A Non-Lethal Traumatic/Hemorrhagic Insult Strongly Modulates the Compartment-Specific PAI-1 Response in the Subsequent Polymicrobial Sepsis

Introduction: Plasminogen activator inhibitor 1 (PAI-1) is a key factor in trauma-and sepsis-induced coagulopathy. We examined how trauma-hemorrhage (TH) modulates PAI-1 responses in subsequent cecal ligation and puncture (CLP)-induced sepsis, and the association of PAI-1 with septic outcomes. Methods: Mice underwent TH and CLP 48 h later in three separate experiments. In experiment 1, mice were sacrificed pre-and post-CLP to characterize the trajectory of PAI-1 in plasma (protein) and tissues (mRNA). Post-CLP dynamics in TH-CLP (this study) and CLP-Only mice (prior study) were then compared for modulatory effects of TH. In experiment 2, to relate PAI-1 changes to outcome, mice underwent TH-CLP and were sampled daily and followed for 14 days to compare non-survivors (DEAD) and survivors (SUR). In experiment 3, plasma and tissue PAI-1 expression were compared between mice predicted to die (P-DIE) and to live (P-LIVE). Results: In experiment 1, an early post-TH rise of circulating PAI-1 was contrasted by a delayed (post-TH) decrease of PAI-1 mRNA in organs. In the post-CLP phase, profiles of circulating PAI-1 were similar between TH-CLP and CLP-Only mice. Conversely, PAI-1 mRNA declined in the liver and heart of TH-CLP mice versus CLP-Only. In experiment 2, there were no DEAD/SUR differences in circulating PAI-1 prior to CLP. Post-CLP, circulating PAI-1 in DEAD was 2-4-fold higher than in SUR. PAI-1 increase heralded septic deaths up to 48 h prior but DEAD/SUR thrombomodulin (endothelial injury marker) levels were identical. In experiment 3, levels of circulating PAI-1 and its hepatic gene expression were higher in P-DIE versus P-LIVE mice and those increases closely correlated with liver dysfunction. Conclusions: Trauma modulated septic PAI-1 responses in a compartment-specific fashion. Only post-CLP increases in circulating PAI-1 predicted septic outcomes. In posttraumatic sepsis, pre-lethal release of PAI-1 was mostly of hepatic origin and was independent of endothelial injury.

Cellular uptake of modified mRNA occurs via caveolae-mediated endocytosis, yielding high protein expression in slow-dividing cells

Summary: Nucleic acids have great potential for gene therapy, with plasmid DNA as the first therapeutic nucleic acid and mRNA as a safer and more affordable option. This study investigated the uptake mechanisms and efficiencies of genetic material by cells, focusing on different nucleic acids, delivery vectors, and human primary cells. The study found that caveolae-mediated uptake was the main route for gene delivery, pDNA had higher expression levels in fast-dividing fibroblasts, cmRNA was responsible for high protein production in slow-dividing osteoblasts, and in mesenchymal stem cells, the combination of vector and nucleic acid was more relevant. Protein expression was higher when cells were seeded on 3D scaffolds.

From Gut to Blood: Spatial and Temporal Pathobiome Dynamics during Acute Abdominal Murine Sepsis

Summary: Abdominal sepsis leads to the migration of microorganisms from the gut to the peritoneum and bloodstream, but there are limited methods and biomarkers to study and monitor this process. In this study, cecal ligation and puncture (CLP) were used to induce abdominal sepsis in mice, and NGS was employed to analyze the microbial species composition in fecal, peritoneal lavage, and blood samples. The results showed rapid changes in gut microbial communities after CLP, with pathogenic species detected in the peritoneum and blood as early as 24 hours post-CLP. NGS of cfDNA from blood samples as small as 30 microliters was able to accurately identify pathogenic species in a time-dependent manner.

Surgical suction filter-derived bone graft displays osteogenic miRNA and mRNA patterns

Summary: Recently, a surgical suction filter device was introduced to generate a suction filter-derived bone grafting substitute (SF-BGS). This study aimed to investigate the quality of the SF-BGS by determining the expression of miRNAs and osteogenic mRNAs. The results showed distinct differences in miRNA expression and overall upregulation of osteogenic marker genes in SF-BGS.

MiRNAs as Potential Regulators of Enthesis Healing: Findings in a Rodent Injury Model

Summary: In this study, the miRNA expression profile in enthesis tissue following injury was investigated. Target prediction and mRNA expression analysis suggest that certain miRNAs may regulate the expression of mRNA targets relevant for enthesis healing. Additionally, the protein expression levels of collagens I, II, III, and X were found to change after injury.

Circulating miRNA expression in extracellular vesicles is associated with specific injuries after multiple trauma and surgical invasiveness

Summary: This study investigates the effects of Early Total Care (ETC) and Damage Control Orthopedics (DCO) on microRNA (miRNA) expression in extracellular vesicles (EVs) and identifies their mRNA targets. The results show that the miRNA expression patterns in the ETC group are more pro-inflammatory, indicating potentially decreased tissue regeneration capabilities. In silico target prediction reveals mRNA targets related to inflammation, fibrosis, and Wnt signaling. Validation of these targets in lung tissue shows treatment-specific deregulations that match the expression of their upstream miRNAs, while no significant deregulations were observed in the liver.

Summary: Low-dose irradiation is an effective treatment option for persistent lymphatic fistulas after inguinal vascular surgery, preventing infection, sepsis, bleeding, and possible leg amputation. The recommended dose ranges from 0.3-0.5 to 1-12 Gy. In this study, every patient experienced resolution of the lymphatic fistula without major complications.

Computational evidence for multi-layer crosstalk between the cadherin-11 and PDGFR pathways

Summary: Various cell surface receptors, including cadherin-11 and platelet-derived growth factor receptors (PDGFRs), are involved in the differentiation and self-renewal of human mesenchymal stem cells (hMSCs). This study used a computational model to investigate the interactions between cadherin-11 and PDGFRs, and revealed the existence of crosstalk at both receptor and downstream signaling levels. The findings contribute to a better understanding of the effect of cell surface receptors on hMSCs proliferation.

Activated Carbon for Sepsis Prevention and Intervention: A Modern Way of Utilizing Old Therapies

Summary: This article introduces the application of activated carbon or charcoal in alleviating inflammation and explains its mechanisms of protecting the microbiome and bone marrow, as well as its anti-inflammatory and anti-oxidant effects. Early application of activated carbon and its high adsorption capacity are essential in the treatment of preventing sepsis and multi-organ failure.
