Summary: Detection of aspergillus by PCR is a valuable tool for early diagnosis, with high sensitivity, specificity, and negative predictive value. Standardized DNA extraction methods are recommended for all commercial assays, while awaiting conclusive validation data in different clinical settings. This perspective provides guidance for PCR testing and highlights future prospects such as quantification, species-specific identification, and detection of resistance genetic markers.

Summary: This study evaluated the performance of mNGS in BALF in suspected IPA patients and compared it with other detection tests. The results showed that BALF mNGS had higher sensitivity and specificity, as well as better performance. This indicates the important role of BALF mNGS in diagnosing suspected IPA disease.

Summary: This study retrospectively analyzed 669 patients with MM at Rambam Hospital, among whom 42 (6.2%) were diagnosed with IPA. Compared to other pulmonary infections, IPA was more commonly diagnosed in patients with long-standing disease and those receiving 3 or more lines of myeloma therapies. There was no difference in 30-day mortality rates following diagnostic bronchoscopy between IPA and non-IPA patients.

Summary: In this study, it was found that plasma and BALF levels of PTX3 were significantly higher in COPD patients with IPA. The sensitivity of PTX3 for diagnosing IPA in patients with COPD was superior to that of GM, and the combination of GM and PTX3 was useful for the diagnosis of IPA in these patients.

Summary: The study demonstrates that detection of GM in bronchoalveolar lavage fluid is more sensitive for diagnosing invasive pulmonary aspergillosis in non-neutropenic patients compared to serum GM detection. The optimal cutoff value for BALF GM is 0.87, with high sensitivity and specificity.

Summary: Aspergillus fumigatus is a critical priority fungal pathogen identified by the World Health Organization. Commercially available diagnostics for various forms of aspergillosis often rely on fungal metabolites, including intracellular molecules, cell wall components, and extracellular secretomes. This review highlights the limitations of antibody tests compared to tests of fungal products in body fluids, and discusses the application of beta-D-glucan, galactomannan, and pentraxin 3 in bronchoalveolar lavage fluids. It also explores the detection of nucleic acids, next-generation sequencing, and recent studies on Aspergillus metallophores.

Summary: A consensus statement for the diagnosis and management of COVID-19-associated pulmonary mucormycosis (CAPM) was formulated using a modified Delphi method. Recommendations were proposed for defining, diagnosing, and managing CAPM, but further research is needed to fully understand this underdiagnosed entity.

Summary: This article presents a consensus statement for the diagnosis and management of COVID-19-associated pulmonary mucormycosis (CAPM) using a modified Delphi method. The experts reached consensus on key statements related to CAPM, including its classification, diagnosis, and recommended treatments. However, further research is needed in this field.

Summary: The analysis of diagnostic biomarkers in bronchoalveolar lavage fluid is a clinical standard for diagnosing pulmonary aspergillosis. Pentraxin 3 assay alone can distinguish different forms of aspergillosis, while combining Ptx3 and TafC assays enables the diagnostic discrimination of invasive pulmonary aspergillosis and other fungal infections.

Summary: In this study, BALF samples from 342 critically ill patients with pulmonary infections were analyzed. BAP/FAP was identified in approximately 47% of the subjects using PCR-based methods. PCR showed improved detection performance for fungi, but similar results for bacteria compared to traditional microscopy and culture methods.
