cerebral palsy is common. Impaired cognitive development is more frequent. Its occurrence in case without WMI or early motor disorders makes long-term follow up necessary. The strong association between motor impairments, when they exist, and later cognitive dysfunction supports the hypothesis of a common origin of these difficulties.

Summary: This study aimed to investigate the association between early life growth and later overweight or obesity. The study found that birth weight, increase in weight during neonatal hospital stay, and increase in BMI between discharge and 2 years of age were positively associated with overweight or obesity at 15 years, while increase in length during neonatal hospital stay was negatively associated. These associations were also confirmed at 5 years, suggesting that BMI at 2 years was already related to later overweight or obesity in very preterm born children.

Public health indicators for cerebral palsy: A European collaborative study of the Surveillance of Cerebral Palsy in Europe network

Summary: This study proposed six public health indicators for cerebral palsy (CP) and used population-based data from European CP registries to estimate the extent of CP, effectiveness of perinatal organization, burden of the condition, access to health services and preventive health strategies. The results showed a decreasing prevalence of CP in both preterm and full-term-born children, and the burden of CP was dependent on subtype and associated impairments. The study demonstrated the importance of CP registries in generating key outcomes for public health policies for children with disabilities.

Impact of Early Hemoglobin Levels on Neurodevelopment Outcomes of Two-Year-Olds in Very Preterm Children

Summary: This study examined the relationship between hemoglobin levels in very preterm infants within the first 24 hours of birth and the neurodevelopment outcomes at 24 months of corrected age. The results showed that early low hemoglobin levels were associated with major neonatal morbidities but not with neurodevelopment outcomes at two years of age, except in male infants born at gestational age of more than 26 weeks.

Bumetanide oral solution for the treatment of children and adolescents with autism spectrum disorder: Results from two randomized phase III studies

The sponsor has discontinued the development of bumetanide for the treatment of pediatric ASD.

Long-term outcomes of paediatric Guillain-Barre syndrome

Early Life Factors Influencing Children Gut Microbiota at 3.5 Years from Two French Birth Cohorts

Summary: Early life gut microbiota-influencing factors are crucial for long-term health, and this study aimed to investigate the associations between 20 early life factors and gut microbiota composition at 3.5 years old. The results showed that gestational age, mode of delivery, and breastfeeding were significantly associated with gut microbiota differences. Having a sibling and attending daycare center were also linked to specific enterotypes. Maternal factors such as country of birth and preconception body mass index influenced the microbiota characteristics of the children.

Summary: The initiation of therapeutic hypothermia within 6 hours of life is crucial for treating neonatal hypoxic ischemic encephalopathy. This study aimed to identify clinical and healthcare organizational factors associated with delayed therapeutic hypothermia in a French population-based cohort. Delayed therapeutic hypothermia was more common in neonates with moderate HIE and in maternity units with lower annual birth volumes. Early identification of neonates who could benefit from therapeutic hypothermia and access to appropriate care before transfer are important factors in improving outcomes for neonates with HIE.
