Joint Effect of MCP-1 Genotype GG and MMP-1 Genotype 2G/2G Increases the Likelihood of Developing Pulmonary Tuberculosis in BCG-Vaccinated Individuals

We previously reported that the - 2518 MCP-1 genotype GG increases the likelihood of developing tuberculosis (TB) in non-BCG-vaccinated Mexicans and Koreans. Here, we tested the hypothesis that this genotype, alone or together with the 1607 MMP-1 functional polymorphism, increases the likelihood of developing TB in BCG-vaccinated individuals. We conducted population-based case-control studies of BCG-vaccinated individuals in Mexico and Peru that included 193 TB cases and 243 healthy tuberculin-positive controls from Mexico and 701 TB cases and 796 controls from Peru. We also performed immunohistochemistry (IHC) analysis of lymph nodes from carriers of relevant two-locus genotypes and in vitro studies to determine how these variants may operate to increase the risk of developing active disease. We report that a joint effect between the - 2518 MCP-1 genotype GG and the - 1607 MMP-1 genotype 2G/2G consistently increases the odds of developing TB 3.59-fold in Mexicans and 3.9-fold in Peruvians. IHC analysis of lymph nodes indicated that carriers of the two-locus genotype MCP-1 GG MMP-1 2G/2G express the highest levels of both MCP-1 and MMP-1. Carriers of these susceptibility genotypes might be at increased risk of developing TB because they produce high levels of MCP-1, which enhances the induction of MMP-1 production by M. tuberculosis-sonicate antigens to higher levels than in carriers of the other two-locus MCP-1 MMP-1 genotypes studied. This notion was supported by in vitro experiments and luciferase based promoter activity assay. MMP-1 may destabilize granuloma formation and promote tissue damage and disease progression early in the infection. Our findings may foster the development of new and personalized therapeutic approaches targeting MCP-1 and/or MMP-1.

Mechanisms of carbapenemase-mediated resistance among high-risk Pseudomonas aeruginosa lineages in Peru

Neutralizing Antibodies as Predictors of Vaccine Breakthrough Infection in Healthcare Workers Vaccinated with or without a Heterologous Booster Dose: A Cohort Study during the Third COVID-19 Wave in Peru

Summary: We evaluated the levels of neutralizing antibodies (NAbs) as a protective factor against vaccine breakthrough infection (VBI) in healthcare workers (HCWs) during Peru's third COVID-19 wave. This retrospective cohort study used data from a private laboratory in Lima, Peru, and included HCWs who received either two doses of BBIBP-CorV vaccine or a heterologous booster with BNT162b2. We found that HCWs with higher levels of NAbs, measured at various time points after BBIBP-CorV vaccination, had a lower risk of VBI during the third wave of COVID-19.

Humoral Response after a Fourth Dose with mRNA-1273 in Healthcare Workers with and without a History of SARS-CoV-2 Infection and Previously Vaccinated with Two Doses of BBIBP-CorV Plus BNT162b2 Vaccine

Summary: This prospective cohort study examined the humoral response of healthcare workers who received a fourth dose with a heterologous mRNA1273 booster after a primary regimen of two doses of BBIBP-CorV. The results showed that the fourth dose significantly enhanced the immune response, with antibody levels 2.3 and 1.6 times higher at 30 and 120 days post-vaccination, respectively. Furthermore, individuals who received the mRNA1273 booster and those infected after the third dose with BNT162b2 exhibited higher antibody titers.

First Report of OXA-181-Producing Enterobacterales Isolates in Latin America

Summary: We identified five carbapenemase-producing Enterobacterales (CPE) isolates from two healthcare institutions in Lima, Peru. All isolates were carriers of the bla(OXA-48)-like gene and had the bla(OXA-181) gene as the only carbapenemase gene. Other resistance genes and the IncX3 plasmid were also found. This study highlights the increasing public health problem of CPE isolates harboring bla(OXA)-like genes and the wide dissemination of bla(OXA-181) in Peru.

Summary: A case series of non-HIV patients with cerebral cryptococcosis after SARS-CoV-2 infection is reported. The majority of these patients had severe symptoms or received immunosuppressants. There is a hypothesis that the CD4+ T lymphocytopenia in these patients may increase the risk of cryptococcosis after SARS-CoV-2 infection.

Summary: Insufficient data have been reported about the effect of the inactivated SARS-CoV-2 vaccine (BBIBP-CorV) on the humoral response through time in healthcare workers (HCW). This retrospective cohort study compared the antibody-mediated response provoked by BBIBP-CorV between HCW previously infected with SARS-CoV-2 (PI) and not previously infected (NPI). The study found that there was a progressive decrease in antibody levels over time in both groups, with the NPI group showing a higher decrease.
