Summary: This study retrospectively analyzed 3,013 ischemic stroke patients and found that neutrophil and white blood cell levels were significantly elevated in large-artery atherosclerosis and cardioembolism subtypes, while lymphocyte levels were higher in small-artery occlusion. The neutrophil-lymphocyte ratio and CRP levels were identified as the best independent predictors for stroke outcomes.

Summary: This study found that patients with community-acquired pneumonia (CAP) had higher baseline white cell counts and C-reactive protein levels, as well as a greater decrease in CRP upon hospital admission compared to COVID-19 patients. A multivariate logistic regression model incorporating WCC and CRP changes could distinguish bacterial co-infection and differentiate between CAP and COVID-19.

Summary: This study evaluated serum concentrations of pancreatic stone protein (PSP), copeptin, and apolipoprotein A-V (APOA5) as biomarkers for the diagnosis and prognosis of pediatric sepsis. The results showed that PSP, copeptin, and APOA5 were significantly higher in septic patients compared to healthy controls. PSP and copeptin were associated with non-survival, while APOA5 was inversely related. These biomarkers have potential diagnostic value for pediatric sepsis but are inadequate for predicting mortality.

Summary: The diagnostic test accuracy of inflammatory markers, including CRP and PCT, in identifying and differentiating the severity of DFU has been explored in this systematic review. CRP was found to be the best marker for diagnosing grade 2 DFU, while PCT was the best marker for diagnosing grade 3 DFU. Further research is needed to establish standardized cutoff values for these markers in diagnosing diabetic foot ulcers.

Summary: This study investigated the relationship between routine inflammatory markers and clinical outcomes in acute basilar artery occlusion patients receiving endovascular treatment. The results showed that higher white blood cell count and neutrophil-to-lymphocyte ratio on admission were significantly associated with unfavorable functional outcome and mortality at 90 days.

Summary: This study evaluated the roles of PCT and CRP in febrile cancer patients with solid tumors, identifying cut-off values to distinguish between infection and cancer-associated fever. PCT and CRP/PCT ratio can serve as valuable markers for ruling out infection in these patients.

Summary: This study aimed to identify rapid and accurate early diagnostic indicators for intra-abdominal infection after general surgery. The combination of CRP, PCT, TNF alpha, and IL6 showed higher predictive efficiency for complicated IAIs among surgical patients compared to individual indicators on postoperative days 1 and 3. These composite indicators can serve as predictors with high sensitivity and specificity for postoperative abdominal infectious complications.

Summary: Despite progress in diagnosing and treating maxillofacial infections, the unpredictable course of infection can lead to severe complications, prolonged hospitalization, and high financial costs. This retrospective study analyzed the role of inflammatory markers (CRP, WBC, NLR) in predicting prolonged hospital stays in 108 maxillofacial infection patients. The study confirmed a positive linear correlation (p < 0.001) between the predictors and hospitalization duration, with optimal cut-off values identified for WBC and CRP.

Summary: Infection is a major cause of death in cancer patients, and accurately identifying fever caused by infection is important for avoiding unnecessary treatment. This study evaluated the diagnostic value of various inflammatory markers in suspected infected cancer patients, and found that procalcitonin had the best performance in differentiating infected from uninfected causes of fever.

Summary: This study aimed to develop and validate a prognostic nomogram for predicting sepsis in pyogenic liver abscess (PLA) patients. Independent risk factors for sepsis were identified, and a nomogram was developed based on these factors. The nomogram showed excellent calibration and discrimination, indicating its potential clinical utility.

Summary: The study found that serum PCT and CRP levels were significantly increased in SLE patients with bacterial infections. PCT had better diagnostic performance than CRP, with a higher positive likelihood ratio, making it a good rule-in marker. CRP testing may result in a high false-positive rate due to low positive likelihood ratio.

Summary: This study evaluated the benefit of plasma inflammatory markers for the diagnosis of ventriculostomy-related infection (VRI) in patients with hemorrhagic stroke. The results showed that plasma inflammatory markers were not able to differentiate VRI patients from those without infection, but were higher in patients with non-cerebral infection. Overall, CRP, PCT, and WBC are not suitable parameters for VRI diagnosis in neurocritical care unit patients.

Summary: Community-acquired pneumonia (CAP) has a significant impact on pediatric hospitalizations. The study analyzed the contribution of CAP to hospitalizations, its etiology in relation to age, and the role of inflammatory markers. It found that CAP accounted for a considerable proportion of hospitalizations and patient days, with viral pneumonia being more common than bacterial pneumonia. Serum inflammatory markers can assist in differentiating the causative factors.

Summary: This study aimed to develop clinically tools for early detection of anastomotic leakage (AL) by analyzing inflammatory factors (IFs) in abdominal drainage. The research proposed an Anastomotic Score system (AScore-POD3) based on IFs with high diagnostic efficacy, which was validated in an independent cohort. A nomogram combining AScore-POD3 with other clinical risk factors showed high C-index, suggesting the potential for early diagnosis of AL after gastric cancer surgery.

Summary: Procalcitonin (PCT) may be useful for differentiating between infections and flare in patients suffering from antineutrophil-cytoplasmic-antibody (ANCA)-associated vasculitides (AAV). The study found that PCT levels were significantly higher in the infected group compared to the relapsing group. C-reactive protein (CRP) levels were also higher in cases of infection.

Summary: This study compared the effects of empagliflozin treatment with placebo in outpatients with chronic SIAD-induced hyponatremia. The results showed that empagliflozin treatment significantly increased serum sodium levels and improved neurocognitive function. This suggests that empagliflozin may be a promising treatment option for chronic SIAD-induced hyponatremia.

Summary: Hyponatremia is a common electrolyte disorder, with Syndrome of Inappropriate Antidiuresis (SIAD) being the most frequent cause. This is mediated by the nonosmotic release of arginine vasopressin (AVP), promoting water retention. The underlying causes of SIAD include malignancy, pulmonary pathology, and central nervous system pathology. Fluid restriction is the first-line treatment, but there is controversy regarding second-line therapy.

Summary: This study aimed to investigate the effects of the GLP-1 analogue dulaglutide on smoking cessation and post-cessation weight gain. The results showed that dulaglutide had no effect on abstinence rates but prevented weight gain and decreased HbA1c levels. These findings suggest that GLP-1 analogues may play a role in future cessation therapy targeting metabolic parameters.

Summary: Hyponatremia is a common and poorly understood problem in clinical practice. Its prevalence varies depending on the population and criteria used for diagnosis. Hyponatremia is associated with negative outcomes and its pathogenesis involves the accumulation of electrolyte-free water. Differentiating between etiologies can be done using plasma and urine measurements. The management of hyponatremia depends on symptoms and causes and is discussed in this review.
